Discussion 1 Welcome 2 Apologies 3 Review of minutes of 10th March 2016 Matters arising: twitter feed and website technical difficulties Concern expressed regarding organisation of phlebotomy Update of new staff appointments: x 1 receptionist x 1 Doctor VR asked for clarification of visibility to Doctors of Patients with aspergers/autism Discussion ensued re surgery screen usage Bloods and X rays: difficulty for patient to assess where they are in the queue

4 Patient Reference Group: Health Watch Minutes: Feedback Mtg of 6.7.16. X 11 pharmacies in Sutton facing closure Discussion around provider for out of hours service and 111 service – outsourced 3 yr contract across several London Boroughs. Discussion For information only: patients can choose whether their data is shared and may opt out of integrated care records. JW clarified our process for sharing a short excerpt of records with St. Helier Seldoc. Commissioning priorities workshop re new financial year- proposal for Healthwatch and CCG to talk to as many patients as possible to gather views on topics such as: GP Care, Hospital Care, Ugent and emergency care, mental health, young persons, maternity etc. This is a fact finding exercise. It is necessary to determine the method (s) by which the data will be collated. To pre date September 16 Workshop. Tenants Group Meeting to handle facilities type issues. Dr. Roberts is our representative on the CCG. The Patients Reference Group may send up to 2 delegates per mtg. Next date 14th Sept. 4.45pm – 6.45pm at Civic Centre. Attendees may stand for Chair/Vice JD encouraged all to consider attending.

5 Sutton Patient Network: Enhancing the quality of services for long term conditions – care relating to some chronic diseases may be integrated. Need identified for better management of such patients (instance of one patient calling 999 x40 times in a month) Initiative to empower patients to improve their own health. ‘Crisis Cafes’ – aiming to provide contact and support for mental healthcare patients over the weekend. GPs invited 28th Sept. to participate in health,wellbeing and social care network. ‘Social Prescribing Initiative’ NHS Health checks (National initiative) – JW clarified Surgery process for writing and enclosing blood forms for subsequent nurse follow up. Transforming Cancer Services: facilitated session by Macmillan. Piloting of a ‘ cancer tool’ which will support identification of cancer risk. WFP are not a participant in the pilot. 6 The Patients’ Association: DR reported on her attendance at 20th Mental Health Event for Sutton residents. Key themes: Speech and Language Therapy for Children, Continuing Healthcare Assessment, Good access to Services. Cost of nursing home residency £845 per week. Related matters were better communications, preparation for assessment, care home choice and advocacy (relative or a.n other) and separation of complaint processes for residential and domiciliary care. DR has identified 3 possible topic areas for next year’s event: Strokes/speech and language therapy, Neurology and Eye Services. JD expressed to DR her thanks for attendance and feedback.
