WORLD YOUTH CONGRESS 2016 REGISTRATION GUIDELINES

 This is the official World Youth Congress (WYC) 2016 Registration Form. Please do not use any other forms. See separate file for the registration form.

 Only the CFC YFL District Couple Coordinators or his authorized representative may submit / transmit the completed registration form.

 Registration is now OPEN. The World Youth Congress 2016 Registration Fee is PHP 1,200.00.

 The deadline of registration for Core Servants (District Servants, Cluster Servants, Chapter Servants, District Couple Coordinators, and Cluster Couple Coordinators, Chapter Couple Coordinators will be on February 19, 2016; while the deadline of registration for Committed Servants (Household Servants and potential leaders) will be on March 18, 2016.

 Strictly no walk-ins will be allowed.

 For CFC YFL Districts OUTSIDE of Metro Manila only: CFC YFL District Couple coordinators may remit the corresponding payments to the designated CFC YFL depository banks on or before February 19, 2016 for District Servants, Cluster Servants, Chapter Servants, District Couple Coordinators, and Cluster Couple Coordinators, Chapter Couple Coordinators , and March 18, 2016 for Household Servants and potential leaders.

 Changes to the official list of registered delegates will be allowed only until March 23, 2016.

 In all cases, this form should be used and instructions contained herein shall apply.

INSTRUCTIONS

 Please fill out PRINT all the blank spaces provided. Complete the registration form and affix your signature where required.

 For CFC YFL Metro Manila Districts: Submit the completed registration forms and remit the corresponding payments at the CFC FFL Home Office: Apartelle 12, Star Mall Complex, EDSA cor. Shaw Blvd., Mandaluyong City 1552 Metro Manila.

 For CFC YFL Districts OUTSIDE of Metro Manila only: Go to any Allied Bank / BPI branch in your area and deposit the full amount corresponding to the number of delegates listed in the registration form.

 Fill out two copies of Allied Bank or BPI savings deposit slip and ensure that the following details are written accurately.

 On the day of the WYC, kindly bring all the documents with you, including your Registration Forms and Deposit Slip/s, to claim your delegations’ congress kits at the venue.
