BLUE STAR LEVEL  Program Audit – One per school via QUIA online Survey – NO additional document required  Yearlong Master Calendar* - One per school (targeted activities month by month for 9 months)  Weekly calendar* - One per counselor (“Real Life” snapshot of one week)  Identify School Counseling Program Goals– Minimum 3 program goals, one per target area (attendance, behavior, academic or other) based on school goals  Closing the Gap Action Plan - One per school, one Target Area (NOT RBES)(academic, behavior, or attendance)  Partnership Agreement - One per counselor  End of Year Results Report- Submit results for Closing the Gap Action Plan

RED STAR LEVEL  All items from the Blue Star Level (Yearlong Master Calendar, Weekly Calendars, 3 Program Goals, Closing the Gap Action Plans, and Partnership Agreements must be current for the school year)  Closing the Gap Action Plans – Two per school (consider continuing to target area from previous year and add new target area - academic, behavior, or attendance)  Advisory Council – Identify members; provide agendas and minutes from two meetings (could be a spring meeting from previous year with fall meeting for current year) (Brainstorming Worksheet available for planning)  Mission Statement, Vision Statement, Philosophy Statement  Standards and Competencies Crosswalk Tool  Annual End of Year Results Report – Submit results for two Closing the Gap Action Plans  Results Report: Impact Over Time – Updated annually

SILVER STAR LEVEL  All items from the Blue and Red Star Level (Yearlong Master Calendar, Weekly Calendars, Closing the Gap Action Plans, Partnership Agreements, 3 Program Goals, Advisory Council information, Mission, Vision and Philosophy, Standards and Competencies Crosswalk Tool)  Closing the Gap Action Plan - Three per school (academic, behavior, and attendance)  Classroom Curriculum – One unit/topic per counselor (at least 3 lessons) linked to standards and competencies; linked to at least one of the program goals, process, perception, and clear and relevant data  Classroom Curriculum Results Report  Annual End of Year Results Report – Submit results for three Closing the Gap Action Plans  Results Report: Impact Over Time – Updated annually

GOLD STAR LEVEL  All items from Blue, Red and Silver Star Level (Yearlong Master Calendar, Weekly Calendars, Closing the Gap Action Plans, Partnership Agreements, 3 Program Goals, Annual End of Year Results Report for 3 Goals, Results Report: Impact Over Time (current year updated), Advisory Council, Mission, Vision and Philosophy, Standards and Competencies Crosswalk Tool and Classroom Curriculum) (information must be current for the school year, not historical)  Small Group Curriculum – Minimum one per school (at least four sessions same topic) linked to standards and competencies and school counseling program goals; process, perception, and results data.  Small Group Curriculum Results Report – Reflects standards and competencies and is linked to school program goals.
