The borrowing coach/team must have this form completed and filed with the League office and obtain an official stamp before a player is used as a guest in a tournament or is allowed to practice with the borrowing team.

Teams failing to follow these procedures are subject to sanctions, including fines and suspensions.

Note: If a guest player is son or daughter of team coach, then signature of club president or travel commissioner is needed.
