In order to participate in the above Isaac's Social Event, I, on behalf of myself and on behalf of my heirs, assignees, personal representatives and next of kin and intending to be legally bound, consent to the following:

1. I agree to release, indemnify, and hold harmless Isaac's Deli, Inc. and its officers, directors, officials, agents, subsidiaries and employees, from any and all liability, for loss, harm, damage, injury, cost or expense whatsoever including without limitation, property damage, personal injury (including emotional distress), and/or death which may occur in connection with this Isaac's-organized social event and any event-related activities, and for any claims or causes of action. I agree that any claims or disputes arising from my participation in the event shall be governed by the laws of the Commonwealth of Pennsylvania, without regard to its conflict of laws provision, and the Court of Common Pleas of Lancaster County, Pennsylvania, shall have exclusive jurisdiction and venue.

2. I acknowledge and agree that it is my responsibility to be at the designated departure locations, at the designated departure times, in both Lancaster, PA and New York City, NY. I further agree that all expenses and any other resulting consequences incurred, as a result of my non-compliance with departure timing, is my responsibility.

3. I acknowledge that photos, slides, film, video or sketches may be taken during the trip and may be used by Isaac’s Deli, Inc. for publicity, advertising, promotion or any other purpose in connection with the company. I give my permission for their use.

4. I acknowledge that this trip is organized by Isaac's, but is NOT an Isaac's employment-related event. I am attending on my own free will and am under no obligation to attend as a result of my, or any other person's, continued employment with Isaac's.

5. I acknowledge and agree that on this trip I am not representing Isaac's nor acting on behalf of Isaac's Deli, Inc., its officers, directors, officials, agents, subsidiaries or employees.

6. This form is required to be completed in its entirety in order to participate in this event. No refunds will be issued for any reason after November 09, 2017 unless the bus trip is cancelled.

I HAVE READ THE FOREGOING WAIVER AND RELEASE, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
