2 The group and guest speaker introduced themselves. Introductions

Linda O’Neil, Horizons 3 Linda O’Neil introduced herself as a Senior Practitioner from Horizons. Linda explained that this service was funded by the lottery two years ago and they currently have 1 year of this funding left. The group agreed to support the service in any future applications for funding. Horizons have family workers and have various partnerships with other services including the YMCA in Consett. Horizons also offer a telephone/texting service. People between the ages of 14 – 25 years of age who are unpaid carers can access the Horizon’s service. As the age range can include transitional age groups there are many issues that Horizons can help with including education, housing and welfare and benefit issues. 30% of young carers care for a parent with mental health problems. Being a young carer can have an emotional impact including social isolation on a carer. Horizon have peer mentoring and also forum groups to support young carers with this. Carers breaks and trips are also offered. Carers can also gain access to driving lessons, gym membership, anger management sessions, light boxes for SAD (seasonal adjustment disorder) etc. Linda explained that if a young carer presents the referral process to the Horizon’s service is very simple. A simple telephone call will suffice and then a full assessment will be carried out with the young carer. Young carers will have access to a carers card for up to three years. This card can be shown in many shops etc to gain huge discounts. Horizons also work closely with Family Action and Durham County Carers Support. The Horizons Service covers the whole of County Durham. Last year the service received 240 referrals and to date this year they have received 200 referrals. Linda distributed information leaflets etc to the group.

CQC Update 4 Julia explained to the group that the practice had their CQC visit on 11th November 2013. Julia thanked the patient forum group members that came into the practice on the day of the inspection to speak to the inspectors. The practice was assessed on the following standards:  Respecting and involving people who use Services  Safeguarding people who use services from Abuse  Cleanliness and infection control  Supporting workers  Assessing and monitoring the quality of service Provision The practice met all of the standards. The report from the inspection is available on the CQC website and there is also a link to this via the practice website (http://www.cqc.org.uk/node/734894). This was an in depth inspection. 20% of all practices in Co Durham and nationally will be inspected. The practice will be due for another routine inspection in 2 years time but the CQC can inspect the practice at any time if a problem is highlighted to them. David highlighted that in the recent media reports regarding GP practices and CQC inspections it has given the public the perception that only practices where problems were highlighted have been inspected. This is not the case. Dr. Chadwick has written to the BMA regarding this. The group congratulated the practice on the successful inspection.

Practice Update 5  Patient Survey Cherrie explained to the group that the completed patient surveys would be sent to CFEP very shortly as 180 questionnaires have currently been completed. 200 questionnaires need to be submitted to produce a valid report. When the report has been produced Cherrie will contact patient forum members to arrange a date to meet to discuss the results and form an action plan for 2014/15. Cherrie explained to the group that there are currently 411 patients currently signed up to use the Patient Access service  Patient Access/Mobile Phone Number Coverage enabling them to book appointments online and also order repeat medications. By the end of March 2014 we need to ensure that at least 5% of the practice population are signed up to use this service. This would equate to approximately 454 patients. This service is advertised widely in various forms including, notice boards within the practice, in the practice newsletter, via the practice website. During the flu campaign and open flu clinics this service was widely advertised to patients also. Cherrie informed the group that the practice currently has 44% of the practice population’s mobile telephone numbers. Once again, during the flu campaign and open flu clinics patients were encouraged to update their contact details if needed. A message regarding this is also displayed on the self check in when used. The practice will continuously work to improve the mobile telephone coverage rate.

 BHAW Gold Award Cherrie explained to the group that the practice have currently been assessed on the Gold Better Health at Work Award and are awaiting the results. Health advocates within the practice have been working through the Better Health at Work Award since 2011 successfully achieving the bronze and silver awards. The practice have also worked with the Spectrum as part of the gold award, mentoring an organisation who were just beginning the bronze award.

Cherrie explained to the group that a patient had recently queried why there was a £50 charge if they requested copies of  NHS Choices Website their full medical record. Cherrie explained that all of this information was available via the NHS Choices website along with a lot of other useful information. Practice performance information will also be available via this website.

 Flu Clinics/Winter Pressures Julia explained to the group that the open flu clinics in September/October went well. The target that practices need to be working towards with regard to vaccination coverage is 75% of all patients over the age of 65. The practice has currently vaccinated approximately 78/9% of these patients. The chronic disease targets are all between 95/6% and the practice have currently vaccinated approximately 90% of patients within the chronic disease areas. The practice has currently vaccinated approximately 78% of all eligible patients.

 RCGP Carers Action Plan Julia explained to the group that Jan Burden from Durham County Carers Support informed Julia of the RCGP Carers Application which would give the practice funding to improve the service that is offered to carers by the practice. An application was submitted and was successful. To begin with a 6 week action plan was submitted and the practice worked towards this action plan. We have now put together a 12 month look forward strategy that the whole practice team will work towards. Carers have historically been offered an annual health check. When carers are identified they will now be offered a carers health check as routine procedure. As part of the carers health check the carer will be given a consent form. This form is to be completed by the cared for to enable the carer to have access to their medical records to enable them to query medications etc. The GPs now have referral forms on prescription pads to make it easy for them to refer unpaid carers to the appropriate service. The practice website now has links to three different carers organisations including Horizons, The Bridge and Durham County Carers Support. The question of ‘Are you a carer?’ has been added to clinical templates that the GP use to prompt this. The practice is also working with the wider healthcare team to try and identify more unpaid carers. Jan Burden from Durham County Carers Support has just recently circulated information regarding training for carers. It is Carers Awareness Week in June 2014. Roger offered to help the practice with their 12 month look forward strategy. Roger informed the group that carers services were merging and would be based at South Church.

Cherrie explained to the group that the practice are currently experiencing problems in relation to anti social behaviour. The police are aware of this. Cherrie has contacted the local schools asking for this to be mentioned in assemblies and  Anti Social Behaviour also asking for an article to be put in the school newsletters. Graham will also highlight this at the police meeting that he attends.

Copies of this survey were distributed to the group. The group were asked if they were happy to complete this survey.  People’s Mental Health and Cherrie informed the group that she was happy to forward the completed surveys to the freepost address. Wellbeing Survey

Cherrie had invited someone from the North of England Commissioning Support Team (NECS) to present information to the group with regard to the Call To Action National Engagement Exercise. Unfortunately no one responded to the invite.  Call To Action Cherrie distributed powerpoint slides and information sheets regarding this exercise to the group and gave a briefing on the exercise which is that NHS England is calling on patients, the public and staff to join in a discussion about the future of the NHS so it can plan how best to deliver services, now and in the years ahead. There are certain issues to be considered which are as follows:

. Over 15 million people have a long-term condition and account for over 70% of all NHS spend. . People living in the poorest parts of England will, on average, die 7 years earlier than those living in the richest areas

. Nearly two thirds of people admitted to hospital are over 65 years old.

. Around 80% of deaths from the major diseases such as cancer are attributable to lifestyle risk factors such as alcohol, smoking, diet and lack of exercise.

. By 2021 there could be more than one million people in the UK with dementia – more the twice the number who are currently living with the disease. . If the current models of care remain unchanged in England, the gap between funding and spending requirements will grow to £30 billion by 2020/21 in England.

The group were asked to complete a questionnaire in relation to this. The group will forward their responses to Cherrie via e-mail and she will forward them on NECS.

Cherrie explained to the group that the Health and Social Information Centre are planning to run a care.data programme. This programme consists of data being extracted from GP practices to use to look at statisitics etc. It is important that the NHS can use this information to get a complete picture of what is happening across health and social care and to plan services according to what works best. The new system will provide joined-up information about the care received from all of the different parts of the health service, including hospitals and GP practices. The programme will begin in Spring  Care.Data 2014. A mail drop will be done in January 2014 informing the general public of this programme and what to do if they decide to opt out. Cherrie explained that any queries regarding this from patients can be directed to Julia/Cherrie. A notice board and information leaflets have been provided within the practice and information about this is also on the practice website.

The group worked through the action plan that was put in place in March of last year. More work needs to be done around DNA figures.

 Action Plan 2013/14 Graham will provide feedback after the next meeting. Feedback from Durham Dales Patient 6 Forum Group Meeting David informed the group that when the national prescription came out regard patient involvement and the Durham Dales Patient Forum Group he was certain that there were roles and specifications that had to be met with regard to representation etc these including the role of chair and also finance. Graham will bring this up at the next meeting for clarification.

A Practice Manager representative now attends this meeting.

Open Evening Session 7 David explained to the group that as the patient forum group meetings take place on an afternoon it does restrict attendance for some patients who may be interested in joining the group. David suggested that an open evening session once in a while would be beneficial. David and Graham will discuss this and feed back to the group.

Willington Care Village Update 8 Unfortunately Julie Gray couldn’t attend the meeting today but did send an update as follows: WCV just had their CQC Inspection and all homes within WCV have met all standards needed. The have also had a Local Authority Commissioning Quality Audit done and they have maintained Grade 1 which is the top Grade, and this consisted of a 3 day audit on each of the units within WCV.

On Lumley and Richmond Court they have completed the Sterling University Dementia Audit Tool and from that an action plan has been put together to enhance the environment in relation to dementia residents and hope to progress in the New Year.

SLAM Update 9 Dan feedback that there are still ongoing issues in relation to the exercise on prescription scheme. The cost of this scheme is more expensive that signing up for gym membership.

WCAC Update 10 Margaret had to leave the meeting and was not available to give an update
