Application Raymond K. Foley Award Awarded by the Naugatuck Parent School Council

The Raymond K. Foley Award, presented by the Naugatuck Parent School Council, honors an individual or group who has demonstrated outstanding service and unselfish dedication contributing to the betterment of Naugatuck children.

Please attach letter of nomination to the application, which should include: a. Position of service in the group b. Length of volunteer service to community (at least five years) c. Additional group affiliations of nominee d. Major accomplishments of nominee e. Personal attributes that support nomination f. Any other letter of support may be added.

The PSC chooses the recipient of the Raymond K. Foley Award by a vote of Council members. Only the letter of nomination will be read aloud during the voting process. Letters of support may be summarized and distributed to Council members prior to the voting meeting. These letters may also be available for Council members to read, in their entirety, prior to the vote.

*Please include the names of persons associated with the nominee willing to speak at the Award’s dinner. The dinner will be held on Tuesday, May 12, 2015 at Leary’s Crystal Room in Naugatuck, Connecticut.
