Authorized Signature(s) of all handlers (parent or guardian must sign if under 18) By attending and/or participating in the 2012 Mountain Dog Canine Carnival (the "Event), and affixing my signature above I agree to the following:

THIS SIGNED WAIVER MUST BE PRESENTED BEFORE PETS WILL BE ALLOWED INTO THE EVENT VENUE

LEASH REQUIRED: All dogs will be kept ON LEASH AT ALL TIMES in the Event Venue. Dogs must be over 6 months of age, not in heat, sociable, non-aggressive, healthy, well groomed and NO FLEXI LEASHES please (or lock at 4-6ft). All dogs shall have current vaccines to the best of the handlers knowledge, including rabies, parvo and kennel cough. Consult your vet for additional recommended vaccine protection at a large event with many other dogs in attendance including shelter animals. We further recommend keeping a distance of 5-6 feet between all dogs at all times while in the Event Venue.

2. Release of Liability by Attendees-Participants-Volunteers and Vendors: Mountain Dogs of Colorado, Inc. and Event Organizations including Vail Pet Partners and Eagle Valley Humane Society do not assume any responsibility for lost, stolen or injured dogs at the event or in or around the Event Venue. By bringing a dog to this Event Venue and/or participating in the event in any matter or competing in an event sport or contest with or without a dog, you do so entirely at your own risk and assume all responsibility and liability including cost and expenses in the event of any harm to the dog in your care, to other dogs or persons and to yourself.. By attending this Event should the dog in your care incur or cause any injury to any person, including yourself, other dog(s) or property as a result of the behavior of the dog in your care (or anyone else’s care) at this Event or in the Event Venue, whether expected or unexpected, regardless of the stimulus for the behavior (i.e., animal’s spontaneous conduct, the conduct of another animal, the conduct of any person or thing, or a combination of any of these), as the dog handler, dog owner or other and as the Event attendee, participant, volunteer or vendor you are solely liable and be held responsible for any injury or property damage that results there from (including reasonable attorneys fees). As an attendee, participant, volunteer or vendor, you further agree that you will indemnify, hold harmless, protect and defend Mountain Dogs and the Event Organizations, for any and all claims of liability and damages from any injury, property damage, loss, claim, or expense resulting from the behavior of any dog in your care, custody or control, temporary or immediate, and to report any and all such injuries immediately to Mountain Dogs Event Committee.

3. DOG POOP AND PEE: There are designated special areas for dogs to be walked and for ‘relieving’ themselves. Please be considerate of our venue and clean up after your animals. Bags and trash receptacles will be provided at the Event Venue. Participants are required to clean up all animal messes with a plastic baggie and/or paper towels immediately after any occurrence.

4. EXPEL: Management reserves the right to expel participants if, in Management’s opinion, their conduct or presentation is objectionable to the public.

6. PHOTOS: Any photos, video, or audio taken by management are the property of Mountain Dogs Canine Carnival and its organization and Participants’ image or likeness can be used by show management for any form of advertising or promotion.

ATTENDEE REPRESENTATION: I certify that my dog has never injured or bitten a person or other animal to my knowledge and I have read and understand the above rules and requirements of participation in Mountain Dogs Canine Carnival and agree to indemnify and hold harmless Mountain Dogs of Colorado, Inc and Event Organizations including Vail Pet Partners and Eagle Valley Humane Society, event sponsors, exhibitors, employees, volunteers, managers, members and officers, from any and all claims of liability, personnel injuries, losses or damage to property caused by, or arising from the event described herein. I/we execute this waiver voluntarily and with full knowledge of its significance.

I HAVE READ THIS RELEASE, FULLY AND UNDERSTAND ITS CONTENTS. I HAVE SIGNED THIS RELEASE FREELY AND WITHOUT PROMISE OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
