The Carle Foundation Hospital General Policy CFH183 Subject Code Silver Response - Person on campus using or displaying a weapon Review 01/08 Revised Purpose To coordinate a proper response that would minimize the risk and number of casualties in the event of a person using or displaying a weapon on the Carle Medical Campus. To assist law enforcement in locating and disarming the suspect.

Definitions Code Silver Exterior - Report of a person(s) using or displaying a weapon outside of Carle Buildings but on the Carle Medical Campus Grounds. Code Silver “Location” - Report of a person(s) using or displaying a weapon inside the Carle Medical Campus. Example: “Code Silver North Tower 1.” Facility Lockdown - Locking of all exterior doors to the facility. Evacuation - Evacuate the building using your department specific fire evacuation route. Shelter in Place - Lock department or office door, turn off lights, advise patients and visitors to stay within secured area and hide from danger. Statement of Policy 1. Carle is committed to maintaining a safe and secure environment. Carle strives to eliminate the occurrence of all workplace violence incidents. All suspicious activity, threats, battery or any other workplace violence occurrence will be reported to the Carle Security Department for appropriate follow up activity. Refer to Corporate Human Resources Policy CF443 Workplace Violence. 2. In the event of an individual on the Carle’s Main Medical Campus using or displaying a weapon, Carle will institute procedures to minimize the risk to our patients, visitors and employees by evacuation, facility lockdown and/or “shelter in place” procedures and by containing the incident as much as possible. Procedure – Outside of Buildings but on the Medical Campus 4. Person Displaying or Using Weapon outside of buildings but on the Medical Campus: a. Upon observing an individual(s) displaying or using a weapon outside the facility but on the Carle Medical Campus; report to Carle Security (3-3911) immediately. b. After ensuring that you are in a safe location; provide Carle Security with the following:  Location  Type of weapon displayed or using  Any casualties  Direction of Travel  Physical Description of the Suspect (include clothing description). c. Carle Security Department  Security Control Center Officer will overhead page “Code Silver- Exterior” three times.  Security Control Center Officer will immediately contact the Urbana Police (via the direct line to METCAD 911) and provide as much information as possible.  Security Control Center Officer will dispatch incident to all on duty Security Officers.  Security Control Center Officer will immediately lock down all Access Controlled exterior doors to the facility.  Security Control Center Officer will immediately close all interior fire doors (via the Simplex Fire Station) in an attempt to put up barriers to protect innocent persons and to contain the suspect.  Security Control Center Officer will send a house wide email alerting employees of the threat.  Security Control Center Officer will attempt to track the suspect on CCTV and provide continuous information to METCAD 911.  Security Officers on interior patrol will manually lockdown all exterior doors that are not on the access control system.  Security Officers on exterior patrol will respond to the general vicinity (at a safe distance) to observe and report suspect information to the Security Control Center Officer.

Procedure - Person Displaying or Using Weapon outside of buildings but on the Medical Campus (cont.)  Security Officers will communicate with as many people as possible to alert them of the potential danger and instruct them not to leave the facility.  Security Control Center Officer will contact the Director of Security and Administrative House Officer as soon as possible to advise them of the incident.  Document the incident in Security Incident Report Form and assist Local Law Enforcement with evidence preservation (CCTV video, etc) and investigation. d. Carle Hospital and Carle Clinic Employees  Upon hearing a “Code Silver Exterior” overhead page; employees shall Shelter in place.  A designated member of the department (employee with a key) shall lock department door and initiate the Shelter in Place procedure.  Employees shall remain calm. Confidently and calmly assure staff to stay within the secured area/department.  Listen for additional overhead page announcements and follow procedures after announcements are made.  Wait for all “Code Silver all clear” overhead page before resuming normal operations. e. Carle Communications Center  Upon hearing a “Code Silver” overhead page, the Communication Center will overhead page “Code Silver” every ten minutes until Security pages a “Code Silver all clear.”  Page information on Weather Alert pagers to SBU’s f. Carle Administration  Institute the Hospital Incident Command System (HICS) as soon as possible in the Carle Foundation Board Room.  HICS team to alert Emergency Department on possible number of inbound patients. g. Local Law Enforcement  Will respond and neutralize the offender. Procedure – Inside the Carle Medical Campus 5. Person Displaying or Using Weapon inside the Carle Medical Campus: a. Upon observing an individual(s) displaying or using a weapon within the facility; go to a safe location and report to Carle Security (Orange Button or 3-3911) immediately. b. Provide Carle Security with the following:  Location  Type of weapon displayed or using  Any casualties  Direction of Travel  Physical Description of the Suspect (include clothing description c. Carle Security Department  Security Control Center Officer will overhead page “Code Silver- Location” three times.  Security Control Center Officer will immediately contact the Urbana Police (via the direct line to METCAD 911) and provide as much information as possible.  Security Control Center Officer will dispatch incident to all on duty Security Officers.  Security Control Center Officer will immediately lock down all Access Controlled exterior doors to the facility to minimize the number of people coming into the building.  Security Control Center Officer will immediately close all interior fire doors (via the Simplex Fire Station) in an attempt to put up barriers to protect innocent persons and to contain the suspect.  Security Control Center Officer will send a house wide email alerting employees of the threat.  Security Control Center Officer will attempt to track the suspect on CCTV and provide continuous information to METCAD 911.  All Security Officers will report to the General Vicinity but not in an area of danger (Security Officers discretion) and divert foot traffic away from the area.  Security Officers will communicate with as many people as possible to alert them of the potential danger and instruct them to shelter in place and/or evacuate.  The Security Control Center Officer will contact the Director of Security and House Officer as soon as possible to advise them of the incident. Procedure – Inside the Carle Medical Campus (cont.)  Document the incident in Security Incident Report Form and assist Local Law Enforcement with evidence preservation (CCTV video, etc) and investigation. e. Carle Hospital and Carle Clinic Employees  Upon hearing a “Code Silver Location” overhead page; employees located in the affected department(s) or on the same floor adjacent to the “Code Silver Location” shall evacuate using their department specific fire evacuation route. Employees shall make every reasonable attempt to assist patients and visitors in department evacuation. In some cases it may be necessary to close or close and lock doors to patient areas or rooms to allow for them to “Shelter in Place.”  All other employees upon hearing a “Code Silver Location” overhead page; employees shall “Shelter in place.”  A designated member of the department (employee with a key) shall lock department door and initiate the “Shelter in Place” procedure.  Employees shall remain calm. Confidently and calmly assure staff to stay within the secured area/department.  Listen for additional overhead page announcements and follow procedures after announcements are made.  Wait for all “Code Silver all clear” overhead page before resuming normal operations. h. The HICS team shall focus on the following items during the Recovery phase: a. Facility Perimeter Control & Media Access b. Damage Assessment & Repair c. Cleanup d. Incident Debriefing e. Business Continuation Plan Training 7. Carle Foundation Hospital will train all hospital personnel on this policy at least once annually within the Carle Safety Training structure. Drills will be conducted periodically. The drills will be evaluated with plans for improvement implemented Signatures on File in Administration
