Dato’ Sri Liow Tiong Lai (Malaysia) and Ms Amenta Matthew (Marshall Islands) were elected Chairperson and Vice-Chairperson, respectively. Mr Taniela Sunia Soakai (Nauru) and Dr Bounpheng Philavong (Lao People’s Democratic Republic) were elected Rapporteurs.

I. INTRODUCTION AND SUMMARY

The sixty-fi rst session of the Regional Committee for the Western Pacifi c was held in Putrajaya, Malaysia, from 11 to 14 October 2010. The session was attended by representatives of , Darussalam, , , Cook Islands, Fiji, (China), , Kiribati, the Lao People’s Democratic Republic, Macao (China), Malaysia, the Marshall Islands, , Nauru, New Zealand, Niue, Palau, Papua New Guinea, the , the Republic of Korea, Samoa, , Solomon Islands, Tokelau, Tonga, Tuvalu, Vanuatu and Viet Nam, and by representatives of France and the United States of America as Member States responsible for areas in the Region.

Representatives of the Commonwealth Secretariat, the Global Alliance for Vaccines and Immunisation, the Global Fund to Fight AIDS, Tuberculosis and Malaria, the Joint United Nations Programme on HIV/ AIDS, the Secretariat of the Pacifi c Community, and the United Nations Population Fund; observers from the Heart Foundation of Malaysia, Millennium Challenge Account Mongolia Health Project, the International Federation of Non-Government Organisations for the Prevention of Drug and Substance Abuse, the Korea Foundation for International Health Care—Dr Jong Wook-Lee Memorial Fund, Urbani International, the Pacifi c Island Health Offi cers Association; and representatives of 21 nongovernmental organizations also attended.

At the opening ceremony, the retiring Chairperson of the Regional Committee noted the achievements of the Region’s Member States over the previous year, especially in standing up against pandemic infl uenza H1N1 (2009), leading the fi ght against tobacco, promoting healthier lifestyles and protecting health and the environment in a sustainable and context-relevant manner. The Regional Director expressed special appreciation to the for hosting the Regional Committee on short notice. He called for renewed leadership in dealing with the issues on the agenda, particularly further actions to meet the commitments towards achievement of the United Nations Millennium Development Goals. The Deputy Director-General noted the foresight that had been shown in establishment of the Regional Committee, with shared health concerns as its basis, and the even greater interconnectivity of countries today. As the most diversifi ed of all the WHO regions, the Western Pacifi c was more representative of the world and its Regional Committee’s discussions were therefore of special importance. The Minister of Health of Malaysia, Mr Liow Tiong Lai, offered the Committee the hospitality of the multicultural community of Malaysia and expressed his hope that the session would be a productive and successful forum. The Prime Minister of Malaysia, the Honourable Mohd Najib bin Tun Abdul Razak, welcomed the participants and noted that the Regional Committee was an important forum for health ministers and experts in the Region. The sixty-fi rst session was particularly signifi cant as it gave an opportunity to examine the response to the effects of pandemic infl uenza H1N1 (2009) and could provide input into future global actions.

The retiring Chairperson and the Director-General (by video) addressed the Committee. The Regional Director then presented his report on the work of WHO in the Region for the period from 1 July 2009 to 30 June 2010. Several representatives congratulated the Regional Director on his comprehensive report. Other comments fell into the following main categories: health system strengthening; preparedness for pandemic and emerging diseases, including WHO’s response to the pandemic H1N1 (2009); and progress towards achievement of the health-related Millennium Development Goals.

The second meeting began with an address by the incoming Chairperson. The fi nal report on budget performance for the Programme Budget 2008–2009 was then examined. The Director, Programme Management, explained that implementation of assessed contributions had amounted to US$ 79.2 million or 99.8% of the fi nal working allocation, while implementation of voluntary contributions, which had increased by 36.7% compared with the previous biennium, had amounted to US$ 150.8 million or 70.4% of available resources. The Western Pacifi c Region had continued the trend of implementing at a higher level in countries. The performance assessment of the Programme Budget 2008–2009 had been the fi rst to be carried out within the framework of the Medium-term Strategic Plan (2008–2013). A systematic review of WHO workplans had led to a series of organizational changes, including a reduction in the number of technical units from 31 to 17 and the introduction of Strategic Frameworks at the country and regional levels.

Commenting on the report, representatives praised the clearer presentation of outputs and results of the Programme Budget 2008–2009 and their appraisal as regards achievement. They also commended the organizational changes that had been made. While the improved budget implementation was noted, there were calls for analysis of the reasons for only partial achievement of some objectives. The Director, Programme Management, responding to comments, said that another element would be added to the implementation analysis, based on the indicators in the Strategic Frameworks, and that the Regional Offi ce would broaden its search for donors to fi nd support for underfunded areas (decision WPR/RC61(1)).

The Director, Programme Management, then presented the Proposed Programme Budget 2012–2013, informing the Committee that it would be developed further in close collaboration with country offi ces and WHO Headquarters and aligned with the fi nal Organization-wide Proposed Programme Budget 2012–2013. The overall format was similar to the 2010–2011 budget, with refi nements that refl ected shifts in emphasis, better alignment with country priorities and steps taken to address the gap between the previous programme budgets and actual implementation rates. The Director, Department of Planning, Resource Coordination and Performance Monitoring, WHO Headquarters, explained that the proposed Organization—wide Programme Budget 2012–2013 aimed to make a timely contribution to achievement of the Millennium Development Goals, improving performance at the country level, increasing work on noncommunicable diseases, contributing to health governance, and responding better to Member States’ demands for greater transparency and accountability. The 13 strategic objectives had remained largely unchanged, with no new Organization- wide expected results. Several representatives, while welcoming the shift to a fully integrated budget and the greater integration of objectives across programmes, raised the possibility of reducing or eliminating earmarking and having greater fl exibility in allocation, especially as regards voluntary contributions (resolution WPR/RC61.R1).

The Regional Committee was then asked to consider the draft Western Pacifi c Regional Strategy on Health Systems Based on the Values of Primary Health Care, which was discussed at the second and third meetings. The Regional Director said that the draft Strategy had been developed through extensive REPORT OF THE REGIONAL COMMITTEE 5

consultation with Member States, involving interviews in 28 countries, and had undergone expert review. The representatives were also asked to consider the draft global Framework for National Health Strategies, Policy and Plans, drafted at WHO Headquarters. The Deputy Director-General explained that renewed emphasis was being given to health policies and plans as research had shown that a strong national health agenda was a major factor in achieving greater health gains. The representatives who spoke on the item were strongly supportive of the draft Strategy and the global Framework, a well as the renewed focus on primary health care (resolution WPR/RC61.R2).

The Committee then turned its attention to women’s health. The Regional Director said that the discussion paper summarized the issues and challenges in the area of women’s health, covering two aspects: areas where, without special services, women would be especially vulnerable; and social, economic and cultural situations in which women’s health was worse than men’s. Many representatives, supporting the renewed commitment to women’s health and agreeing with the need for women-friendly and gender-sensitive policies, stressed the importance of such determinants of women’s health as education, employment and political empowerment. They also outlined the situation as regards women in their countries. The Director, Building Healthy Communities and Populations, noting the representatives’ emphasis on the need for monitoring, said that WHO would strengthen the collection of information on violence against women to support Member States in addressing that issue. The Regional Director told representatives that a cross-cutting working group had been set up at the Regional Offi ce to facilitate the collection of data from all programme areas, which would be used to prepare a comprehensive report on all aspects of women’s health in the Region.

The fourth meeting began with examination of issues that had emerged in an informal consultation at WHO Headquarters that had focused on: (1) the future role and function of WHO; and (2) the future fi nancing of WHO. The Assistant Director-General for General Management informed the Committee that appropriate proposals would be drafted, based on consultations with Member States, and presented to the Executive Board for its consideration. Representatives agreed on the need for redetermination of WHO’s role and key functions, the general consensus being that the Organization should strengthen its role as the recognized global authority in health matters, setting standards, guidelines and recommendations. WHO’s leadership was indispensible in health security. As regards fi nancing, there was general consensus concerning the need for better prioritization and coordination, as well as improved fi nancial management.

The Regional Committee was then asked to discuss the report on healthy settings. The Regional Director noted that rapid urbanization and globalization had led to changes in the determinants of health and that healthy settings approaches, such as Healthy Cities and Healthy Islands, needed to be scaled up. Representatives welcomed the report and the expansion of the healthy settings approach, and shared experiences in their countries. Many successes with Healthy Cities and Healthy Islands initiatives were reported, but the need for multisectoral collaboration and community involvement was emphasized (resolution WPR/RC61.R6).

Consideration was then given to the draft Regional Strategy to Stop Tuberculosis in the Western Pacifi c (2011–2015), which was discussed in the fourth and fi fth meetings. The Regional Director told the Committee that the Western Pacifi c Region had reason to be proud of its achievements in controlling tuberculosis (TB). The draft Strategy was meant to ensure that all countries in the Region could reach and care for all TB patients at an early stage of their disease. Its endorsement would be an important step towards modernization of TB control in the Region. Representatives, indicating their support for the draft Strategy, described the TB situation in their countries and progress in controlling the disease (resolution WPR/RC61.R4). 6 REGIONAL COMMITTEE: SIXTY-FIRST SESSION

The Committee was then asked to review and consider endorsing the draft Asia Pacifi c Strategy for Emerging Diseases (2010). The Director, Programme Management noted that the fi rst Asia Pacifi c Strategy for Emerging Diseases (APSED), which had been endorsed in 2005, had been used extensively by WHO and Member States to strengthen capacity. As it was nearing the end of its lifespan, WHO had consulted with Member States to produce APSED (2010), which took into account the lessons learnt during the H1N1 pandemic in 2009. The many representatives who spoke attested to the usefulness of APSED (2005) and endorsed the new APSED (2010) (resolution WPR/RC61.R5).

The report on vaccine-preventable diseases was discussed at the fi fth and sixth meetings. The Director, Programme Management, reminded the Committee that, although it had been 10 years since the declaration of the Region’s poliomyelitis-free status, it was still necessary to remain vigilant. The Region was also continuing to work hard to achieve measles elimination and hepatitis B control by 2012, but the report pointed to the need for renewed commitment and outlined the steps to be taken if those goals were to be achieved (resolution WPR/RC61.R7).

The Director, Programme Management, then presented progress reports on three areas of work: achievement of the Millennium Development Goals; dengue fever and dengue haemorrhagic fever prevention and control; and protecting health from the effects of climate change. In the discussions that followed, representatives outlined the situation in their countries as regards the areas under review.

The Committee then turned its attention to the procedures for the nomination of the Regional Director. The Legal Counsel presented his report, containing a number of proposals for a cost-effective interview process that would enable all Member States of the Region to familiarize themselves with the candidates, as well as proposals on the possible format and content of a code of conduct for Member States during the nomination process. There was general consensus in favour of an interview process being conducted, with most favouring interviews at the session of the Regional Committee (resolution WPR/RC61.R3).

The seventh meeting began with examination of a number of resolutions adopted at the Sixty-third World Health Assembly that were of particular interest to the Western Pacifi c Region. Those included: WHA63.14, on the marketing of food and non-alcoholic beverages to children; WHA63.16, on the WHO Global Code of Practice on the International Recruitment of Health Personnel; WHA63.19, on the WHO Global HIV/ AIDS Strategy for 2011-2015; and WHA63.28, on the establishment of a consultative expert working group on research and development: fi nancing and coordination. The Committee nominated the following experts in the Region as candidates to participate in the Consultative Expert Working Group on Research and Development: Financing and Coordination: Professor Guo Yan (China); Mr Shozo Uemura (Japan); Dr Chansaly Phommavong (Lao People’s Democratic Republic); and Dr Leizel Lagrada (Philippines) (decision WPR/RC61 (2)).

The Committee next selected Malaysia to serve on the Policy and Coordination Committee of the Special Programme of Research, Development and Research Training in Human Development (decision WPR/RC61 (3)), and the Lao People’s Democratic Republic as one of the representatives of the Region to serve on the Joint Coordinating Board of the Special Programme for Research and Training in Tropical Diseases (decision WPR/RC61 (4)).

Regarding future sessions, the Regional Committee agreed that the sixty-second session would be held from 19 to 23 September 2011 at the Regional Offi ce in Manila (resolution WPR/RC61.R8). REPORT OF THE REGIONAL COMMITTEE 7

At the closure of the session, the Regional Director and the Committee expressed their appreciation to the Government of Malaysia for hosting the sixty-fi rst session; the Chairperson, Vice-Chairperson and Rapporteurs for their hard work; and the representatives of the intergovernmental and nongovernmental organizations for their oral statements (resolution WPR/RC61.R9). 8 REGIONAL COMMITTEE: SIXTY-FIRST SESSION

The Regional Committee, Having examined the draft Organization-wide Proposed Programme Budget 2012–2013 1 and the draft Proposed Programme Budget 2012–2013 of the Western Pacifi c Region 2 to be fi nanced from assessed contributions and voluntary contributions; Appreciating the presentation of the draft Organization-wide Proposed Programme Budget 2012–2013 and the draft Proposed Programme Budget 2012–2013 of the Western Pacifi c Region according to the principles of results-based budgeting; Commending regional efforts to more fi rmly base strategic and operational planning processes on the priorities of individual countries and the agreed collective regional technical agenda, and to improve accountability for clearer results within the framework of the Medium-term Strategic Plan, 1. APPRECIATES the comprehensive presentation by the Secretariat of the draft Organization-wide Proposed Programme Budget 2012–2013 and the draft Proposed Programme Budget 2012–2013 of the Western Pacifi c Region;

2. THANKS the Regional Director for his commitment to results-based budgeting and the continuous improvement of the presentation of the draft Proposed Programme Budget;

3. REQUESTS the Regional Director:

(1) to convey to the Director-General the comments of the Regional Committee on the draft Organization-wide Proposed Programme Budget 2012–2013, so that these can be taken into consideration when the Programme Budget 2012–2013 is fi nalized by the Director-General;

(2) to present to the Regional Committee, at its sixty-second session, a proposal for the Programme Budget 2012–2013 of the Western Pacifi c Region, taking into account comments by Member States and discussions at the sixty-fi rst session of the Regional Committee, and resolutions of the 128th session of the Executive Board and the Sixty-fourth World Health Assembly.

WPR/RC61.R2 WESTERN PACIFIC REGIONAL STRATEGY FOR HEALTH SYSTEMS BASED ON THE VALUES OF PRIMARY HEALTH CARE

The Regional Committee, Mindful that a health system consists of all organizations, people and actions intended to promote, restore or maintain health, and that a good health system delivers effective, safe and quality interventions, when and where needed;

Recognizing that the enjoyment of the highest attainable standard of health is one of the fundamental rights of every human being and that health systems are an important contributor to the realization of that right; Acknowledging that strengthened health systems contribute to the realization of the right to the highest attainable standard of health and the achievement of global health goals, such as the Millennium Development Goals; Reaffi rming that strong health systems based on the values of primary health care and focused on a vision of providing universal coverage for quality health services can be an effi cient and effective way to contribute to improved and equitable health outcomes; Noting that the values of primary health care to be considered for health systems, as contained in the Western Pacifi c Regional Strategy for Health Systems Based on the Values of Primary Health Care, include equity, social justice, universality, people-centredness, community protection, participation, scientifi c soundness, personal responsibility, self-determination and self-reliance; Further noting that there are existing strategies in the areas of health fi nancing, laboratory services, access to essential medicines, human resources for health, noncommunicable diseases and emerging diseases, and that these are consistent with the Western Pacifi c Regional Strategy for Health Systems Based on the Values of Primary Health Care; Recognizing that the Regional Strategy builds upon and is consistent with The World Health Report 2000—Health Systems: Improving Performance; The World Health Report 2006: Working Together for Health; Everybody’s Business: Strengthening Health Systems to Improve Health Outcomes; and The World Health Report 2008 — Primary Health Care: Now More Than Ever; Acknowledging that health systems are complex and diverse and that no single model of health systems strengthening is suitable for all countries and areas, 1. ENDORSES the Western Pacifi c Regional Strategy for Health Systems Based on the Values of Primary Health Care;1

2. URGES Member States:

(1) to commit to the development of strong and robust health systems based on the values of primary health care, leading to universal coverage of quality health services as each Member State defi nes its own path towards achievement of that vision;

(2) to conduct an ongoing public dialogue on the national vision for their health system and to update health strategies and policies at appropriate times to articulate that vision;

(3) to disseminate, as appropriate, to relevant stakeholders the core elements of the Regional Strategy;

3. REQUESTS the Regional Director:

(1) to provide technical cooperation as requested by Member States to facilitate implementation of the strategy;

(2) to work with Member States to develop and further refi ne indicators and guidelines for health systems;

(3) to work with Member States in developing methods of health systems performance assessment that are tailored to their specifi c needs;

(4) to advocate for health systems strengthening based on the values of primary health care and, when appropriate, convene Member States and other stakeholders;

(5) to promote a more integrated health systems approach in WHO’s support to Member States;

(6) to report periodically to the Regional Committee on implementation of the strategy.

WPR/RC61.R3 PROCEDURE FOR THE NOMINATION OF THE REGIONAL DIRECTOR

The Regional Committee, Having considered the report by the Legal Counsel on the procedure for nomination of the Regional Director; Wishing to improve the fairness of the process for the nomination of the Regional Director; Having considered the practice followed within the World Health Organization for the nomination of other elected offi cials, 1. APPROVES the amendments to Rule 51 of the Rules of Procedure of the Regional Committee contained in Annex 3 to the report by the Legal Counsel (WPR/RC61/11), which will become effective at the end of the sixty-fi rst session of the Committee;

2. DECIDES, with regard to the interviews of candidates for the post of Regional Director, that:

WPR/RC61.R4 REGIONAL STRATEGY TO STOP TUBERCULOSIS IN THE WESTERN PACIFIC (2011–2015)

The Regional Committee, Acknowledging reports that indicate that the Western Pacifi c Region is on track to achieve the regional goal of halving tuberculosis prevalence and mortality, thus contributing to the achievement of the Millennium Development Goals; REPORT OF THE REGIONAL COMMITTEE 11

Appreciating that the fi rm political commitment of Member States and the efforts of international partners made it possible to implement the Strategic Plan to Stop Tuberculosis in the Western Pacifi c (2006–2010); Realizing that further effort is needed to ensure universal access to diagnosis, treatment and care for all people suffering from tuberculosis; Recognizing the emergence and spread of drug-resistant tuberculosis, and particularly multidrug- resistant tuberculosis, pose serious public health threats; Expressing further concern that the HIV/AIDS epidemic may reverse gains achieved in tuberculosis control,

1. ENDORSES the Regional Strategy to Stop Tuberculosis in the Western Pacifi c (2011–2015); 1

2. URGES Member States:

(1) to develop and/or update national tuberculosis strategic plans, consistent with the Regional Strategy to Stop Tuberculosis in the Western Pacifi c (2011–2015);

(2) to strive for universal and equitable access to diagnosis, treatment and care for all people suffering from tuberculosis by strengthening public-private partnerships, by implementing case-fi nding approaches that target high-risk populations, and by addressing access barriers to tuberculosis care for vulnerable populations;

(3) to strengthen tuberculosis laboratory capacity to diagnose susceptible and drug-resistant tuberculosis using, where appropriate, the most effective latest technologies;

(4) to scale up the health systems management of drug-resistant tuberculosis by developing and implementing comprehensive plans with political commitment and fi nancial support;

(5) to develop and implement a comprehensive policy framework for collaborative

TB-HIV activities; (6) to strengthen tuberculosis programme management capacity through suffi cient fi nancial and human resources;

3. REQUESTS the Regional Director:

(1) to work closely with Member States and partner organizations to sustain support for tuberculosis control;

(2) to provide support to Member States in developing and updating national tuberculosis strategic plans, using the Regional Strategy to Stop Tuberculosis in the Western Pacifi c (2011–2015) as a framework;

(3) to provide technical support to Member States to strengthen evidence-based policy development.

The Regional Committee, Recalling resolutions WHA59.2 on Application of the International Health Regulations (2005) and WHA61.2 on the implementation of the International Health Regulations (2005); Recalling resolutions WPR/RC56.R4 on the Asia Pacifi c Strategy for Emerging Diseases (APSED), WPR/RC57.R2 on APSED, the International Health Regulations (2005) and avian infl uenza, and WPR/ RC58.R3 on avian and pandemic infl uenza, the International Health Regulations (2005) and APSED; Aware that APSED has proved to be a useful framework for Member States, WHO and partners to work collectively to ensure that all the countries and areas of the Western Pacifi c Region are better prepared in confronting emerging disease threats; Recognizing the continuing threats to regional health security arising not only from emerging diseases, but also other public health events and emergencies in Asia and the Pacifi c; Acknowledging the experiences and lessons learnt from pandemic (H1N1) 2009, including the value of investments in routine capacity-building for emerging diseases, the need for pandemic preparedness and risk communications; Recognizing the urgent need to continue to strengthen and maintain effective national and regional systems and capacities to rapidly detect, assess and respond to public health events, in particular those caused by emerging diseases; Further recognizing that the progress made in strengthening emerging disease surveillance and response now provides a solid foundation for early detection of and rapid response to other public health events; Reaffi rming the importance and value of a common framework for effective planning and coordination of various capacity-building activities at both national and regional levels; Having reviewed the updated draft Asia Pacifi c Strategy for Emerging Diseases (2010),

(1) to use the Asia Pacifi c Strategy for Emerging Diseases (2010) (APSED 2010) as a strategic framework to guide national and local capacity-building programmes for emerging diseases and public health events;

(2) to use APSED (2010) as a tool when developing relevant national plans to ensure effective preparedness for emerging diseases and the development and strengthening of the core capacities required under the International Health Regulations (2005), including enhanced surveillance and response;

(3) to strengthen or sustain political commitment, mobilize human and fi nancial resources to facilitate effective implementation of national plans in line with APSED (2010);

(4) to actively participate in regional preparedness, alert and response systems and networks;

(2) to advocate for and disseminate APSED (2010) to partner agencies to mobilize, coordinate and harmonize partner support in strengthening national and regional capacities to deal with emerging diseases and other public health emergencies;

The Regional Committee, Noting that progress has been achieved in implementing resolution WPR/RC49.R6 on Healthy Cities and Healthy Islands; Acknowledging that healthy settings approaches can offer effective ways to address public health priorities and integrate health promotion and health protection; Reaffi rming that factors such as political commitment, community support, multisectoral participation, local ownership, and social, environmental and economic determinants of health contribute greatly to successful Healthy Cities and Healthy Islands programmes; Recognizing that the Healthy Cities approach has been widely applied in many countries and areas through the Alliance for Healthy Cities, and the vision of Healthy Islands has been promoted by Pacifi c island health ministers and implemented in many communities in the Pacifi c; Noting, further, that recent rapid economic, environmental and social changes call for scaling up and revitalizing the Healthy Cities and Healthy Islands approaches, 1. URGES Member States:

(1) to advocate for Healthy Cities and Healthy Islands as multisectoral approaches to establish partnerships between health and other development sectors;

WPR/RC61.R7 VACCINE PREVENTABLE DISEASES: MEASLES ELIMINATION, HEPATITIS B CONTROL, AND POLIOMYELITIS ERADICATION

The Regional Committee, Noting resolution WPR/RC56.R8 that called for the elimination of measles by 2012 and a reduction in the seroprevalence of hepatitis B surface antigen (HBsAg) to less than 2% in 5-year-old children by 2012 as an interim milestone towards the fi nal regional goal of less than 1% HBsAg; Aware of the ongoing risk that importations of wild poliovirus pose to the maintenance of poliomyelitis- free status in the Western Pacifi c Region; Noting resolution WPR/RC54.R3 that calls for the use of measles elimination and hepatitis B control strategies as means to strengthen the Expanded Programme on Immunization and other public health programmes, such as those intended to prevent the spread of congenital rubella syndrome; Mindful of the positive impact of poliomyelitis eradication, measles elimination and hepatitis B control strategies in the Western Pacifi c Region on child health, immunization systems and overall health; Recognizing that many countries and areas have made dramatic progress towards the achievement of the 2012 goals of measles elimination and the reduction in seroprevalence of HBsAg to less than 2%, but that many others face substantial fi nancial and operational challenges to achieve the 2012 goals, 1. REAFFIRMS the 2012 measles elimination goal and the hepatitis B control goal and milestone, and the maintenance of poliomyelitis-free status; REPORT OF THE REGIONAL COMMITTEE 15

2. URGES Member States:

(1) to commit the human and fi nancial resources necessary to achieve and sustain the measles elimination and hepatitis B control goals, and to maintain poliomyelitis-free status;

(2) to develop and implement workplans to ensure high immunization coverage against measles, hepatitis B and poliomyelitis, and sensitive and timely epidemiologic and laboratory surveillance to achieve measles elimination and maintain poliomyelitis-free status;

(3) to report measles and poliomyelitis, and where feasible rubella, surveillance data to the Regional Offi ce in a regular and timely manner;

(4) to establish an independent national verifi cation process for measles elimination following the establishment by the WHO Regional Offi ce for the Western Pacifi c of standardized regional verifi cation mechanisms;

(5) to accelerate control of rubella and the prevention of congenital rubella syndrome;

(6) to vigorously implement all activities to maintain poliomyelitis-free status;

3. REQUESTS the Regional Director:

(1) to establish regional verifi cation mechanisms for measles elimination;

(2) to strengthen technical cooperation with Member States to achieve regional immunization goals;

(3) to seek additional resources to achieve regional goals utilizing frequent interagency coordination committee meetings and other mechanisms;

(4) to report progress periodically to the Regional Committee.

I. INTRODUCTION AND SUMMARY

The sixty-fi rst session of the Regional Committee for the Western Pacifi c was held in Putrajaya, Malaysia, from 11 to 14 October 2010. The session was attended by representatives of Australia, Brunei Darussalam, Cambodia, China, Cook Islands, Fiji, Hong Kong (China), Japan, Kiribati, the Lao People’s Democratic Republic, Macao (China), Malaysia, the Marshall Islands, Mongolia, Nauru, New Zealand, Niue, Palau, Papua New Guinea, the Philippines, the Republic of Korea, Samoa, Singapore, Solomon Islands, Tokelau, Tonga, Tuvalu, Vanuatu and Viet Nam, and by representatives of France and the United States of America as Member States responsible for areas in the Region.

At the opening ceremony, the retiring Chairperson of the Regional Committee noted the achievements of the Region’s Member States over the previous year, especially in standing up against pandemic infl uenza H1N1 (2009), leading the fi ght against tobacco, promoting healthier lifestyles and protecting health and the environment in a sustainable and context-relevant manner. The Regional Director expressed special appreciation to the Government of Malaysia for hosting the Regional Committee on short notice. He called for renewed leadership in dealing with the issues on the agenda, particularly further actions to meet the commitments towards achievement of the United Nations Millennium Development Goals. The Deputy Director-General noted the foresight that had been shown in establishment of the Regional Committee, with shared health concerns as its basis, and the even greater interconnectivity of countries today. As the most diversifi ed of all the WHO regions, the Western Pacifi c was more representative of the world and its Regional Committee’s discussions were therefore of special importance. The Minister of Health of Malaysia, Mr Liow Tiong Lai, offered the Committee the hospitality of the multicultural community of Malaysia and expressed his hope that the session would be a productive and successful forum. The Prime Minister of Malaysia, the Honourable Mohd Najib bin Tun Abdul Razak, welcomed the participants and noted that the Regional Committee was an important forum for health ministers and experts in the Region. The sixty-fi rst session was particularly signifi cant as it gave an opportunity to examine the response to the effects of pandemic infl uenza H1N1 (2009) and could provide input into future global actions.

The retiring Chairperson and the Director-General (by video) addressed the Committee. The Regional Director then presented his report on the work of WHO in the Region for the period from 1 July 2009 to 30 June 2010. Several representatives congratulated the Regional Director on his comprehensive report. Other comments fell into the following main categories: health system strengthening; preparedness for pandemic and emerging diseases, including WHO’s response to the pandemic H1N1 (2009); and progress towards achievement of the health-related Millennium Development Goals.

The second meeting began with an address by the incoming Chairperson. The fi nal report on budget performance for the Programme Budget 2008–2009 was then examined. The Director, Programme Management, explained that implementation of assessed contributions had amounted to US$ 79.2 million or 99.8% of the fi nal working allocation, while implementation of voluntary contributions, which had increased by 36.7% compared with the previous biennium, had amounted to US$ 150.8 million or 70.4% of available resources. The Western Pacifi c Region had continued the trend of implementing at a higher level in countries. The performance assessment of the Programme Budget 2008–2009 had been the fi rst to be carried out within the framework of the Medium-term Strategic Plan (2008–2013). A systematic review of WHO workplans had led to a series of organizational changes, including a reduction in the number of technical units from 31 to 17 and the introduction of Strategic Frameworks at the country and regional levels.

Commenting on the report, representatives praised the clearer presentation of outputs and results of the Programme Budget 2008–2009 and their appraisal as regards achievement. They also commended the organizational changes that had been made. While the improved budget implementation was noted, there were calls for analysis of the reasons for only partial achievement of some objectives. The Director, Programme Management, responding to comments, said that another element would be added to the implementation analysis, based on the indicators in the Strategic Frameworks, and that the Regional Offi ce would broaden its search for donors to fi nd support for underfunded areas (decision WPR/RC61(1)).

The Director, Programme Management, then presented the Proposed Programme Budget 2012–2013, informing the Committee that it would be developed further in close collaboration with country offi ces and WHO Headquarters and aligned with the fi nal Organization-wide Proposed Programme Budget 2012–2013. The overall format was similar to the 2010–2011 budget, with refi nements that refl ected shifts in emphasis, better alignment with country priorities and steps taken to address the gap between the previous programme budgets and actual implementation rates. The Director, Department of Planning, Resource Coordination and Performance Monitoring, WHO Headquarters, explained that the proposed Organization—wide Programme Budget 2012–2013 aimed to make a timely contribution to achievement of the Millennium Development Goals, improving performance at the country level, increasing work on noncommunicable diseases, contributing to health governance, and responding better to Member States’ demands for greater transparency and accountability. The 13 strategic objectives had remained largely unchanged, with no new Organization- wide expected results. Several representatives, while welcoming the shift to a fully integrated budget and the greater integration of objectives across programmes, raised the possibility of reducing or eliminating earmarking and having greater fl exibility in allocation, especially as regards voluntary contributions (resolution WPR/RC61.R1).

The Regional Committee was then asked to consider the draft Western Pacifi c Regional Strategy on Health Systems Based on the Values of Primary Health Care, which was discussed at the second and third meetings. The Regional Director said that the draft Strategy had been developed through extensive REPORT OF THE REGIONAL COMMITTEE 5

consultation with Member States, involving interviews in 28 countries, and had undergone expert review. The representatives were also asked to consider the draft global Framework for National Health Strategies, Policy and Plans, drafted at WHO Headquarters. The Deputy Director-General explained that renewed emphasis was being given to health policies and plans as research had shown that a strong national health agenda was a major factor in achieving greater health gains. The representatives who spoke on the item were strongly supportive of the draft Strategy and the global Framework, a well as the renewed focus on primary health care (resolution WPR/RC61.R2).

The Committee then turned its attention to women’s health. The Regional Director said that the discussion paper summarized the issues and challenges in the area of women’s health, covering two aspects: areas where, without special services, women would be especially vulnerable; and social, economic and cultural situations in which women’s health was worse than men’s. Many representatives, supporting the renewed commitment to women’s health and agreeing with the need for women-friendly and gender-sensitive policies, stressed the importance of such determinants of women’s health as education, employment and political empowerment. They also outlined the situation as regards women in their countries. The Director, Building Healthy Communities and Populations, noting the representatives’ emphasis on the need for monitoring, said that WHO would strengthen the collection of information on violence against women to support Member States in addressing that issue. The Regional Director told representatives that a cross-cutting working group had been set up at the Regional Offi ce to facilitate the collection of data from all programme areas, which would be used to prepare a comprehensive report on all aspects of women’s health in the Region.

The fourth meeting began with examination of issues that had emerged in an informal consultation at WHO Headquarters that had focused on: (1) the future role and function of WHO; and (2) the future fi nancing of WHO. The Assistant Director-General for General Management informed the Committee that appropriate proposals would be drafted, based on consultations with Member States, and presented to the Executive Board for its consideration. Representatives agreed on the need for redetermination of WHO’s role and key functions, the general consensus being that the Organization should strengthen its role as the recognized global authority in health matters, setting standards, guidelines and recommendations. WHO’s leadership was indispensible in health security. As regards fi nancing, there was general consensus concerning the need for better prioritization and coordination, as well as improved fi nancial management.

The Regional Committee was then asked to discuss the report on healthy settings. The Regional Director noted that rapid urbanization and globalization had led to changes in the determinants of health and that healthy settings approaches, such as Healthy Cities and Healthy Islands, needed to be scaled up. Representatives welcomed the report and the expansion of the healthy settings approach, and shared experiences in their countries. Many successes with Healthy Cities and Healthy Islands initiatives were reported, but the need for multisectoral collaboration and community involvement was emphasized (resolution WPR/RC61.R6).

Consideration was then given to the draft Regional Strategy to Stop Tuberculosis in the Western Pacifi c (2011–2015), which was discussed in the fourth and fi fth meetings. The Regional Director told the Committee that the Western Pacifi c Region had reason to be proud of its achievements in controlling tuberculosis (TB). The draft Strategy was meant to ensure that all countries in the Region could reach and care for all TB patients at an early stage of their disease. Its endorsement would be an important step towards modernization of TB control in the Region. Representatives, indicating their support for the draft Strategy, described the TB situation in their countries and progress in controlling the disease (resolution WPR/RC61.R4). 6 REGIONAL COMMITTEE: SIXTY-FIRST SESSION

The Committee was then asked to review and consider endorsing the draft Asia Pacifi c Strategy for Emerging Diseases (2010). The Director, Programme Management noted that the fi rst Asia Pacifi c Strategy for Emerging Diseases (APSED), which had been endorsed in 2005, had been used extensively by WHO and Member States to strengthen capacity. As it was nearing the end of its lifespan, WHO had consulted with Member States to produce APSED (2010), which took into account the lessons learnt during the H1N1 pandemic in 2009. The many representatives who spoke attested to the usefulness of APSED (2005) and endorsed the new APSED (2010) (resolution WPR/RC61.R5).

The report on vaccine-preventable diseases was discussed at the fi fth and sixth meetings. The Director, Programme Management, reminded the Committee that, although it had been 10 years since the declaration of the Region’s poliomyelitis-free status, it was still necessary to remain vigilant. The Region was also continuing to work hard to achieve measles elimination and hepatitis B control by 2012, but the report pointed to the need for renewed commitment and outlined the steps to be taken if those goals were to be achieved (resolution WPR/RC61.R7).

The Director, Programme Management, then presented progress reports on three areas of work: achievement of the Millennium Development Goals; dengue fever and dengue haemorrhagic fever prevention and control; and protecting health from the effects of climate change. In the discussions that followed, representatives outlined the situation in their countries as regards the areas under review.

The Committee then turned its attention to the procedures for the nomination of the Regional Director. The Legal Counsel presented his report, containing a number of proposals for a cost-effective interview process that would enable all Member States of the Region to familiarize themselves with the candidates, as well as proposals on the possible format and content of a code of conduct for Member States during the nomination process. There was general consensus in favour of an interview process being conducted, with most favouring interviews at the session of the Regional Committee (resolution WPR/RC61.R3).

The seventh meeting began with examination of a number of resolutions adopted at the Sixty-third World Health Assembly that were of particular interest to the Western Pacifi c Region. Those included: WHA63.14, on the marketing of food and non-alcoholic beverages to children; WHA63.16, on the WHO Global Code of Practice on the International Recruitment of Health Personnel; WHA63.19, on the WHO Global HIV/ AIDS Strategy for 2011-2015; and WHA63.28, on the establishment of a consultative expert working group on research and development: fi nancing and coordination. The Committee nominated the following experts in the Region as candidates to participate in the Consultative Expert Working Group on Research and Development: Financing and Coordination: Professor Guo Yan (China); Mr Shozo Uemura (Japan); Dr Chansaly Phommavong (Lao People’s Democratic Republic); and Dr Leizel Lagrada (Philippines) (decision WPR/RC61 (2)).

The Committee next selected Malaysia to serve on the Policy and Coordination Committee of the Special Programme of Research, Development and Research Training in Human Development (decision WPR/RC61 (3)), and the Lao People’s Democratic Republic as one of the representatives of the Region to serve on the Joint Coordinating Board of the Special Programme for Research and Training in Tropical Diseases (decision WPR/RC61 (4)).

Regarding future sessions, the Regional Committee agreed that the sixty-second session would be held from 19 to 23 September 2011 at the Regional Offi ce in Manila (resolution WPR/RC61.R8). REPORT OF THE REGIONAL COMMITTEE 7

The Regional Committee, Mindful that a health system consists of all organizations, people and actions intended to promote, restore or maintain health, and that a good health system delivers effective, safe and quality interventions, when and where needed;

Recognizing that the enjoyment of the highest attainable standard of health is one of the fundamental rights of every human being and that health systems are an important contributor to the realization of that right; Acknowledging that strengthened health systems contribute to the realization of the right to the highest attainable standard of health and the achievement of global health goals, such as the Millennium Development Goals; Reaffi rming that strong health systems based on the values of primary health care and focused on a vision of providing universal coverage for quality health services can be an effi cient and effective way to contribute to improved and equitable health outcomes; Noting that the values of primary health care to be considered for health systems, as contained in the Western Pacifi c Regional Strategy for Health Systems Based on the Values of Primary Health Care, include equity, social justice, universality, people-centredness, community protection, participation, scientifi c soundness, personal responsibility, self-determination and self-reliance; Further noting that there are existing strategies in the areas of health fi nancing, laboratory services, access to essential medicines, human resources for health, noncommunicable diseases and emerging diseases, and that these are consistent with the Western Pacifi c Regional Strategy for Health Systems Based on the Values of Primary Health Care; Recognizing that the Regional Strategy builds upon and is consistent with The World Health Report 2000—Health Systems: Improving Performance; The World Health Report 2006: Working Together for Health; Everybody’s Business: Strengthening Health Systems to Improve Health Outcomes; and The World Health Report 2008 — Primary Health Care: Now More Than Ever; Acknowledging that health systems are complex and diverse and that no single model of health systems strengthening is suitable for all countries and areas, 1. (3) to disseminate, as appropriate, to relevant stakeholders the core elements of the Regional Strategy;

(1) to provide technical cooperation as requested by Member States to facilitate implementation of the strategy;

(2) to work with Member States to develop and further refi ne indicators and guidelines for health systems;

WPR/RC61.R3 PROCEDURE FOR THE NOMINATION OF THE REGIONAL DIRECTOR

The Regional Committee, Having considered the report by the Legal Counsel on the procedure for nomination of the Regional Director; Wishing to improve the fairness of the process for the nomination of the Regional Director; Having considered the practice followed within the World Health Organization for the nomination of other elected offi cials, 1. APPROVES the amendments to Rule 51 of the Rules of Procedure of the Regional Committee contained in Annex 3 to the report by the Legal Counsel (WPR/RC61/11), which will become effective at the end of the sixty-fi rst session of the Committee;

2. DECIDES, with regard to the interviews of candidates for the post of Regional Director, that:

(1) the order in which candidates will be interviewed shall be drawn by lot;

(2) the interviews shall be limited to 60 minutes, equally divided between (i) an oral presentation of the candidate’s vision of the future priorities for the Region with an analysis of current problems facing it and suggestions as to how those should be addressed; and (ii) a question-and-answer period.

WPR/RC61.R4 REGIONAL STRATEGY TO STOP TUBERCULOSIS IN THE WESTERN PACIFIC (2011–2015)

The Regional Committee, Acknowledging reports that indicate that the Western Pacifi c Region is on track to achieve the regional goal of halving tuberculosis prevalence and mortality, thus contributing to the achievement of the Millennium Development Goals; REPORT OF THE REGIONAL COMMITTEE 11

Appreciating that the fi rm political commitment of Member States and the efforts of international partners made it possible to implement the Strategic Plan to Stop Tuberculosis in the Western Pacifi c (2006–2010); Realizing that further effort is needed to ensure universal access to diagnosis, treatment and care for all people suffering from tuberculosis; Recognizing the emergence and spread of drug-resistant tuberculosis, and particularly multidrug- resistant tuberculosis, pose serious public health threats; Expressing further concern that the HIV/AIDS epidemic may reverse gains achieved in tuberculosis control,

(2) to strive for universal and equitable access to diagnosis, treatment and care for all people suffering from tuberculosis by strengthening public-private partnerships, by implementing case-fi nding approaches that target high-risk populations, and by addressing access barriers to tuberculosis care for vulnerable populations;

(3) to strengthen tuberculosis laboratory capacity to diagnose susceptible and drug-resistant tuberculosis using, where appropriate, the most effective latest technologies;

(4) to scale up the health systems management of drug-resistant tuberculosis by developing and implementing comprehensive plans with political commitment and fi nancial support;

(5) to develop and implement a comprehensive policy framework for collaborative

TB-HIV activities; (6) to strengthen tuberculosis programme management capacity through suffi cient fi nancial and human resources;

3. REQUESTS the Regional Director:

(1) to work closely with Member States and partner organizations to sustain support for tuberculosis control;

(2) to provide support to Member States in developing and updating national tuberculosis strategic plans, using the Regional Strategy to Stop Tuberculosis in the Western Pacifi c (2011–2015) as a framework;

The Regional Committee, Noting that progress has been achieved in implementing resolution WPR/RC49.R6 on Healthy Cities and Healthy Islands; Acknowledging that healthy settings approaches can offer effective ways to address public health priorities and integrate health promotion and health protection; Reaffi rming that factors such as political commitment, community support, multisectoral participation, local ownership, and social, environmental and economic determinants of health contribute greatly to successful Healthy Cities and Healthy Islands programmes; Recognizing that the Healthy Cities approach has been widely applied in many countries and areas through the Alliance for Healthy Cities, and the vision of Healthy Islands has been promoted by Pacifi c island health ministers and implemented in many communities in the Pacifi c; Noting, further, that recent rapid economic, environmental and social changes call for scaling up and revitalizing the Healthy Cities and Healthy Islands approaches, 1. WPR/RC61.R7 VACCINE PREVENTABLE DISEASES: MEASLES ELIMINATION, HEPATITIS B CONTROL, AND POLIOMYELITIS ERADICATION

The Regional Committee, Noting resolution WPR/RC56.R8 that called for the elimination of measles by 2012 and a reduction in the seroprevalence of hepatitis B surface antigen (HBsAg) to less than 2% in 5-year-old children by 2012 as an interim milestone towards the fi nal regional goal of less than 1% HBsAg; Aware of the ongoing risk that importations of wild poliovirus pose to the maintenance of poliomyelitis- free status in the Western Pacifi c Region; Noting resolution WPR/RC54.R3 that calls for the use of measles elimination and hepatitis B control strategies as means to strengthen the Expanded Programme on Immunization and other public health programmes, such as those intended to prevent the spread of congenital rubella syndrome; Mindful of the positive impact of poliomyelitis eradication, measles elimination and hepatitis B control strategies in the Western Pacifi c Region on child health, immunization systems and overall health; Recognizing that many countries and areas have made dramatic progress towards the achievement of the 2012 goals of measles elimination and the reduction in seroprevalence of HBsAg to less than 2%, but that many others face substantial fi nancial and operational challenges to achieve the 2012 goals, 1. REAFFIRMS the 2012 measles elimination goal and the hepatitis B control goal and milestone, and the maintenance of poliomyelitis-free status; REPORT OF THE REGIONAL COMMITTEE 15

2. URGES Member States:

(1) to commit the human and fi nancial resources necessary to achieve and sustain the measles elimination and hepatitis B control goals, and to maintain poliomyelitis-free status;

(2) to develop and implement workplans to ensure high immunization coverage against measles, hepatitis B and poliomyelitis, and sensitive and timely epidemiologic and laboratory surveillance to achieve measles elimination and maintain poliomyelitis-free status;

(3) to report measles and poliomyelitis, and where feasible rubella, surveillance data to the Regional Offi ce in a regular and timely manner;

(4) to establish an independent national verifi cation process for measles elimination following the establishment by the WHO Regional Offi ce for the Western Pacifi c of standardized regional verifi cation mechanisms;

(5) to accelerate control of rubella and the prevention of congenital rubella syndrome;

(6) to vigorously implement all activities to maintain poliomyelitis-free status;

3. REQUESTS the Regional Director:

(1) to establish regional verifi cation mechanisms for measles elimination;

(2) to strengthen technical cooperation with Member States to achieve regional immunization goals;

(3) to seek additional resources to achieve regional goals utilizing frequent interagency coordination committee meetings and other mechanisms;

(4) to report progress periodically to the Regional Committee.
