Neonatal Jaundice
Neonatal jaundice is a yellowing of the skin and other tissues of a newborn infant. A bilirubin level of more than 85 umol/l (5 mg/dL) manifests clinical jaundice in neonates whereas in adults a level of 34 umol/l (2 mg/dL) would look icteric. In newborns jaundice is detected by blanching the skin with digital pressure so that it reveals underlying skin and subcutaneous tissue. Jaundice newborns have an apparent icteric sclera, and yellowing of the face, extending down onto the chest. This condition is common in upwards of 80% of newborns.
In neonates the dermal icterus is first noted in the face and as the bilirubin level rises proceeds caudal to the trunk and then to the extremities.
Normally, about 1% of our red blood cells retire every day, to be replaced by fresh red blood cells. The old ones are processed in the Liver and disposed in the form of billirubin and the billirubin will leaves The body through stool or urine. In the case of premature baby the liver may not matured for process the Bilirubin and the Bilirubin content in the blood will increase more than 2 mg/dl and causes Jaundice
Causes
1. Increase bilirubin load on liver cells:
Increased red blood cell (RBC) volume Increased labeled bilirubin Increased circulation of bilirubin in the liver Decreased RBC survival
2. Defective hepatic uptake of bilirubin from blood plasma: Decreased ligadin (Y protein) Increased binding of Y proteins by other anions Decreased liver uptake especially in phase two
3. Defective billirubin conjugation:
Decreased UDPG activity
4. Defective bilirubin excretion
Phototherapy
Phototherapy is the conventional and easy way of treatment for jaundice, a common condition in newborn infants caused by high levels of seric bilirubin, which may cause chronic bilirubin encephalopathy (kernicterus). Phototherapy treatments decrease the bilirubin levels in the blood by changing the trans-bilirubin into cis-bilirubin isomer, which is water-soluble. Phototherapy exposure blue light ( 420- 470 nm ) causes photoisomerization of billirubin deposited at the skin surface, and conversion to a structural isomer called lumirubin. Lumirubin is rapidly excreted in the bile

Different Phototherapy Treatments
There are different methods of phototherapy treatment for neonatal jaundice. They are Blue Tube Phototherapy Blue CFL Phototherapy Halogen photherapy LED Phototherapy
Blue Tube Phototherapy In conventional phototherapy specially made blue tube ( Philips TL /52 .20 W) are using as the blue light source. In this method Four blue tube from top side and four blue tube from the bottom side are using for get max light intensity , Two white tubes are attached in each side for observation application.
Disadvantages of Blue tube Phototherapy
Low Irriadance – 10 to 12mW/cm²/nm Its emit wide band wavelength , so It emits Ultra violet and Infra red content along with blue light Ultra violet rays Causes Skin rashes In Newborns Infra red causes possibility of Hyperthermia in baby and it causes fluid losses . 360o viewing angle of blue tube causes headache in nursing staff and care takers Bulb life – 2000 Hrs , bulb cost Large size of the unit Power consumption
CFL Phototherapy
CFL Phototherapy is a second generation phototherapy device . The only advantage they offered is its compact size compare to conventional method CFL also producing wide band including ultraviolet and Infra red along with Blue light Low irriadiance – 18 to 24mW/cm²/nm Halogen Phototherapy
It’s a rarely using method , Spot halogen lamps are using as light source. Hyperthermia is the disadvantage of Halogen Phototherapy
LED Phototherapy
Phototherapy is the most effective treatment when the light used is around 450nm of the light spectrum wavelength, which corresponds to the blue color. LEDs provide light precisely at this wavelength and have an abrupt attenuation of the irradiance from the infra-red and ultra- violet wavelength ranges, reducing undesired effects to the infant's skin. The radiance levels are higher, which decreases treatment time considerably. Also, the power consumption and the heating caused by the LEDs are very low when compared with other sources of light.

I Rex LED Phototherapy is truly a next generation phototherapy unit. This machine is unique and innovative compared to all other machines. I Rex LED Phototherapy machine has unique 4 modes of operation, which makes more user-friendly in operating. The four modes of the operation are
Swift Mode - It is a preset mode with 35 mW at 30 cm distance , ideal for patient having more than 15 mg/dl. Recovery Mode - It is also preset mode with 15 mW, it will be useful for patients who required low intensity. It is called Advanced Phototheraphy, that is baby with 5mg/dl can take Phototheraphy treatment in this mode Auto Mode - User can set the treatment time. In between if the mother taking the baby for feeding means, they can switch off the machine, and when they are again switch it on, it will start from the time they stopped.
