The aim of this study was to determine the impact of catastrophizing, anxiety, and depression on in-hospital opioid consumption, pain scores, and quality of recovery in adults who underwent spine surgery. METHODS Patients undergoing spine surgery were enrolled in this study, and the preoperatively completed question- naires included the verbal rating scale (VRS), Pain Catastrophizing Scale (PCS), Hospital Anxiety and Depression Scale (HADS), and Oswestry Disability Index (ODI). Quality of recovery was assessed using the 40-item Quality of Recovery questionnaire (QoR40). Opioid consumption and pain scores according to the VRS were recorded daily until discharge. RESULTS One hundred thirty-nine patients were recruited for the study, and 101 completed the QoR40 assessment postoperatively. Patients with higher catastrophizing scores were more likely to have higher maximum pain scores postoperatively (estimate: 0.03, SE: 0.01, p = 0.02), without increased opioid use (estimate: 0.44, SE: 0.27, p = 0.11). Preoperative anxiety (estimate: 1.18, SE: 0.65, p = 0.07) and depression scores (estimate: 1.06, SE: 0.71, p = 0.14) did not correlate with increased postoperative opioid use; however, patients with higher preoperative depression scores had lower quality of recovery after surgery (estimate: -1.9, SE: 0.56, p < 0.001). CONCLUSIONS Catastrophizing, anxiety, and depression play important roles in modulating postoperative pain. Preop- erative evaluation of these factors, utilizing a validated tool, helps to identify patients at risk. Methods: A computer search using PubMed and the Rehabilitation Reference Center generated articles relevant to pain catastrophizing, depression and multidisciplinary pain treatment programs. Discussion: “Pain catastrophizing is characterized by the tendency to magnify the threat value of pain stimulus and to feel helpless in the context of pain, and by a relative inability to inhibit pain-related thoughts in anticipation of, during or following a painful encounter” (6). The author then reviews the current assessment measurements of pain catastrophizing. Then the author discusses the proposed mechanisms of pain catastrophizing; cognitive, psychosocial, central nervous system mechanisms, cortical changes and physiological. The author concludes the review with a discussion of the importance on implementing a multidisciplinary treatment program for pain that involves a cognitive-behavioral approach. Conclusion: The current literature recognizes a significant correlation between pain catastrophizing and depression that exists within the general population. The researchers in this field have developed assessment tools to quantify a measure of pain catastrophizing. In addition, several mechanisms have been proposed to explain the pain catastrophizing experience. The researchers describe a mutually inclusive model of pain catastrophizing that includes cognition, psychosocial, central nervous system changes, neural correlates and physiological responses. Furthermore, the literature states the importance and effectiveness of implementing a multidisciplinary approach to treating chronic pain that includes a cognitive-behavioral approach.

Linear mixed effects analyses revealed a negative relationship between pain catastrophizing and beta information flow from the right temporal cortex to the frontal regions and a positive relationship between pain catastrophizing and increased beta information flow from the right somatosensory cortices to the right temporal cortices when thinking about pain. These patterns were not found in other imagery conditions. Taken together, this study suggests that individual differences in pain catastrophizing might be related to an altered frontotemporal regulatory loop and increased connectivity between pain and affective systems.

    Research Paper Experimental reduction of pain catastrophizing modulates pain report but not spinal nociception as verified by mediation analyses Ellen L. Terry, Kathryn A. Thompson, Jamie L. Rhudy* Abstract Pain catastrophizing is associated with enhanced pain; however, the mechanisms by which it modulates pain are poorly understood. Evidence suggests that catastrophizing modulates supraspinal processing of pain but does not modulate spinal nociception (as assessed by nociceptive flexion reflex [NFR]). Unfortunately, most NFR studies have been correlational. To address this, this study experimentally reduced catastrophizing to determine whether it modulates spinal nociception (NFR). Healthy pain- free participants (N 5 113) were randomly assigned to a brief 30-minute catastrophizing reduction manipulation or a control group that received pain education. Before and after manipulations, 2 types of painful stimuli were delivered to elicit (1) NFR (single trains of stimuli) and (2) temporal summation of NFR (3 stimulations at 2 Hz). After each set of stimuli, participants were asked to report their pain intensity and unpleasantness, as well as their situation-specific catastrophizing. Manipulation checks verified that catastrophizing was effectively reduced. Furthermore, pain intensity and unpleasantness to both stimulation types were reduced by the catastrophizing manipulation, effects that were mediated by catastrophizing. Although NFRs were not affected by the catastrophizing manipulation, temporal summation of NFR was reduced. However, this effect was not mediated by catastrophizing. These results indicate that reductions in catastrophizing lead to reductions in pain perception but do not modulate spinal nociception and provides further evidence that catastrophizing modulates pain at the supraspinal, not the spinal, level.
