It is a great orthopedic technical challenge to effectively reduce the accompanying pain and to help patients regain and keep their mobility. Due to difficult therapies and poor compliance, a surgical intervention to brace the spine is usually the first therapeutic choice. This article presents two cases in which individualized torso orthoses were successfully used to treat patients with degenerative diseases and disorders of the sagittal line as well as three dimensional deformities of the spine. Using torso orthoses allows treatment of these patients with as few invasive measures as possible without losing maximal functionality. Bones and Joints in Marfan Syndrome
BONES AND JOINTS IN MARFAN SYNDROME Marfan syndrome often causes problems in the bones and joints—in fact, these are often the features that first lead a person to suspect Marfan syndrome and seek a diagnosis. These features (called skeletal features) happen when bones grow extra-long or ligaments (connective tissue that holds joints together) become stretchy—like loose rubber bands. Only about one-third of people with Marfan syndrome have skeletal features so severe that they require treatment. There are several skeletal features associated with Marfan syndrome. Many people with Marfan syndrome have more than one skeletal feature, but very few people have them all. While it is important for the skeletal features to be evaluated by an orthopedist (bone and joint doctor), only about one-third of people with Marfan syndrome have skeletal features so severe that they require treatment. What are the common types of bone and joint problems in people with Marfan syndrome? Here are some facts about common types of bone and joint problems in people with Marfan syndrome: General Body Type A person with Marfan syndrome will usually—but not always—be tall, slender, and somewhat loose-jointed or limber. The arms, legs, fingers, and toes may be disproportionately long when compared with the trunk. In some cases, they may not appear tall compared to the general public, but instead be tall for their family. Oscar may delegate utilization management decisions of certain services to third-party delegates, who may develop and adopt their own clinical criteria. Clinical guidelines are applicable to certain plans. Clinical guidelines are applicable to members enrolled in Medicare Advantage plans only if there are no criteria established for the specified service in a Centers for Medicare & Medicaid Services (CMS) national coverage determination (NCD) or local coverage determination (LCD) on the date of a prior authorization request. Services are subject to the terms, conditions, limitations of a member’s policy and applicable state and federal law. Please reference the member’s policy documents (e.g., Certificate/Evidence of Coverage, Schedule of Benefits) or contact Oscar at 855-672-2755 to confirm coverage and benefit conditions. Summary Oscar members with conditions affecting the back or spine may be eligible for coverage of special types of back braces known as spinal orthoses. Spinal orthoses function by providing a rigid structure to stabilize, align, and/or support the spine. These devices can be used for a number of conditions, including pain, deformities, and for rehabilitation after certain surgical procedures. Spinal orthoses come in several levels of customization, ranging from pre-fabricated to fully customized braces. They are also categorized based on which region(s) of the back they support (e.g., lumbosacral, thoracolumbosacral, etc.). These devices are considered durable medical equipment and must be prescribed by a licensed physician. -
