THE COMBINED EFFECT OF STJLFANILAMIDE AND PENICILLIN IN TREATMENT OF EXPERIMENTAL ERYSIPELOTHRIX RHUSIOPATHIAE INFECTION OF MICE* JOSEPH V. KLAUDER, M.D. AND ANNA M. RULE In a previous communication (1) we reported the results of determinations oi the therapeutic effect of sulfonamide compounds in mice inoculated with Er- ysipelothrix rhusiopathiae. A report was also made of the ineffective use of these compounds in treatment of patients with erysipeloid of Rosenbach and in treatment of one patient with the septicemic form of the infection (2). In our experimental study sulfanilamide, sulfapyridine, sulfathiazole and sul- fadiazine were separately administered to mice orally in doses of 0.2 Gm. per kilogram of body weight. To some the compounds were administered twice daily for two days before inoculation with a virulent strain of Erysipelothrix rhu- siopathiae and twice daily thereafter for six additional days. For others treat- ment was begun four hours after inoculation and then administered twice daily for six additional days; in still others, for eight additional days. It was observed that 12.5 per cent of mice treated before or after the admin- istration of these compounds survived. Additional evidence of some therapeutic effect was the greater percentage (50 per cent) of survival of the animals treated before inoculation and the longer time of survival of animals treated after inocu- lation compared with those of the untreated control group. The therapeutic effect of these compounds is therefore limited. Sulfanilamide and sulfapyridine appeared to give better results than sulfathiazole and sulfadiazine. The thera- peutic effect of these compounds was slightly enhanced when they were employed in conjunction with subcurative injections of immune serum. -

INTRODUCTION Bacterial diseases following aquatic injuries occur frequently worldwide and usually develop on the extremities of fishermen and vacationers, who are exposed to freshwater and saltwater.1,2 Though plenty of bacterial species have been isolated from marine lesions, superficial soft tissue and invasive systemic infections after aquatic injuries and exposures are related to a restricted number of microorganisms including, in alphabetical order, Aeromonas hydrophila, Chromobacterium violaceum, Edwardsiella tarda, Erysipelothrix rhusiopathiae, Myco- bacterium fortuitum, Mycobacterium marinum, Shewanella species, Streptococcus iniae, and Vibrio vulnificus.1,2 In particular, skin disorders represent the third most common cause of morbidity in returning travelers and are usually represented by bacterial infections.3–12 Bacterial skin and soft tissue infectious conditions in travelers often follow insect bites and can show a wide range of clinical pictures including impetigo, ecthyma, erysipelas, abscesses, necro- tizing cellulitis, myonecrosis.3–12 In general, even minor abrasions and lacerations sustained in marine waters should be considered potentially contaminated with marine bacteria.3–12 Despite variability of the causative agents and outcomes, the initial presentations of skin and soft tissue infections (SSTIs) complicating marine injuries are similar to those occurring after terrestrial exposures and usually include erysipelas, impetigo, cellulitis, and necrotizing infections.3 Erysipelas is characterized by fiery red, tender, painful plaques showing well-demarcated edges, and, though Streptococcus pyogenes is the major agent of this pro- cess, E. rhusiopathiae infections typically cause erysipeloid displays.3 Impetigo is initially characterized by bullous lesions and is usually due to Staphylococcus aureus or S. -

Aureus. 4 Sycosis Vulgaris (Sycosis Barbae) • Perifollicular, Chronic , pustular staph infection of the bearded region. • Presentation: Itch/burn followed by small, perifollicular pustules which rupture. New crops of pustules frequently appear esp after shaving. • Slow spread. • Distinguishing feature is upper lip location and persistence. – Tinea is lower. – Herpes short lived – Pseudofolliculitis Barbea ingrown hair and papules. 5 Sycosis Vulgaris 6 Sycosis Lupoides • Staph infection that through extension results in central hairless scar surrounded by pustules. Pyogenic folliculitis and perifolliculitis with deep extension into hair follicles often with edema. • Thought to resemble lupus vulgaris in appearance. • Etiology: S. Aureus 7 Treatment of Folliculitis • Cleansing with soap and water. • Bactroban (Mupirocin) • Burrows solution for acute inflammation. • Antibiotics: cephalosporin, penicillinase resistant PCN. 8 Furunculosis • Presentation: Perifollicular, round, tender abscess that ends in central suppuration. • Etiology: S. Aureus • Breaks in skin integrity is important. – Various systemic disorders may predispose. • Hospital epidemics of abx resistant staph may occur – Meticulous hand washing is essential. -

KERBY Department of Microbiology, Harrogate General Hospital, Knaresborough Road, Harrogate, North Yorkshire HG2 1ND (Received 27 March 1981) SUMMARY Outbreaks of wound infection with Streptococcus pyogenes and Staphylococcus aureus occurred in an abattoir and a pork-processing factory in Autumn 1980. Investigations showed that staff handling the meat before de-boning were partic- ularly affected and that bone was the cause of 48 % of the wounds which became clinicially infected. Of the total isolates of Strep, pyogenes and Staph. aureus from wounds of known cause 75% were from lesions caused by bone. In one outbreak streptococcal infection was seen principally in newly-employed staff. Group L streptococci were found in several sites in the pork factory and were isolated from infected lesions in two workers. Strep, pyogenes and Staph. aureus were cultured from tap handles in the lavatories. Sixty-six workers were examined in a non-epidemic period and 59 % were found to have current wounds on the hand or wrist; 13% of wounds were infected but Strep, pyogenes was not isolated. Bone was the commonest of the many causes of injury, accounting for 31 % of all wounds. Butchers had the highest rates of wounding and most bone-inflicted injury was seen in this group. Packers were the only group to wear protective gloves regularly. Damaged fingernails were found in 50% of workers, including a high proportion of nail-biters (33% of all workers).
