This directory is updated once each year. The continued availability of physicians, medical centers, hospitals, and other providers cannot be guaranteed. If you are a woman age 18–75, please select an obstetrician/gynecologist in addition to your primary care physician. Enrolling in the Kaiser Permanente Flexible Choice plan does not guarantee services by a particular physician in this list. If you wish to receive care from a specific physician listed, call in advance of your appointment to be sure he or she is still participating and is accepting new patients. To inquire about an Option 1 Permanente physician, go to kp.org or call our Member Services department toll free at 1-888-225-7202. He or she coordinates your care, orders needed tests, and consults with specialists, pharmacists, and others as needed. When you have a PCP, you have a physician who knows you, understands your health goals and preferences, and advocates for you. The information you need to choose your PCP can be found in this directory or online at kp.org/doctor. The next pages show you how to do both. If you’re a woman age 18 or older, you also have a personal obstetrician/gynecologist who coordinates your special health care needs. NEW! In 2016, you’ll be able to make a video appointment with your Permanente personal doctor. You’ll need a smartphone, tablet, or computer with a camera and you must be present in Maryland, DC, or Virginia during the appointment. CONTINUING YOUR CARE WITH SPECIALISTS* Should you ever need a specialist (for example, a cardiologist, neurologist, podiatrist, or dermatologist), we have doctors who are able to address health care needs across more than 50 medical and surgical specialties. Our specialists are experts in their fields and are ready to help diagnose and treat you whether you have a broken bone or a chronic condition. Having a team that knows you and is focused on you can lead to quicker, better diagnoses and treatments.
