Колонка главного редактора Юбилейная конференция «Илизаровские чтения» В год 95-летия со дня рождения ака- демика РАН, профессора, доктора меди- цинских наук Г.А. Илизарова, 65-летия метода Илизарова и 45-летия Центра Илизарова в РНЦ «ВТО» им. академика Г.А. Илизарова была успешно проведе- на международная научно-практическая конференция «Илизаровские чтения». Конференция состоялась 16-18 июня 2016 года в г. Кургане. Организаторами конференции выступили ФГБУ «РНЦ «ВТО» имени академика Г.А. Илиза- рова» МЗ РФ (г. Курган) и Texas Scottish Rite Hospital for Children (USA) при поддержке международных общественных организаций ASAMI International, ASAMI Russia, LLRS –ASAMI North America и Общероссийской обществен- ной организации «Ассоциация травматологов-ортопедов России». На конференции был зарегистрирован 501 участник из 32 стран и 25 городов Российской Федерации. В програм- му конференции вошли 12 лекционных сессий, на которых было прочитано 62 лекции; 9 тематических заседаний (круглых столов), где 130 докладчиков представили 150 сообщений; 13 мастер-классов. Международная научно-практическая конференция «ИЛИЗА- РОВСКИЕ ЧТЕНИЯ» получила аккредитацию EACCME® – Евро- пейского Аккредитационного Совета Непрерывного Медицинско- го Образования, действующего в сотрудничестве с Европейским союзом медицинских специалистов. В рамках конференции в торжественной обстановке учным из разных стран мира присвоено звание «Почетный профессор Центра Илизарова», как символ признания заслуг и выражения благодарности лицам, способствовавшим своей деятельностью динамичному развитию, международному признанию, росту престижа и благосостояния Цен- тра, внесшим заметный вклад в популяризацию метода чрескостного остеосинтеза по Илизарову. (A): Sagittal PD FS of the ankle demonstrating full thickness midsubstance tear of the Achilles tendon with tendon gap of approximately 4.0 cm (bracket). (B and C): Sagittal T1 and PD FS postoperative images 3 years after primary end-to-end repair. There is expected thickening of the repaired tendon, which is intact. Linear intermediate intrasubstance signal (arrows) within the mid substance may represent minimal degeneration or postoperative changes. Additionally, there is loss of the calcaneus declination angle, indicative of possible lengthening of the Achilles. Abstract The objective of this study was to evaluate the operative management of flexible flatfoot in adolescent by calcaneal lengthening osteotomy described by Evans and dynamic soft tissue reconstruction of medial foot. To elevate the collapse of the medial longitudinal arch on the weight bearing state and correct the deformity which has three components heel valgus, forefoot abduction in addition to collapse of the medial longitudinal arch and centralizes the motion of the subtalar joint. Methods: In our study, (42) feet in (21) patients who had symptomatic flatfoot were treated with lateral calcaneal lengthening (LCL) osteotomy, accessory navicular excision, anatomical reconstruction of the spring ligament complex (internal brace) augmentation and flexor digitorum longus (FDL) transfer to tibialis posterior tendon (TPT) and gastrocnemius recession. After LCL medial longitudinal arch is reestablished and soft tissues of medial foot usually require reconstruction. Thus, we decided to perform combination surgeries, we believe that using (FDL), as reported in this study, is a more anatomically correct procedure, supporting (TPT) without harmful effect on foot function, while simultaneously checking spring ligament.

First line treatments are parental reassurance and conservative measures; however, surgical treatment may be needed in some cases. A number of surgical techniques with varying results have been described in the literature. Here, we present our clinical and radiological outcomes of calcaneal lengthening osteotomy for pediatric idiopathic flexible flatfoot. Methods: Calcaneal lengthening osteotomy was performed in 20 patients, 30 feet, with idiopathic flexible flatfoot that were resistant to conservative treatment between 2007 and 2011. Patients were evaluated according to ACFAS universal evaluation scoring scale and radiographic indexes. The mean follow up duration was 23.1 ± 9.9 months. Results: The average age was 10.4 ± 0.9 years. Achilles tendon lengthening was performed in 28 feet. ACFAS score at the final follow up had improved significantly compared to pre-operative score (37 to 88, P<0.0001). Radiographic parameters also showed significant improvement after surgery ((P<0.0001)). Distal segment displacement and hardware irritation as postop complications were observed in 2 and 3 cases, respectively, with no long-term clinical impact. Conclusion: Calcaneal lengthening osteotomy is an appropriate and safe operation in symptomatic idiopathic flexible flat foot that is resistant to conservative treatment.
