The Practice Vision:

PRACTICE PREMISES

Morgan Street, Pontypridd Ashgrove is a purpose-built Practice with one reception area, two waiting areas, twenty-four clinical rooms made up of consulting rooms, treatment rooms, a wound dressing room and an operating theatre.

CAR PARKING

There are two large public car parks within sight of the Practice. Access for disabled persons arriving by car is available to the rear of the premises for drop and collection only. This is a no parking area that must be kept clear to ensure doctors are not obstructed in attending emergency situations. Traffic wardens regularly patrol this area.

NO SMOKING POLICY

In the interest of health the Practice operates a NO SMOKING policy on our premises.

REGISTRAR DOCTORS

Ashgrove is a training Practice. Registrars are fully medically qualified and many have a great deal of hospital experience, their training in the Practice being centred on the role of the general practitioner. They are available for consultations in exactly the same way as the other doctors in the Practice.

UNDERGRADUATE TEACHING

Ashgrove is also a teaching practice for undergraduates pursuing a medical career.

HEALTH CARE PRACTITIONERS

Anita Cornish (f) Accredited Phlebotomist (RGH) City & Guilds (NVQ 3) in Primary Care. Research Co-ordinator

Cheryl Benson (f) Accredited Phlebotomist (UHW). City & Guilds (NVQ 3) in Health & Social Care.

Duties include taking bloods, urine testing, BP recording, ECG and spirometry recording. All results and readings are interpreted by the clinician requesting the test. Diabetic foot checks and removal of stitches/sutures, simple dressings. Aesthetics, Immunisations (Flu, Fluenz, Pneumovax, B12).

Katherine Pritchard (f) (Trainee HCP) Katherine is an accredited phlebotomist (RGH) and is responsible for taking blood at the clinicians’ request. Katherine is currently undertaking training to become an accredited Health Care Practitioner.

COMMUNITY STAFF

The following are employed by the Cwm Taf Trust and are attached to the Practice.

DISTRICT NURSES

A team of staff nurses and a nursing auxiliary led by Nursing Sister Jane Hooper provide nursing support at home. District Nurses are now based at Dewi Sant Health Park.

HEALTH VISITORS

A team of three, Karen Harvey, Bethan John and Carys Lear, provide child health care, health promotion and child protection via home/school visits and clinics at the Practice to the under five year olds.

They provide the Health Child Wales Programme from late pregnancy to 5 years which includes vaccination schedule at Child Health Clinic and monitoring a child’s growth and development milestone via a home visiting service. Health Visitors and Nursery Nurse Team work with young families through times of difficulty and sometimes safeguarding problems and offer support with behaviour and development concerns. Health Visitors support parents to identify and develop family resilience.

MIDWIVES

A team of midwives support the doctors in providing antenatal care.

Please Note: From August 2018, the Midwives will be based at Pontypridd Health Centre.

PRACTICE MANAGEMENT

PRACTICE BUSINESS MANAGER

Moira Moore is responsible for implementing Practice policy, the performance of Practice Staff and the efficient deployment of all Practice Resources.

OPERATIONAL MANAGER Sarah Powell is responsible for the efficient day-to-day management of operational support.

RECEPTION STAFF

Reception staff will treat you in a friendly, courteous manner and are here to help you.

ADMINISTRATION STAFF

A small team of secretarial and clerical staff provide administrative support for the Practice, thereby releasing reception staff and clinical personnel to devote their time to patients.

DATA BARRING SERVICE

All staff at the Practice have been vetted by the DBS (Data Barring Service).

MEDICAL ADVICE

Please note that all telephone calls may be recorded for training and information purposes.

TELEPHONE ADVICE

Often problems can be dealt with by telephone. The receptionist may take your telephone number and ask an appropriately qualified person to ring you back.

For conditions such as backache, colds, laryngitis, hayfever, diarrhoea/gastroenteritis, cystitis, thrush, period pains, constipation, threadworms, accidents/trauma it may be considered appropriate to offer you a telephone consultation. Should the health professional subsequently feel it necessary for you to be seen at the Practice an appointment will be offered to you.

DISABLED

Wheelchair access to the Practice is via the rear patient entrance. As the Practice building consists of two floors, disabled/wheelchair patients can now use the patient lift. Please note motorised scooters are not allowed within the Practice – please advise reception if you require use of the Practice wheelchair.

DEAF/HARD OF HEARING PATIENTS

Our patient call-in system will speak and display your name when you are called. There is a portable loop induction facility available for your use upon request. Patients with hearing impairment may be interested in registering with My Health Online. This facility allows online prescription requesting and some appointment booking. Please make our reception staff aware of your condition if you require any further assistance.

PATIENTS WITH IMPAIRED VISION

PUSHCHAIRS

We have a dedicated area in the rear foyer for pushchairs as due to restricted space they cannot be accommodated within the public waiting areas

APPOINTMENTS

An appointment system is in operation for all doctors, nurses, clinics and patient care programmes. Patients arriving late may be asked to re-book their appointment.

Appointments may be made in person or by telephoning (01443) 404444 during Practice opening times or by registering for online access.

We have three appointment types available to reflect the urgency of your problem. Routine appointments are available to be booked up to 4 weeks in advance. For more urgent problems appointments are available to be booked within 2 days. For medical emergencies the receptionist will take your details and a Doctor will call you back, wherever possible within 1 hour. Should an appointment still be necessary the doctor will arrange for you to be seen the same day.

Please let us know as soon as possible if cancelling an appointment.

PRACTICE OPENING TIMES

Monday to Friday 8.00am to 6.30pm (Doors open 8.00am for appointment arrivals).

The Practice is closed on Saturdays, Sundays and Bank Holidays. In addition, and in common with other surgeries in the area, on the third Thursday of each month from 12 noon the Practice will be open for non clinical services only to support educational/training for the staff. There will also be occasions when the Practice will be fully closed on the third Thursday from noon when required.

UNIVERSITY OF SOUTH WALES

GP Appointments are available at the University for those students who live in the Treforest/Pontypridd area and are registered with Ashgrove surgery.

Doctors from the Practice hold surgeries for students at the University Wellbeing & Disability Service in the Building during term time as follows:

Students can order repeat medication at the Wellbeing & Disability Service by:

 Completing a repeat medication form and posting in the Health mailbox outside H10  Through an appointment with the University Nurse Adviser via AZO  Via the “My Health Online” service  By arranging with your local pharmacist

CHAPERONE

It is probable when attending a doctor that you may need to be medically examined. Should you not feel comfortable about this please request a chaperone.

HOME VISITS

Where a patient’s medical condition prevents attendance at the Practice and a doctor’s call is required, please telephone (01443) 404444 before 10.00am weekdays. Requests for home visits will be assessed by the clinical team and should it be considered clinically appropriate then a member of the team will visit.

EMERGENCY TREATMENT – PRACTICE HOURS

Please telephone (01443) 404444. After taking brief details the receptionist will ask the Emergency Doctor to call back. If appropriate, and it is medically possible for you to attend the Practice, you will be given an appointment by the Emergency Doctor. However if the patient is unconscious, suffering heavy blood loss, severe breathlessness or chest pain please dial 999 immediately and request an ambulance.

EMERGENCY TREATMENT - OUT OF HOURS

If you require urgent medical attention when the Practice is closed please telephone (01443) 404444. Please have a pen and paper to hand to take down the telephone number of the Out of Hours service. Please note that attendance at the Out of Hours centre is by invitation only.

Please note if the patient is unconscious, suffering heavy blood loss, severe breathlessness or severe chest pain please dial 999 immediately and request an ambulance.

SICKNESS CERTIFICATION

If you are unable to work due to illness, your employer may require sickness certification. For the first week of absence you may provide a self-certificate - Form SC2- obtained from your local Benefits Agency or online at gov.uk. You are not legally required to provide a medical certificate (Fit Note) from your doctor unless your absence is for more than one week.

REPEAT PRESCRIPTIONS

Requests for repeat medication can be made online by registering for a My Health Online account or by delivering the repeat counterfoil (with items required ticked) to the Practice. Pharmacies may offer a repeat prescription request service. Patients are asked to nominate a local chemist (from a list available at reception) and the Practice will arrange for your medication to be available for collection from that said Pharmacy. Please allow two full working days for your prescription to be processed.

REGISTRATION

Patients residing within the Practice boundary and who wish to register with the Practice should contact our Receptionists, who will guide you through the registrations process.

Newly registered patients will be offered a health check appointment if required on registering with the Practice.

We shall endeavour to comply with any reasonable preference expressed to see a particular health professional but need not do so if the preferred practitioner has reasonable grounds for refusing to provide services to the patient, or does not routinely perform the service in question within the Practice.

VIOLENT OR ABUSIVE PATIENTS

In the interests of our staff and patients the Practice upholds a policy of zero tolerance towards any patient who is threatening, violent or abusive whilst at the Practice or when a health professional makes a home visit. Such patients may be reported to the police and/or removed from the Practice’s list. Patients removed from the Practice list will be informed in writing of the reasons for removal and the action they need take to register at another Practice.

OTHER SERVICES

MALE STERILISATION

Vasectomies are carried out in our operating suite by consultant- trained and LHB accredited Partners. Patients considering a vasectomy should make an appointment with a doctor or nurse practitioner of your choice. Following referral, you and your partner will be requested to attend a counselling appointment with Dr Colquhoun or Dr Blair to discuss the procedure in further detail.

Male sterilisation is also available privately to non patients or through the NHS when referred by their own GP.

PRIVATE SERVICES

A fee will be charged for private services at a rate which may include a recommendation by the British Medical Association. Please ask at reception for details, or check on our website. Private services include:-

OCCUPATIONAL HEALTH SERVICE

A private service to industry provided by one Partner specialising in this discipline. Services include pre-employment medical vetting, sickness/absence medicals and hazard risk assessment, etc.

AESTHETICS SERVICE (Save Face Accredited)

Ashgrove's non-surgical aesthetic services which includes the services such as medical “Botox” treatments for cosmetic facial to reduce the appearance of wrinkles and lines, dermal fillers to reduce lines and increase lost volume, and axillary hyperhidrosis (excessive under-arm sweating).

All consultations and treatments are provided exclusively by three of our GP partners who all have established practice in minor surgery, supplemented by an accredited qualification in non- surgical cosmetic & aesthetic procedures.

Consultations with our GPs are entirely free of charge. If after consultation with the GP you wish to proceed with a treatment this will be done during the same appointment. The price of treatment includes any required aftercare.

At Ashgrove Surgery we have a large, dedicated operating theatre where we perform all our procedures. Our operating theatre is thoroughly maintained and meets all the required standards.

In the pursuit of continually improving services to patients we welcome any comments or suggestions you may wish to make. Please pass your views in writing, or via email, or verbally to the manager.

Alternatively, you may either place written comments in the suggestion box situated in the reception area or apply to join the new patient group which will take part in regular on-line surveys to support such improvements. Application forms are available at reception.

PATIENTS CONCERNS

The Practice aims to provide the very best care and treatment and we welcome constructive feedback for this enables us to learn from the patient(s) experiences, good or bad. We strive to offer a good standard of service which is consistent. However, on the occasions

when we fall below our normal standard of service it is important that we identify any failures and address them appropriately.

Therefore, we advocate talking to the staff who have been involved with your care and treatment. This enables the member of staff to address your concern immediately. Alternatively, you can contact the manager or a member of the Reception team who will give you further information.

RESEARCH AND PLANNING

Information about patients is requested for a wide variety of purposes including education, research, monitoring, epidemiology, public health surveillance, and clinical audit and planning. The practice will anonymise data where possible. Only where it is essential for the purpose will identifiable records be disclosed. Such disclosures will be kept to the minimum necessary for the purpose. You have the right to object to any such disclosure and your objection will be respected.

FINANCIAL AUDIT

The Cwm Taf Morgannwg University Health Board (UHB) has a duty to occasionally check on claims made by practices for payments. The person carrying out the check will have been properly trained and authorised by the UHB and will be subject to a duty of confidentiality in their employment contract or because of their registration with a statutory regulatory body. Only that small part

of the record relating to the claim will be made available to the person carrying out the check.

Ashgrove is an innovative practice at the leading edge of Primary Care and as such we set out to provide you, the patient, with a level of health care appropriate to your needs.

As a Practice we strive towards offering our patients a high quality service which includes:  being treated as an individual, with courtesy, respect and dignity at all times  being given the names of people involved in your care  being offered a health check appointment if required on registering with the practice  being seen, under normal circumstances, within 30 minutes of your appointment time or to be given the reason for delay and have the right to make another appointment  being given, under normal circumstances a routine pre- bookable appointment up to 2 weeks in advance, and we also provide pre-bookable appointments, which are released 48 hours in advance, if you wish to be seen earlier.  having access to a GP on the day, when warranted by a condition that you feel needs to be dealt with that day. Emergency access will initially be on the telephone, with an appointment offered if appropriate. Such occasions may result in waiting times exceeding 30 minutes  receiving advice or action you can take to promote good health  being given the most appropriate care by suitably qualified people and no care or treatment will be given without your informed consent  being referred to a Consultant acceptable to you when your doctor feels such a course of action necessary. Please contact the hospital if you have not been contacted within a month of the GP referral.  Choosing whether or not to take part in medical research  receiving medical advice over the telephone where appropriate

 having access to your health records subject to any limitations in the law  having access to suitably qualified clinical personnel at all times for emergencies where appropriate  expecting your concerns to be treated in accordance with NHS Wales “Putting things right” guidelines and to receive a leaflet describing the process  a copy of the practice booklet can be accessed from the Practice Website www.ashgrovesurgery.com which will give you details of all practice services etc that are available to you or alternatively you can request a printed copy at Reception.

To help us to help you we require that you..  telephone the surgery prior to attending for urgent treatment for which you do not have an appointment. This will enable the emergency doctor to discuss your condition and provide you with an arranged appointed that day if appropriate.  attend the surgery instead of requesting a home visit, when at all possible, to gain benefit from facilities available. Home visits will normally be made only in true emergency situations, where for medical reasons the patient is unable to attend surgery. Please note that lack of transport will not be acceptable to warrant a house visit.
