Our services were completed in accordance with our revised proposal for geotechnica l engineering services dated December 2, 2017 and your authorization to proceed with the work. The purpo se of our investigation was to explore the subsurface conditions at the site in order to provide recommendations for foundation design and site preparation . Evaluation of environmental issues and hazardous wastes was not included within the scope of service s provid ed . The opinions, recommendation s and design criteria presented in this report are based on our field exploration program, labor atory testing and engineering analyses. Based on the results of our investigation, it is our professional opinion that the proposed project should be feasible from a geotechnical perspective pro vided that the recommendations presented in this report are implemented in design and carried out through construction. The site is situated on the northern end of APN 572-190-004 and is located at approximately 33.5233 degrees north latitude and 116.7762 degrees west longitude. The approximate location of the site is indicated on the Site Location Map (Figure 1). Our investigation was conducted in order to evaluate the engineering properties of the subsurface materials, to evaluate their in-situ characteristics, and to provide engineering recommendations and design criteria for site preparation, foundation design and the design of various site improvements. This study also includes a review of published and unpublished geotechnical and geological literature regarding seismicity at and near the subject site. PROJECT DESCRIPTION Based on our preliminary discussions, it is our understanding that the proposed project will consist of constructing a new health care facility on the currently vacant property. We anticipate that the project will also include a new on-site sewage disposal system consisting of a septic tank and leach lines, stormwater retention areas, paved parking, underground utilities, concrete flatwork and various associated site improvements. For our analyses we expect that the proposed structure will consist of a single story

    ARCHAEOLOGICAL SURVEY REPORT Interstate 10 Corridor Project San Bernardino and Los Angeles Counties 07-LA-10 PM 44.9/48.3 08-SBD-10 PM 0.0/R37.0 EA 0C2500 EFIS ID 0800000040 April 2015 STATE OF CALIFORNIA Department of Transportation SUMMARY OF FINDINGS The California Department of Transportation (Caltrans), in conjunction with San Bernardino Associated Governments (SANBAG), proposes to improve the Interstate 10 (I-10) corridor. The proposed I-10 Corridor Project (Project) consists of adding lane(s) and providing improvements along all or a portion of the existing 33-mile-long stretch of I-10 from approximately 2 miles west of the Los Angeles/San Bernardino county line in the City of Pomona to Ford Street in the City of Redlands (see Figures 1 and 2 in Exhibit 1 of the Historic Property Survey Report [HPSR] prepared for this Project). The Project considers one “no build” alternative and two “build” alternatives to address existing and future projected traffic demands. The archaeological survey and identification effort included cultural resource records and literature searches, Native American consultation, and reconnaissance and intensive pedestrian (Phase I) surveys of the Area of Direct Impact (ADI). Record searches indicated that nine pending and four known historical archaeological sites were potentially within the Project Area of Potential Effects (APE). Pending archaeological resources that have not been formally recorded onto Department of Parks and Recreation (DPR) 523 forms include an unnamed road (P1074-61H), Tenney Ditch (P1063-49H), Marias Araminta Ditch (P1063-52H), Hunt and Cooley Ditch (P1074-84H), Camp Carlton Ditch (P1074-85H), Jansen Ditch (P1074-86H), Rancheria Ditch (P1074-88H), Old Meeks and Daley Ditch (P1074-104H), and Sunnyside and South Fork Ditches (PSBR-21-H).

    4.5 CULTURAL RESOURCES 4.5 Cultural Resources This section describes the cultural (i.e., archaeological and historical) and paleontological resources in the area of the Proposed Project. Appendix D contains the results of Native American outreach to date, and Appendix G contains cultural and paleontological technical studies. The potential impacts of the Proposed Project and Alternative Project are also discussed. For the purposes of this section, the Area of Potential Effects (APE)/Project Study Area is defined as the area encompassing all of the following: (1) the existing and proposed right-of-way (ROW) for the West of Devers (WOD) Upgrade Project; (2) the existing and proposed access roads associated with the Proposed Project, 25 feet from centerline on each side for a total buffer width of 50 feet; (3) the proposed telecommunications route, 25 feet from centerline on each side for a total buffer width of 50 feet; (4) the proposed 66 kV subtransmission lines, 25 feet from centerline on each side for a total buffer width of 50 feet; and (5) the proposed staging yards outside the existing WOD corridor (see Appendix G, Cultural Resources, Cultural and Paleontological Project Study Area Figure). The buffer width allows for documentation of resources adjacent to the project limits to address any future minor modifications to the Proposed Project preliminary design. 4.5.1 Cultural Resources Environmental Setting 4.5.1.1 Geographic and Geologic Setting Geographic Setting The APE/Project Study Area includes the cities of Banning, Beaumont, Calimesa, Colton, Grand Terrace, Loma Linda, Palm Springs, Rancho Cucamonga, Redlands, San Bernardino, and Yucaipa, and unincorporated areas of Riverside and San Bernardino counties including portions under the jurisdiction of the Bureau of Land Management (BLM) and the Morongo Reservation.
