You must consult your Summary Plan Description to determine what services are covered. Failure to do so before incurring medical expenses may result in you’re not being reimbursed for certain medical expenses. This is a directory of preferred providers participating as independent contractors to the Trust Fund. This list is subject to change without notice. Every effort has been made to obtain and print correct information in this directory; however, the Trust Fund will not be responsible for inadvertent errors or omissions. PLEASE READ CAREFULLY We are very pleased to have added new providers to your Preferred Provider Directory. With preferred providers available, you can now save yourself and your Trust Fund money by selecting preferred providers whenever you need medical, dental or vision care. REMEMBER – Your Network is in a constant state of change. New Providers may enter the Network. Currently listed providers may no longer participate. Before services are rendered, please verify Preferred Provider status by calling the Healthcare Services Department at your Trust Fund Office, (775) 826-7200. Before making an appointment with a provider you have not seen before or to verify that a current PPO contract is in effect. It is your responsibility to verify that your choice of provider and/or location of office/facility are in fact a Preferred Provider. Failure to do so may result in reduced benefits and higher out-of-pocket expense to you. Just because a specialist is listed in your Directory, does not mean that all services provided are covered benefits. READ YOUR SUMMARY PLAN DESCRIPTION CAREFULLY. If you do not understand it or are uncertain if a service is a covered benefit, call the Trust Fund Office for assistance. 2018 - 2019 Preferred Provider Directory - 2 - Northern Nevada Laborers Health & Welfare Trust Fund – Preferred Provider Directory – 2018 - 2019 TABLE OF CONTENTS I. Community Health Needs Plan 2018-2021 RENOWN HEALTH’S MISSION, VISION & VALUES Mission Make a genuine difference in the health and well-being of others. Vision Renown Health, with our partners, will inspire better health in our communities. Values Caring • Integrity • Collaboration • Excellence At Renown Health, we are proud to be Reno’s only locally owned, charitable, not-for-profit integrated health network. Being not-for-profit means that all of our income stays here — and is reinvested in programs, people and equipment to improve the health of our community. Founded in 1862 as the Washoe Clinic to treat patients affected by a smallpox outbreak in Washoe County, Renown Health was created by members of our community. For more than 150 years, we have solidified a trusting partnership upon which residents have come to depend. With community at the center of our mission, we embrace our commitment to make a genuine difference in the lives of others. We take pride in our responsibility to contribute to the community’s overall well-being. Our Community Benefit mission is to reduce health disparities, promote community wellness and improve access to care for vulnerable populations. This includes offering free and discounted care to those unable to afford healthcare, and partnerships with others to address health and well-being. In partnership with many community-based organizations, area schools, human and social service agencies, and government and business leaders, we support a variety of educational, public health outreach, and community development initiatives throughout Nevada. Our Renown Health leaders, physicians and staff actively participate and support community non-profit organizations by serving on their boards of directors and offering expertise and resources to advance these organizations.

Contact: Renown Public Relations M: 775-691-7308 E: [email protected] MEDIA ADVISORY – VIDEO & PHOTO OPPORTUNITY RENOWN HEALTH ADDS PROACTIVE LEVEL OF PROTECTION TO ADDRESS COVID-19 Deployable Medical Structure at Renown Regional Erected as Precautionary Measure WHAT: As we continue our planning and preparation for the impact of COVID-19, Renown Health now has an alternate care site – a deployable medical facility to serve as an additional respiratory illness evaluation and screening center for Emergency Room patients at Renown Regional. Media is invited to tour the tent before it opens and learn more about how Renown Health physicians and staff are prepared to ensure the safety of patients, staff and our community. It’s important to note there are no confirmed COVID-19 cases at Renown at this time. This deployable medical structure is simply a precautionary measure and adds a proactive level of protection if needed because Renown is committed to doing everything possible to protect patients, staff and community members. WHEN AND WHERE Thursday, March 12 at 1 p.m. in the parking lot outside the Renown Regional Medical Center Emergency Room at 1155 Mill St, Reno, NV. All media must be escorted on hospital property. Mill Street Parking is most convenient. WHY: Due to increasing levels of respiratory illness in our community, as a way to manage influx and limit the exposure between individuals suspected of having respiratory illness and patients and care providers in other areas of the hospital, Renown Health worked with the State Public Health Preparedness to request a deployable medical facility through the Washoe County Emergency Manager.
