Community Health Trainers  Health trainers can help you get started on making health changes.  They will have one to one meetings with you.  They can help you to set and achieve your goals in relation to the changes you wish to make. For example, they can help you find ways to get more physically active in your daily life and help in relation to weight loss and healthy eating.  They are able to point you in the direction of health related services.  They can accompany you to new places and groups.  Health trainers are sometimes based with different services in Manchester.  If you would like a health trainer to contact you or you would like more information contact 0161 861 2548.

People • Health • Partnerships The Healthy Living Network works with people and organisations to encourage healthier lifestyles in South Manchester communities.

The Network provides access to a range of health/wellness improvement activities including: volunteering and employment opportunities, training volunteers to deliver chair-based exercises, running cook and taste sessions to promote healthy eating, health information at Wythenshawe Forum, Community Hospital (with Macmillan) and Health Care Centre for public to access relevant leaflets.

With over 200 Network members, HLN works in partnership to create better health in South Manchester. BOOST: Emotional Resilience Course – Self Help Services

This is a 6-week structured course aimed at individuals who are struggling to cope with life‟s ups and downs. The course explores practical ways to improve emotional wellbeing and ability to deal with life challenges. The course gives people the opportunity to meet and talk together in a mutually supportive environment and build upon strategies to improve their mental health, confidence and general wellbeing. The course operates on a rolling 6-week basis, with a pre-course information session and post-group optional review session.

A series of courses will run in North, South and Central Manchester locations running on a rolling basis.

8 SUPPORT FOR MANAGING LONG TERM CONDITIONS

The Expert Patients Programme (EPP): For people living with a long-term health condition

 The Expert Patients Programme is a course for anyone with long-term health conditions, who would like to find ways of managing their illness more positively in order to improve their quality of life. It is a national programme. People who attend include many who have depression and/or other mental health issues such as anxiety and stress, bi-polar disorder, schizophrenia.

 The programme builds on the principal that people with long-term health problems including mental health, know best how the condition affects the way they feel, on a day to day basis, how it affects their lifestyle and their ability to accomplish activities important to us.

 The course also looks at ways to positively manage symptoms, to improve on daily activities and manage the changing emotions that can be brought about by their health condition.

 The course is free and consists of 6 weekly sessions – each lasting 2 ½ hours.

 It is delivered by two volunteer tutors who themselves have long-term health problems and have previously attended an Expert Patient Course prior to becoming a tutor. This means they understand the challenges people face in having health problems and coming on the course.

 Weekly sessions provide information on specific topics such as managing symptoms, relaxation techniques, the importance of diet, exercise, although they don‟t put on the lycra and do a work out, they talk about the benefits of physical activity and healthy eating and how this can help reduce symptoms and why, how to communicate effectively with health care practitioners. Time will be given for participants to share experiences and learn from each other, and to develop ways of overcoming specific difficulties.

 On each course there are between 10 and 12 people. The atmosphere is very informal which enables people to relax and get to know each other. The venues are checked to ensure they are accessible for wheelchair use.

 The positive impact of attending the programme has seen a number of patients reducing the number of exacerbations or chaotic episodes related to their condition. Head of clinical psychology at North Manchester discharged one volunteer for the programme after 20 years of treatment. This patient has gone on to become a volunteer for the Expert Patients Programme.

 People have commented that the course has improved their outlook on life, helped them to become more active and able to take control over their lives. An additional bonus has been the support and friendship that has been gained from other course members.

 A monthly drop-in is available to help break barriers and support attendance and give information about EPP. It is held at the Harpurhey Well Being Centre, 1pm-3.30pm, on the 1st Tuesday afternoon of each month. Workers can support clients and attend with them or give the client the dates and times so they can attend independently. Workers who may need more information or wish to discuss anything can meet with Caroline Powell about a client.

 Referrals can be made by workers and people can self refer. There is a referral form to complete. The referral form can be completed over the phone, sent by post or fax. You can email the form only if you have an nhs.net email account.

9 The Chronic Fatigue Programme

Who would you refer to the service? You would refer patients who have a chronic health condition where fatigue and pain are the main problems. The service is available for Manchester adults over the age of 18 years. The service offers a pain management course as well as mindfulness course for chronic illness.

Who can refer to the service? Any health professional can initiate a referral to the service where a bio-psychosocial approach is felt to be appropriate. Self referral directly to the service is also possible.

What can we offer? Following an initial assessment, patients will be able to access either: 1. Pain management course or; 2. Mindfulness course for chronic illness.

The courses are delivered in a number of locations across Manchester City. On completion of the course, patients will be able to access a monthly meditation „drop-in‟ session until discharge

Referral criteria You can refer patients with a diagnosis of a Chronic Health condition (onset of more than 6 months) where Chronic Fatigue and/or Chronic Pain are the main problems, e.g., Chronic Fatigue Syndrome / ME, Fibromyalgia, Rheumatoid Arthritis, Osteoarthritis, Musculoskeletal conditions

Exclusion criteria: They will be unable to accept patients who are:  Not registered in Manchester  Waiting for further investigations or treatment  Under the age of 18 years  Housebound or have high physical care needs. Patients need to be self-caring, and well enough to attend the courses

They will be unable to accept referrals for:  Patients who require individual psychological therapy or have high levels of anxiety or depression  Patients with active psychosis or suicidal thoughts  Patients who require individual specialist management for their condition  Patients who do not have a diagnosed long-term condition  Patients with drug or alcohol dependency

Manchester Supporting Health Programme The Manchester Supporting Health Programme is in two parts, one part is focussed on improving physical health of people with severe mental illness and the other part is focussed on improving physical health of people with dementia and their carers.

The two programme teams which develop the work include Supporting Health Nurses and Public Health Development Advisors, based within Manchester Mental Health and Social Care Trust.

Manchester Supporting Health Dementia Programme There is strong evidence that loneliness and isolation are significant threats to the health of older people and that support in social activities is necessary to enable people to look after their own health. This is why they work at fostering dementia friendly communities.

Cheetham and Crumpsall - building dementia friendly communities Building dementia friendly communities will take a lot of time and effort. As a small programme, they concentrate on Cheetham and Crumpsall to show what can be done on a bigger scale. This is an area that has a very ethnically diverse population so they can also show how understanding for people with dementia can be fostered in communities with different cultures. They have found that local community organisations and local services have shown a lot of interest in this work.

Manchester Supporting Health Programme - SMI - This programme includes Supporting Health Nurses who have built experience in providing detailed physical health checks to people living with severe mental illness. This experience is being utilised to support and guide others to improve health of this client group. - The programme works partly with specialist mental health services so that supporting improved physical health can become a part of care planning for the service user. - They are now developing partnership work with voluntary and independent sector organisations which support people with SMI. This is to offer support for them to improve physical health and improve access to health promotion advice/services for clients. - They will work alongside people living with severe mental illness to hear what their needs are and support them on their journey to improve their own health. - They are making links with community organisations to explore social support and activities for wellbeing which they could offer to people with SMI. This is recognised as an important aspect of their journey to better health as isolation and loneliness are big issues. For example, links with befriending services have been established.

13 PHYSICAL ACTIVITY SERVICES AND SUPPORT

Physical Activity Referral Services (PARS) PARS is for people living with conditions that would benefit from increased physical activity to increase their activity levels in a safe and appropriate way. They are specifically targeting those with long term physical health problems like COPD, heart disease, diabetes. They accept referrals of people living with a severe mental illness who also have a long term physical health condition or who have a BMI of over 30.

This service has brought together PACE, ALFA, SMILE into one city wide service.

What the service offers  Gives the client an assessment of medical/health and physical activity history; this is to find out what activity they have done, what they are interested in, tell them about activities they offer and what would suit them. This is done by a Physical Activity Referral Officer. There are follow up appointments.

The client will be expected to attend sessions with the PARS team for the first twelve weeks until they have their first follow up. After this the client will be referred to different sessions (usually with Leisure Services) which will be similar to the classes with the PARS Team. This is to ensure that all those individuals who can progress do so but there is the option for those who are not able to progress at twelve weeks to stay with the PARS team for the following three months. This will be decided by the Referral officer at the follow up appointment.

 Provides support to become active in a safe or supervised environment.

 A range of physical activity including gentle exercise classes in community venues across Manchester. These are specifically aimed at people who have not done any exercise for a long time and who do not feel confident about going local gyms etc.

They may be able to refer the client to their local leisure centre or gym if this would help their condition.

The type of activity available in the different parts of Manchester varies but the client may be able to access something they are interested in if they can travel there. Examples of activity are guided health walks, both outdoors and indoors, Nordic Walking (walking with poles), walk to run sessions, dancing, aqua aerobic classes, T‟ai Chi, Qigong, swimming, supervised gym sessions, chair based exercise classes.

Where there are cycling sessions bikes are provided, they cater for people of all levels: those who want to learn to ride, those who are less confident, those who ride well and people who want to cycle in a group.

 Can refer to Community Health Trainers who can provide support with health goals.

 Can refer people to NHS services like weight management, falls prevention (arm based exercise) and cardiac rehabilitation if they need it.

How to access this service This service can be accessed through the General Practitioner, Practice Nurse or Health Care Provider who will be able to make a referral to the scheme. There is no self referral. Other professionals may refer people into the service, including mental health referrals.

The service can book appointments for clients at almost all of the Health Centres across Manchester. They will book an appointment at the health centre nearest to the client.

Success Through Sport Programme

This programme is for unemployed Mancunians aged 16+, for both men and women. They must have an interest in physical activities and a desire to get motivated towards improving their situation and becoming more employable and gain a basic sports qualification.

The programme will provide them with opportunities to get involved in free football and go for their FA level 1 football coaching badge. Professional coaching, volunteering opportunity to practice the coaching skills and training programmes designed to enhance their employability will also be offered.

Manchester's Green Corridor –a walking circuit around the city

The city of Manchester has 138 parks, 31 of which have been awarded green flag status and a walking route has now been identified that links these green spaces. The Green Corridor is made up of 14 routes with each route averaging 4 miles. The tree-lined routes have been identified and „developed‟ by volunteers from the Ramblers. This has involved them: - identifying specific roads suitable for walking - developing detailed directions for walkers to follow including timings - describing the route in terms of quality, accessibility, facilities, potential hazards etc and risk assessing routes - recording local points of interest including historical ones - identifying relevant groups in the area - identifying useful bus routes

Many of the parks already have established activities and health walks associated with them, so the development of the Green Corridor is seen as an opportunity to promote these and to extend the provision.

This initiative is a partnership between Manchester City Council; Manchester Mental Health & Social Care Trust‟s Physical Activity Referral Scheme and Public Health Development Service; Red Rose Forest and The Ramblers.

Dementia Support Service The Dementia Support Service offers information, support and guidance to a person with dementia or their carers and wider family or friends. Dedicated Dementia Support Workers can deliver this service in the clients own home, at service offices or at other locations face to face, by phone, letter or email. This service may include signposting and providing referrals to other services.

Befriending Service The Befriending Service offers companionship and friendship to people with dementia. The Befriending Manager recruits and trains volunteers who provide support for people with dementia to continue with daily activities and participation in leisure and social activities where they may no longer be able to do so unsupported.

South Carer Support Group This group is open to anyone caring for a loved one with dementia in the Borough of Manchester. It is facilitated by a Dementia Support Worker.

Carer Information and Support Programme Courses This is a series of 5 weekly workshops for people caring for a person with dementia. The programme provides an opportunity for the carer to talk in a friendly and confidential environment about their experiences and the impact on them. If they cannot leave the person for whom they care, it may be possible to arrange a sitting service at home for them. People taking part should be able to attend all 5 sessions.

Dementia Cafés

Dementia Café South Manchester CareConcepts, Care Centre, Marion Lauder House, 20 Lincombe House, Wythenshawe, M22 This is held the first Wednesday of each month.

Dementia Café Central Manchester Every 1st Thursday in the month, 11 – 1pm, at People First Housing Association, 1 City Road, City Road East, Manchester, M15 4PN

You can come along for a chat, meet friendly new people and enjoy a nice cup of tea or coffee and a cake. You have the chance to sit and discuss any issues in a safe, confidential environment, facilitated by experienced dementia support workers.

A trained benefit worker will be on hand every month to offer general advice around benefits and welfare reform or if needed one to one advice.

A memory box is being developed and a programme of guest speakers. They are hoping to make the cafe fun and will be offering a selection of games and activities. The Cafe is in the initial development stages and they would welcome any input or feedback that you feel that you can give.

Referrals can be from by other agencies or the individuals. It is open to anyone who has an issue around dementia, or anyone who cares for someone with dementia; this can be diagnosed or undiagnosed.

Sweet Memories Dementia Café North Manchester Every Thursday 1-3pm at Woodville Sure Start Centre, Shirley Road, Cheetham, M8 ONE

The Dementia Café is a community friendly, weekly drop in support for people with dementia and carers - an opportunity for people with dementia and their carers to meet new people, exchange experiences and information.

People can come along for information and exchange experiences. They can chat with people they may know and meet new people over a nice cup of tea or coffee and a cake. The cafe also offers chair based exercise, guest speakers, fun and games. All welcome.

Peer mentoring and befriending project Manchester Carer‟s Forum have a group of volunteers who have direct experience and understanding regarding caring for someone with dementia. They are matched to carers. A volunteer can keep in regular with the carer by telephone and through visits. The purpose is to give the carer an opportunity to talk about issues of concern with the volunteer. The volunteer has the benefit of their own experience to support the carer and signpost as to services if needed.

Health Information in Libraries There are Health Information Points located in public libraries for books, leaflets and web-based information on all aspects of physical and mental wellbeing.

There is a service “Reading Well in Manchester Libraries” which includes Books on Prescription. These are recommended self help books to manage mental health problems such as depression and anxiety and promote wellbeing such as self esteem.

This service is also promoting reading as a way to reduce stress and boost mood. Staff in libraries have suggested their favourite mood boosting books as part of this collection. People have an opportunity to recommend their favourite mood boosting book to add to the collection; for more information on this contact Danny Middleton on 0161 234 1056.

Information and support to help quit smoking

 If client is on a ward or an out-patient there should be a trained stop smoking advisor who will be able to help and advise.

Check on MMHSCT Intranet site for a list of smoking cessation advisors.

 For more intensive help and support with quitting, you can make an appointment with a specialist stop smoking advisor. They can either see client on the ward or as an outpatient in any of the Manchester hospitals.
