ENROLLMENT AND ACADEMIC SERVICES INTERNATIONAL STUDENT SERVICES Academic Department Form

Purpose of Form: This form is applicable for any international student who has been enrolled in coursework for their current academic program for at least 1 academic term and needs to obtain a Form I-20 or DS-2019 for a new or renewed F-1 or J-1 nonimmigrant status. The purpose of this form is twofold: (1) students can use the completed form as during their visa interview, at the U.S. Port of Entry, and/or in their application to USCIS to show their academic progress and their need to be in F-1 or J-1 status and (2) ISS will ask for a copy of this completed form in order to determine the length of to grant on the Form I-20 or DS-2019 requested by the student. In most cases, the Form I-20 or DS-2019 will be issued by ISS with a program end date that is no later than the estimated graduation term provided on this form.

Form Requirement and Deadline(s): • If an international student begins their academic program by enrolling from outside of the U.S. and then seeks to enter the U.S. in F-1 or J-1 status to begin in-person enrollment at Texas A&M University, this form will be required as part of the I-20 or DS-2019 request. • If a currently enrolled international student seeks to change their nonimmigrant status to F-1 or J-1, this form will be required as part of the I-20 or DS-2019 request. • If a currently enrolled F-1 or J-1 student has a status violation and seeks to regain F-1 or J-1 status, this form will be required as part of the I-20 or DS-2019 request. • This form may be recommended by ISS in other scenarios, but at this time ISS is only requiring the form in the scenario(s) described above.
