The Carers’ Emergency Planning Service will support you to complete an emergency contingency plan should a carer suddenly be unable to fulfil their caring role. The service operates 24 hours per day, 7 days per week, is free of charge and continues whilst longer term arrangements can be made.

The community COPD team offers specialist care for patients with respiratory disease, particularly chronic bronchitis, emphysema and chronic asthma.

Inclusion criteria All adults over 18 years of age with diabetes mellitus and also one of the following:

• Type 1 diabetes – needing insulin initiation • Poor glycaemic control – needing initiation or titration of treatment • Experiencing hypo or hyperglycaemia • Where there is a concern over diagnosis i.e. genetic variation • For structured education for both Type 1 and Type 2 diabetes • For whom insulin pump therapy is indicated.

Exclusions criteria

• Patients with a diagnosis of diabetes under the age of 18 years • Newly diagnosed patients with diabetes who have poor renal function – CKD 4 and 5 • All adult patients with diabetes who are pregnant • All adult patients with diabetes who are acutely unwell from complications of having diabetes • All adult patients with diabetes who have an “at risk” foot complication

Inclusion Criteria This service acts as a resource to support the generic workforce in the provision of care at end of life. This includes meeting with individuals and their families to discuss advance care planning for people approaching end of life and advocates the use of the three National End of Life Care Tools as recommended in the End of Life Care Strategy DH 2008. The team facilitate this in a variety of ways. • Facilitating conversations to support advance care planning – PPC, DNACPR. • Providing a discharge facilitation pathway between secondary and primary care. • Providing education and support for health and social care staff both in the Care Home environment and the wider community. • Supporting and maintaining the coordinated care register. • Acting as the mechanism to support a change in practice in relation to end of life care e.g. Initial facilitation of GSF meeting and providing education and support around the advance care planning including DNACPR process.

Inclusion Criteria

• Registered with a GP in South West Essex • Adults over the age of 18 or those attending Paediatric Epilepsy Nurse and transitioning to the adult service • Confirmed diagnosis of epilepsy • Those under investigation by neurologist following a ‘first fit’

Assessment and treatment will involve support from diagnosis for service user and/or carers. Long term medication management with monitoring of the individual’s epilepsy. Self-management advice including living with epilepsy e.g. education, employment and driving, working towards the best quality of life through a Personalised Care Plan. Monitoring of changes in treatment relating to altered circumstances e.g. pre-conception and pregnancy. Training and education for carers and staff caring for those with epilepsy.

• Multi-disciplinary Team (MDT) meetings aim to support patient centred clinical decision making and integration of care. This is essential in improving the delivery of care to patients on the unplanned care or frail elderly care pathway.

• The MDT Co-ordinator role helps to deliver MDT meetings seamlessly within the CCG and making the best use of clinicians’ time through chasing information and organising meetings. The MDT co-ordinator identifies patients for discussion prior to the meeting, liaising with primary care colleagues; organising and schedule of MDT meetings; collating information and recording outcomes of case discussion and informing the treating clinician and/or the patient’s general practitioner of meeting outcomes.

Inclusion Criteria All Trauma and Orthopaedics, Pain Management and Rheumatology referrals that do not fit into the below exclusions should be referred to Tier II.

Exclusion Criteria Direct referral to secondary care by GP is appropriate for: • Red Flags – Urgent • Suspicion of systematic inflammatory disease requiring medical management, e.g. morning stiffness present for > 30 minutes or more than 6/52. Refer directly to rheumatology • Suspicion of serious pathology (malignancy, infection), i.e. general malaise, weight loss, night sweats, loss of appetite Refer directly to secondary care. • Signs of cord compression/cauda equina syndrome, i.e. bilateral leg pain, bladder/bowel frequency or retention, saddle anaesthesia, gait disturbance, pins & needles, numbness, or worsening neurological symptoms. Inclusion Criteria • Obesity if BMI is over 30kg/m² with co-morbidities. • Impaired fasting glucose or impaired glucose tolerance. • Diabetes – newly diagnosed, Type 2 with poor control, Type 1 with poor control (NICE guidelines definitions). • Gastroenterological problems – IBS, active Cohn's disease, ulcerative colitis, intestinal failure, liver disease, pancreatitis. • Coeliac disease. • Confirmed food allergies and food intolerances. • Dietary deficiencies e.g. iron deficiency anaemia. • Those requiring nutrition support for long term conditions with MUST score of 3 or more, or BMI ≤ 18kg/m² with unplanned weight loss of 5% or more over 3-6 months. • Those requiring texture modification after assessment from a Speech & Language Therapist (SALT). • Those with nutritional problems following a confirmed diagnosis of an acquired or progressive neurological disorder • Those requiring artificial nutrition

Exclusion Criteria Simple overweight/obesity – BMI below 35kg/m², or BMI over 30kg/m² with no co-morbidities. PALLIATIVE CARE TEAM

Inclusion Criteria • People needing specialist palliative care with specific problems such as pain and symptom management or needing psychological, emotional and spiritual support. • People requiring specialist palliative care advice and support through episodes of treatment • People needing specialist Occupational Therapy assessment and support • Specialist palliative care advice needed for patients and carers about medication or information about the disease process • Emotional support required by bereaved carers and relatives of patients know to the Service • People must be over the age of 18, although for people between 16-18 years the service will be accessible if following multi-disciplinary discussion it is deemed the most appropriate service to support them through transition • People residing in the South West Essex locality excluding Brentwood – people residing in Brentwood will be able to access services via St Francis Hospice.

PARKINSON’S DISEASE

The Community Parkinson’s service works with anyone who has a confirmed diagnosis of Parkinson’s disease and offers coordinated care across the pathway, providing and facilitating care and support for both patients and their carers.

The service provides: • Long term medication management with monitoring of the individuals condition • Direct liaison with GPs, neurologists and the wider Multidisciplinary team • Support and advice for care home staff, residents and their families • Self-management advice including living with Parkinson's e.g. education, employment and driving, working towards the best quality of life through a Personalised Care Plan. • Telephone support • Named specialist key worker for all clients

Inclusion criteria: • Adults over the age of 18 • Confirmed diagnosis of Parkinson's or associated condition e.g. PSP • Registered with a GP in South West Essex • Carers of those diagnosed with Parkinson's or associated condition

SPEECH & LANGUAGE THERAPY

Adult speech and language therapy service is provided to people with acquired disorders of speech, language, communication and/or swallowing, and available to all people aged 16 and over.

Referral criteria: • Any patient with an acquired disorder of speech, language, voice or communication and/or swallowing • Registered with a South West Essex GP or under the care of a BTUH consultant where their local GP will accept referral to the service

Exclusion criteria • Patients with dysfluency or for patients with communication disorders arising from dementia • Children under 16 years

The Stroke Specialist team are a multi disciplinary team of stroke specialists who coordinate and manage the stroke pathway from admission to the acute setting and facilitate transitions into community or long term care as appropriate. They facilitate the early Supported Discharge service able to carry out rehabilitation programmes to an acute level in the clients own home whilst also supporting those clients requiring more complex rehabilitation as an inpatient in Brentwood Stroke Unit. They maintain appropriate levels of contact with every patient within the first year of stroke, including a specialist review at 6 weeks, 6 months and one year.

Inclusion Criteria • Adults over the age of 18 • Confirmed diagnosis of Stroke within the first year of onset and requiring specialist input from a Multi disciplinary team – Physiotherapists, Occupational Therapists, Dietician, Speech and language therapy, Nursing input, Social Workers (Essex and Thurrock) and Clinical Psychology. • Registered with a GP in South West Essex

The Tissue Viability Specialist Service provide: • Assessment and treatment which will involve diagnostic assessment and a care management plan that is evidence based. • Self-management advice working towards the best quality of life. • Monitoring of changes in treatment according to changes in situation. • Training and education for clients, carers and staff caring for those at risk of developing a wound e.g. pressure ulcer prevention or those already with a wound.

Inclusion Criteria

• Adults over the age of 18. Although the specialist service is not provided to children, the service will provide specialist advice and support to community paediatric services. • Mental capacity to receive care outside hospital • Registered with a GP in South West Essex In addition one of the following: • No obvious improvement in wound condition or response to treatment after 1 month of treatment • Sudden deterioration of wound • Complex wounds such as fungating , malignant or requiring larval or TNP therapy • Significant risk of Pressure Ulcers developing that cannot be managed by generic primary and community services
