Out of 352 UPHCs a total number of 258 UPHCs have qualified this year for peer assessment of Sushree (A) Peer assessment of UPHCs will have to be completed as per date schedule and guidelines given in Annexure A. (B) A team of two assessors from UPHCs mentioned in Column E of Table 1 of Annexure A will visit and do peer assessment of UPHCs mentioned in Col umn C of the same table. The team of assessors will comprise of the following members - (a) FTMO/PTMO (ifFTMO is not present) (b) Nursing staff of UPHC 1Pharmacist ( If Nursing staff is not present) (C) A fund of Rs 20001- (Rupees Two thousand) only has been allotted for each UPHC going to be assessed by peer assessment, this fund may be utilised for photocopying of checklist pages, refreshment cost of assessors and closure of small gaps as observed during Internal assessment. IT Cell, Swasthya Bhawan for web-posting ~ _ \.~ AdditiOn~ Director, NHM & Joint Secretary, Govt. of West Bengal Page 2 of28 ANNEXURE A Guidelines for peer assessment of UPHCs under Sushree (Kayakalp) 2019-20- For assessment, a team comprising of the following two assessors will visit the UPHC on date as given in Table I - (a) FTMO or PTMO (if FTMO is not present) He/she will be referred to as Assessor 1 (b) Nursing staffofUPHC or Pharmacist (if Nursing staff is not present) He/she will be referred to as Assessor 2 Thus each peer assessment team will comprise of two assessors - Assessor 1 and Assessor 2 In case where District team has been mentioned as peer assessor, CMOH of District may order a team of assessors based on the same format given above from any UPHC which has scored close to 70% (i.e. below 70%), and not qualified for peer assessment, and the same UPHC is close to the UPHC being assessed. Roster for assessment with dates for peer assessment of UPHCs is given in Table 1. Refreshment of asseSSOI'S- Med ical Officer in charge of assessed UPHC wiII arrange for refreshment of these two assessors, Checklist- Medical Officer in charge of assessed UPHC will arrange for photocopying of checklist out of fund allotted, Time of assessment- The team for assessment will reach the UPHC to be assessed by 1030 AM and must complete assessment by 3 PM. Scoring system- The team members will enter the score in separate checklists assigned for them and will do assessment separately. They must write name of the UPHC to be assessed positively in space provided. Name of assessor and his/her signature must be in the checklist he/she has assessed. Full compliance (100%) is to be given two (2) marks. Partial compliance (51%-99% compliance) is to be given 1 mark and no compliance (0%-50%) is to be assigned 0 marks. Remarks are to be written preferably in the specified column, by assessor where assessor has assigned zero (0) or one (1) for non compliance and partial compliance respectively. All these six checklists are to be assembled together and put in a sealed envelope and is to be submitted to the MO in charge of the facility assessed. It must be checked that all assessors have signed their respective checklists before the envelope is sealed. All two assessors and FTMO/PTNO ofthe UPHC assessed will sign on the sealed envelope. Total scores thus obtained by UPHC are not to be calculated by team members themselves at the facility being assessed. For districts, these score sheets will be collected by staff of District Programme Management Unit (DPMU) of NUHM, and DQAC and for Kolkata Municipal Corporation these score sheets will be collected by Staff of PMU (NUHM) of KMC. Data are to be entered in excel sheet and are to be submitted by 20.03.2020 to [email protected]. Total score obtained is to be sent in a broad sheet to [email protected] by 20.03.2020 positively with copy to DQAC/KMC for information.
