Prophylaxe von VTE bei immobilisierten Patienten mit hohem VTE Risiko, die wegen einer akuten Erkrankung wie einer Herzinsuffi zienz, akuter respiratorischer Insuffi zienz, akuter Infektion oder Entzündung behandelt werden. Behandlung der tiefen Venenthrombose (TVT) und Behandlung der akuten Lungenembolie (LE) wenn keine Lysebehandlung oder Embolektomie durchgeführt wird, in Kombination mit einem Coumarinderivat. Behandlung des akuten Koronarsyndroms mit instabiler Angina pectoris oder Myokardinfarkt ohne ST-Hebung (UA/NSTEMI). Behandlung des akuten Myokardinfarkts mit ST-Hebung (STEMI). D: Prophylaxe in der Chirurgie: postoperativ 1x täglich 2.5 mg subkutan, mindestens 6 Stunden nach Abschluss des Eingriffs und nach Überprüfung der lokalen Hämostase. Behandlung solange fortführen, bis kein Thrombo- embolie-Risiko mehr besteht, mindestens aber über 7 ± 2 Tage nach dem chirurgischen Eingriff. Bei Risikopatienten Behandlung um 24 Tage verlängern. Prophylaxe in der Inneren Medizin: immobilisierte Patienten erhalten 1x täglich 2.5 mg subkutan für eine Behandlungsdauer von 6–14 Tagen. Behandlung der TVT sowie der LE mit 7.5 mg 1x täglich subkutan (50–100 kg KG). Unter 50 kg KG beträgt die empfohlene Dosis 5 mg, über 100 kg KG 10 mg. Die Behandlung ist während mindestens 5 Tagen fortzusetzen, bis eine adäquate orale Antikoagulation erreicht ist. Behandlung UA/NSTEMI: 2.5 mg s.c. 1x täglich. Behandlung STEMI: 2.5 mg 1x täglich. Die erste Dosis von Arixtra wird als i.v. Bolusinjektion oder Injektion über 2 Minuten, alle folgenden Dosen s.c. gegeben. KI: akute schwere Blutungen, be- kannte Überempfi ndlichkeit gegenüber Fondaparinux, akute bakterielle Endokarditis, schwere Niereninsuffi zienz (ClKreat < 20 ml/min). VM: Bei Spinal-Epidural Anästhesie oder einer Lumbalpunktion, können spinale oder epidurale Hämatome nicht ausgeschlossen werden. Bei STEMI-Patienten wird die Anwendung vor und während einer primären PCI nicht empfohlen. IA: Die Behandlung mit Medikamenten, welche das Blutungsrisiko erhöhen können, muss abgesetzt werden. UW: Häufi g: Anämie, Blutungen, Thrombozytopenie, Purpura, Hypokaliämie, Schlafl osigkeit, Anomalien der Leberwerte, Makulo- papulärer Rash, bullöse Eruption, Harnwegsinfektionen, Ödeme. Gelegentlich: Thrombozytose, Schwindel, Benommenheit, Kopfschmerzen, Hypotonie, Übelkeit, Erbrechen, Bauchschmerzen, Dyspepsie, Gastritis, Obstipation, Diarrhö, erythemischer Rash, Rash, Pruritus, Reaktionen an der Einstichstelle, Fieber, periphere Ödeme, Wundsekretion. Selten: intrakranielle/intrazerebrale und retroperitoneale Blutungen, allergische Reaktion. González Additional information is available at the end of the chapter http://dx.doi.org/10.5772/64128 Abstract Infection with herpes simplex viruses type 1 (HSV-1) and type 2 (HSV-2) is extremely frequent in the human population, as well as recurrent reactivations due to lifelong infection. Infection and persistence of HSVs within healthy individuals likely results as a consequence of numerous molecular determinants evolved by these pathogens to escape both immediate and long-term host antiviral mechanisms. Indeed, HSVs harbor an arsenal of proteins that confer them stealth by negatively modulating immune function. Consequently, these viruses perpetuate within the host, altogether silently shedding onto other individuals. In this chapter, we discuss HSV determinants that interfere with cellular antiviral factors, as well as viral determinants that hamper innate and adaptive immune components intended to control such microbes. The identifica‐ tion of HSV evasion molecules that modulate the immune system, as well as the understanding of their mechanisms of action, should facilitate the design of novel prophylactic and therapeutic strategies to overcome infection and disease elicited by these viruses. This chapter is intended to provide an overview of the evasion mecha‐ nisms evolved by herpes simplex viruses to escape numerous host antiviral mediators. Abstract: Filoviruses, such as Ebola virus and Marburg virus, are of significant human health concern. From 2013 to 2016, Ebola virus caused 11,323 fatalities in Western Africa. Since 2018, two Ebola virus disease outbreaks in the Democratic Republic of the Congo resulted in 2354 fatalities. Although there is progress in medical countermeasure (MCM) development (in particular, vaccines and antibody- based therapeutics), the need for efficacious small-molecule therapeutics remains unmet. Here we describe a novel high-throughput screening assay to identify inhibitors of Ebola virus VP40 matrix protein association with viral particle assembly sites on the interior of the host cell plasma membrane.

The heat shock proteins (hsps) are a group of proteins that are overexpressed in cells in response to temperatures above the cells optimum growth tenq)erature. Cerebral ischaemia in vivo increases the levels of heat shock proteins and their mRNAs. A brief ‘sub-lethal’ ischaemic insult prior to a ‘lethal’ ischaemic stress has two effects. Firstly, hsps are overexpressed, and secondly, the extent of neuronal damage is significantly reduced as con^ared with models that undergo the single ‘lethal’ ischaemic insult.
