2016 COMMUNITY HEALTH NEEDS ASSESSMENT EXECUTIVE SUMMARY Community Health Needs Assessment, 2016 Update Purpose of the Community Health Needs Assessment This 2016 Community Health Needs Assessment (CHNA) updates the CHNA completed in 2013 to meet the requirements of Section 9007 of the 2010 federal law, The Affordable Care Act (“ACA”). The ACA requires that any tax-exempt, IRS- designated 501(c) (3) hospital complete or update a publicly-available, comprehensive CHNA every three years in order to document the extent to which it understands the unique characteristics and needs of the local communities it serves, and responds to these needs by delivering meaningful and effective community benefit through clinical services and other programming. Required Components A CHNA report has five required components: 1) Definition of community served 2) A prioritized description of the significant health needs of the community 3) Transparency in the process and methods used to conduct the CHNA, including how it took into account input from the community served and prioritized community health needs 4) A description of the resources potentially available to address the identified significant prioritized community health needs 5) An evaluation of the impact of actions taken to address the significant health needs identified in the previous CHNA report (June 2013). A CHNA report is considered complete when it is adopted by a governing body of the facility and made widely available to the public. Community Served NYC Health + Hospitals serve all New Yorkers in every neighborhood in New York City regardless of their ability to pay. Addressing disparity throughout New York City, NYC Health + Hospitals is the safety-net for the uninsured and underserved in New York City. -

Savoy and Regent Label Discography
Discography of the Savoy/Regent and Associated Labels Savoy was formed in Newark New Jersey in 1942 by Herman Lubinsky and Fred Mendelsohn. Lubinsky acquired Mendelsohn’s interest in June 1949. Mendelsohn continued as producer for years afterward. Savoy recorded jazz, R&B, blues, gospel and classical. The head of sales was Hy Siegel. Production was by Ralph Bass, Ozzie Cadena, Leroy Kirkland, Lee Magid, Fred Mendelsohn, Teddy Reig and Gus Statiras. The subsidiary Regent was extablished in 1948. Regent recorded the same types of music that Savoy did but later in its operation it became Savoy’s budget label. The Gospel label was formed in Newark NJ in 1958 and recorded and released gospel music. The Sharp label was formed in Newark NJ in 1959 and released R&B and gospel music. The Dee Gee label was started in Detroit Michigan in 1951 by Dizzy Gillespie and Divid Usher. Dee Gee recorded jazz, R&B, and popular music. The label was acquired by Savoy records in the late 1950’s and moved to Newark NJ. The Signal label was formed in 1956 by Jules Colomby, Harold Goldberg and Don Schlitten in New York City. The label recorded jazz and was acquired by Savoy in the late 1950’s. There were no releases on Signal after being bought by Savoy. The Savoy and associated label discography was compiled using our record collections, Schwann Catalogs from 1949 to 1982, a Phono-Log from 1963. T H E I N F O R M E R S Screenplay by Bret Easton Ellis & Nicholas Jarecki Based on the book "The Informers" by Bret Easton Ellis Black water flickers with light. Suddenly a pool is radiated an intense aqua blue. The sounds of a party fade in along with the ominous opening of The Go-Go's "This Town". A party in the backyard of a Bel Air mansion. 1983. It's paradisiacal: huge flowering trees lit by ground lamps, the massive steaming pool, a giant fountain spraying water, an endless maze of gardens, a house the size of a hotel looming over the scene. It's a dream, a mirage. The college kids that make-up the party are all interchangeable: blond, beautiful, tan. And everyone is dressed in the clothing of the era: pastels, Polo shirts with the collars up, tight jeans, pennyloafers, mini-skirts and Camp Beverly Hills tank-tops. GRAHAM (21) and CHRISTIE (19) are two of these perfect specimens. They make-out while sharing a joint on a chaise lounge by the pool. They blow smoke into each other's mouths and kiss hungrily, oblivious to the rest of the party. On the chaise lounge opposite them: MARTIN (20) and TIM (20). Tim does bumps of coke from a vial and is talking nonstop at Martin who just sips a Corona and stares coolly at the couple grinding into each other on the chaise. We can't hear anybody because of the music. BRUCE (20) makes his way down the steps leading to the pool, followed by a drunken RAYMOND (20), who's trying to explain something to him. -
