Contract Forms ..........................................................................................78 Section VIII Draft of Proposed Authorization ..............................................................86 Section IX Compliance Check List .................................................................................90 Section X Annexes ............................................................................................................93 NCB: IFA No. 02/NTA/2073/074 to build broadband network and provide internet access connectivity services in earthquake affected 7 districts. 4 Request for Application NEPAL TELECOMMUNICATIONS AUTHORITY (NTA) Kamaladi, Kathmandu, Nepal Invitation for Application to Build Broadband Network and Provide Internet Access Connectivity Services in Earthquake Affected 7 Districts (First Date of Publication: 17 November 2016) Nepal Telecommunications Authority, Kamaladi, Kathmandu is collecting two percent of Gross Adjusted Revenue as a Rural Telecommunication Development Fund (RTDF) every year from the Service providers in telecommunications sector as per the Telecommunication Act, 2053 B.S. and intends to apply a part of RTDF to cover eligible financial subsidy to the selected (based on this RFA) Internet Service Provider (ISP) licensee(s) to build Broadband Network and provide Internet Access connectivity services in earthquake affected 7 disctricts viz., Gorkha, Dhading, Makawanpur, Dolakha, Ramechhap, Sidhuli and Okhaldhunga by installing broadband access equipment and provide internet Access connectivity services to Public Colleges, Higher Secondary Schools, Secondary Schools, Health Centers and Village Development Committee offices in all the above mentioned districts. The Chairman, on behalf of NTA, invites Requests for Application (RFA) for the award of the contract from existing ISP licensees to build Broadband Network and provide Internet Access connectivity services in earthquake affected 7 districts within 12 months. These 7 districts are splitted into two packages and NTA will provide subsidy to the successful Applicant from RTDF to establish broadband network and provide internet/data access connectivity in all VDC offices, health centers and public educational institutions (colleges and high schools) for each packages that has quoted for the lowest subsidy for each packages. RFA document shall be made available from 20 November 2016. Eligible Applicants may obtain further information and inspect the RFA documents at the office of Nepal Telecommunications Authority, Kamaladi, Kathmandu, Nepal, Tel:+977-1-4255474, Fax: +977-1-4255250, Email: [email protected], www.nta.gov.np. The soft copy of the RFA document shall be uploaded to the NTA website for viewing purpose. RFA documents may be purchased from the above office by eligible Applicants on the submission of a written Application, and upon payment of a non-refundable fee of Rs 10,000.00. Pre-Application meeting shall be held at Nepal Telecommunications Authority, Kamaladi, Kathmandu, Nepal on 19 December 2016, 14:00 Hrs. Sealed Applications shall be submitted to the above office on or before 3rd January 2017, 15:00 Hrs. Applications received after this deadline shall not be accepted. Applications shall be opened, in the presence of Applicants' representatives who desire to attend, on 3rd January 2017, 16:00 Hrs at the office of NTA, Kamaladi, Kathmandu, Nepal. Application must be valid for a period of 90 (Ninety) days counting from the day of Application opening and must be accompanied by bid security, amounting to a minimum of NPR 6,250,000.00 (NRs. Six Million Two Hundred and Fifty thousand only) for Package A and NPR 7,500,000.00 (NRs. Seven Million Five Hundred thousand only) for Package B for this bidding, which shall be valid for minimum 30 days beyond the Application validity period. If the last date of purchasing, submission and opening falls on a public holiday then the next working day and stipulated time shall be considered the last day and time. NTA reserves the right to accept or reject, wholly or partly any or all the Applications.     Nepal Earthquake District Profile - Gorkha OSOCC Assessment Cell 09.05.2015 This report is produced by the OSOCC Assessment Cell based on secondary data from multiple sources, including the Government of Nepal, UNDAC, United Nations Agencies, non-governmental organisation and media sources. I. Situation Overview Gorkha, with a population of more than 271,000, is one of the worst-affected districts.1 The epicenter of the earthquake was in Brapok, 15km from Gorkha town. As of 6 May, 412 people have been reported killed and 1,034 injured. In the southern part of the district, food has been provided, but field observations indicate that the food supplied might not be enough for the actual population in the area. Several VDCs in the mountainous areas of Gorkha are yet to be reached by humanitarian assistance. There are no roads in these northern areas, only footpaths. The level of destruction within the district and even within VDCs varies widely, as does the availability of food. A humanitarian hub has been set up at the Chief District Officer’s (CDO) premises in Gorkha town. Reported number of people in need (multiple sources) The figures featured in this map have been collected via multiple sources (district authorities, Red Cross, local NGO, media). Where multiple figures for the same location have been reported the highest one was taken.     WASH Cluster Nepal 4W - May 12th 2015 Please find following the analysis of the 4W data – May 12th Introduction (Round 2) This is the second round of the 4W analysis. As this is the second round and still early in the emergency response, many agencies are still planning their interventions and caseloads, hence much of the data is understandably incomplete. In the coming week/s we will receive far more comprehensive partner data and will be able to show realistic gaps. In addition, we are receiving better affected population data and there are many ongoing assessments, the results of which will help us to understand both the response data and the affected population data and enable us to deliver a far more profound analysis of the WASH response. Please assist us as we have a lot of information gaps in the data provided so far and hence the maps are not yet providing a true picture of the response. We would like to quickly move to VDC mapping including planned/reached beneficiaries.
