To ensure this happens you need to: 1 Choose a dental care provider from the list below. 2 Complete the enclosed Transfer to Adolescent Oral Health Service letter 3 Return this to your current Dental Therapist/Oral Health Therapist before the end of the school year to ensure that your teenager is transferred
