Pizza Fridays For grades K – 5 5 – 21

Pizza Dates 14 Fridays of pizza and water: , 19, 26 2, 9, 16, 30 7, 14, 21 , 11, 18, 25

Pizza will be served every Friday that school is in session from October 12 – .

***Orders are due by Tuesday, *** to ensure that you child is served when Pizza Friday begins

1 slice + water = $65 OR 2 slices + water = $115

Payment Options:  Make a check payable to “PTA of PS 372.” If you have more than one child at the school, please send in separate orders. Enclose your order and payment in an envelope clearly marked “Pizza Fridays”.

 Pay online via paypal at http://inclusions.org (a surcharge of $2 - $3 will be applied).

 Not ordering? Please tell your child that he/she will not be receiving pizza, especially if he/she has participated previously.  Donations needed! Can you help sponsor a child? Consider giving an extra $5, $10 or $20!  Can’t afford pizza right now? No problem! If ordering presents a hardship, complete this form to request sponsorship. We will provide your child with 1 slice at no cost. Please consider volunteering to serve pizza.

PLEASE REMEMBER TO VOLUNTEER!!! PIZZA FRIDAY VOLUNTEER SIGN-UP SHEET Pizza Friday has been our school’s top fundraiser, and its success depends upon parent participation. Volunteers are needed weekly to serve the pizza to students from grades K through 5. It is a fun and easy way for you to help the school in a big way, and the children just love seeing their parents at lunch. Please sign up to volunteer for one or more weeks, as your schedule permits. Please only commit to those dates and times for which you will be available, as we rely on your commitment.
