Greetings and Peace in Our Lord Jesus Christ!

I have received your request for admission to the order of the [diaconate/priesthood] together with the Declaration of Freedom [and Profession of Faith with the Oath of Fidelity*] made by you according to the norm of 1036 of the Code of .

In fulfillment of my duty to exercise diligent care that Holy Mother the Church be enriched with worthy sacred ministers, I testify:
