Welcome to our Surgery. We are a practice combining three GP practices. Our practice teams have come together to provide excellent patient care for people in Thurrock. We are working together to deliver excellent care. Our aim is to provide high quality responsive healthcare delivered by an experienced, dedicated and supported primary healthcare team.

We offer a personal and friendly service and encourage our patients to take an active interest in their health care. We manage your illnesses and help to keep you fit by offering a range of preventative services. The practice is committed to improv- ing the healthcare experience for all its patients. Every part of the work is led by practising clinicians who listen to the views of their patients and will tailor their care to their needs. All the practices will close at 1pm on the 2nd Tuesday of each month for GP and staff training. In addition there are extended hours provided as part of local PCN. Please enquire at reception as it is currently being modified due to COVID-19 scenario

The surgeries are closed on weekends and public holidays if you need medical advise when the surgery is closed please ring the normal surgery number and your call will be put through to deputising service. You may be offered advice, an appointment to visit emergency surgery or home visit emer- gency surgery or home visit if necessary or call NHS 111.

In an absolute emergency dial 999 Please contact reception in person or by telephone to book an appointment. Make sure we have your contact details. This is to notify you in unlikely event of an emergency where appointment might be cancelled/changed or postponed .

MEDICATION REVIEWS We regularly review patients who are on long - term medication to ensure that they are on the most appropriate treatment and also monitor for patient’s safety. It is important that you attend surgery/telephone appt for monitoring. We may not be able to prescribe your medication if no timely reviews done

Confidentiality, Computer and Test Results All patient records on file/computer are confide ntial. To maintain confidentiality results will only be given to the patients or parent if the personis a minor. When we request investigations like blood, urine, x- ray or scan it is the responsibility of the patient to contact us to receive/review the results. Please call after 2pm .Please allow at least 5 working days for blood and urine results. Allow approx. 14days for any other investigations. ONLY PRACTICE STAFF, GP’S AND NURSES HAVE ACCESS TO PATIENT RECORDS UNDER A CONFIDENTIALITY AGREEMENT FOR KEEPING THESE RECORDS UP TO DATE AND ACCURATE.

Our Practice is committed to safeguarding children, young people at risk and has a responsibility to ensure that their practice staff know what to do if they encounter child or adult abuse or have con- cerns that they may be at risk of harm. The practice is committed to working within agreed policies and procedures and in partnership with other agencies to ensure that the risks of harm to a child or young person are minimised. This work may include direct and indirect contact with children (access to patient’s details, communication via email, text message/phone). The Practice will ensure that all staff are trained to a level appropriate to their role. New members of staff will receive induction training.

We aim to ensuring that all our surgeries are child- safe practice. Patient Participation Group (PPG) The objective of PPG is to promote the health and wellbeing of prac- tice’s diverse population. This is done by encouraging development and quality of existing health care services and includes health promo- tion.This is done by patients liaising with the GPs, practioners, nurses, practice staff and other community health workers. Carers, health au- thorities and other persons or organisations related to care provision may also attended . We request and encourage patients to actively participate in PPG. This helps to improve and standardise our care service provision. It will also aids in tailoring our services according to the needs of the local popula- tion. Complaints and suggestions We aim to provide a high quality service and are interested in learning how to improve the system we have. If you have any constructive sug- gestions please put them in the Suggestion & comments box located in the reception. If you have any complaints please ask to speak to Prac- tice Manager or Deputy who will investigate the matter once a com- plaint has been received or ask for our complaints procedure at Recep- tion. Complaints leaflet are displayed in waiting areas in all surgeries.

Zero Tolerance VIOLENT or ABUSIVE PATIENTS We will try our best to help you but we will not tolerate violent or abusive behaviour. Anyone verbally abusing either members of staff or the public, using inappropriate language will be asked to leave the premises . Such persons may receive a warning letter or may be asked to register elsewhere. ANYONE found to be VIOLENT OR CAUSES DAMAGE WILL BE REMOVED FROM THE PRACTICE IMMIDIATELY. POLICE WILL BE NOTIFIED.

Appointment to see the Doctor/Nurse/Practioners /physiotherapist We usually operate advanced appointment systems bookable up to two weeks ahead. Some appointments become available on the day. Please call at the time of the surgery opening to book, appointments are allocated on a first come first serve basis. Also there are online appointments available

Appointments attendance If you cannot keep appointment, please try to give us at least 2 hours’ notice so the appointment can be offered to other patients. Patients who fail to attend for appointment run the risk of being warned or removed from the practice list.

Test Results It is important that you check the results of any tests or investigations you have. Please do not assume the test is normal unless advised.

REPEAT PRESCRIPTIONS PLEASE NOTE- Requests should be made using the white repeat slip. Re- quests can be made online. Please ask reception for further information. Please allow 2 full working days for your request to be dealt with if the request was made before 11:30 am, after this time please allow another 1 day. We are going paperless with prescription, so we urge all patients to nominate a pharmacy so the prescription can be sent electronically.
