Circle of Friends has been organizing and coordinating special events since 2005 for our friends with special needs to have the opportunity to dance, socialize, have fun and meet new friends in a safe and non-threatening environment. They hold dances the second and third Fridays of every month. Occupational Therapy PACC is now offering Occupational Therapy Services through ODP and the Office of Long Term Living (OLTL) for individuals of all ages. Occupational Therapy encourages rehabilitation through the performance of activities required in daily life. Contact Tyler Kweder at (412) 621-6151 ext. 2022 or [email protected] or contact Heather Tortorea at HTortorea@ paconnectingcommunities.org Supportive Employment Services 1) Job Coaching and Support: Some functions of job support include training of individuals in job assignments, periodic follow-up or ongoing support. Other activities include support to relearn a job, training to assist individuals to use transportation for work, maintenance of appropriate work behavior while on the job. 2) Job Finding and Development: Development of skills; outreach to potential employers; training of job tasks; and other such tasks to assist individual in acquiring a job. Some functions of job finding include interview assistance, employer outreach and orientation, resume preparation, and job skill development. 3) Career Assessment: A service that consists of gathering and reviewing individuals interests, skills, and work/volunteer history. We also conduct assessments to assess individual’s interest and aptitude in a specific job while identifying types of jobs in the community that match interests, strengths, and skills. We also assist with developing a career assessment report with recommendations that address needs, interests, strengths, and characteristics of specific work environments. 4) Supportive Employment: If an individual is deemed non-competitively employable through OVR, they may be eligible to receive Support Employment Services through waivers. Home and Community Habilitation Services W7059 1:2 7058 1:3 W7060 1:1 Services to help people gain, maintain, and improve skills that allow them to live and participate in their local community. This service may be made available to individuals in their own home or in other residential or community settings not subject to licensing regulations. Community Participation Services W5996 100% CPS 1:1: PACC offers 100% Community Participation supports. PA Connecting Communities can provide services for people living in private homes, or supportive living arrangements, including transportation to and from participant home or another agreed upon location. We provide our participants with an opportunity to live ‘Everyday Lives’ by encouraging social activities, friendships, and community inclusion and is a great alternative to traditional day programs and workshops. Each consumer will have an individualized Community Participation Plan based on their goals, wants and needs. Support individuals based upon assessment, those with behaviors that prevent an individual from living or working without crisis. The provision includes the training of individuals, staff, relatives and caregivers. PACC contracts with The Office of Developmental Programs (ODP), The Office of Vocational Rehabilitation (OVR), The Office of Long Term Living (OLTL), and The Bureau of Autism Services. PACC is here to help with the process of transitioning from high school to adulthood. PA Connecting Communities (PACC) is proud to offer our new high school transition program and work based learning experiences (WBLE) through our PETS/OVR contract. As a contracted OVR provider we conduct evaluations and assessments to prepare your child for a successful transition into the workforce. These assessments not only evaluate strengths and needs on the jobsite but also help to identify possible career paths. We have many programs that will benefit your child in school and out in the community. PACC offers life skills, social skills, recreation, transitional development, and vocational planning. In addition we provide programs for youth on the autism spectrum.
