II. EFFECTS:

A. INCIDENTS MONITORED (TAB A)

• As of 07 December 2012, fifteen (15) incidents were monitored ; seven (7) of which were maritime incidents, four (4) were flooding incidents, one (1) trash slide, one (1) whirlwind incident, one (1) stranded shipping and one (1) tornado incident.

B. AFFECTED POPULATION (TAB B)

• As of 07 December 2012, the total number of population affected in 2,178 Barangays / 223 / 31 cities in 27 provinces of Regions IV-B, VI, VII, VIII, X, XI, XII and increases to 1,077,541 families / 5,317,275 persons. • The total number of population currently served inside and outside the evacuation centers are 82,104 families / 393,026 persons .

C. CASUALTIES (TAB C)

• As of 07 December 2012, 456 were reported dead and 445 were injured while 533 are still missing.

D. STATUS OF STRANDEES (TAB D)

• As of 07 December 2012 , a total of 40 passengers , 9 vessels and 5 motor bancas were stranded in ports all over the .

E. STATUS OF LIFELINES (TAB E)

• Thirteen (13) bridges and twelve (12) roads are not passable. • Thirty-four (34) municipalities and cities experiencing power interruption. • Five (5) areas are experiencing interruption in water supply. • Davao Oriental experiencing communication interruption.

F. DAMAGED HOUSES (TAB F)

• A total of 28,587 damaged houses were reported in Regions IV-B, VI , VII, X, XI and CARAGA. Of which 11,002 were partially damaged while 17,585 were totally damaged .

G. COST OF ASSISTANCE (TAB G)

• A total of PHP 35,295,960.93 cost of assistance : PHP 20,918,130.00 from DSWD and PHP 14,342,726.93 from LGUs while PHP 35,104.00 from NGOs/other GOs .

H. COST OF DAMAGES (TAB H)

• A total of PHP 4,001,924,688.02 cost of damages : PHP 630,970,000.00 damages in infrastructure, PHP 3,365,534,688.02 in agriculture, and a total of PHP 5,420,000.00 worth of damaged private properties.

I. DECLARATION OF STATE OF CALAMITY (TAB I)

NATIONAL PREPAREDNESS

1. NDRRMC

• NDRRMC conducted an Emergency Preparedness Meeting re Typhoon ""PABLO"" on December 2, 2012 at 2:00 PM at the NDRRMC Operations Center to discuss the preparedness measures of concerned agencies and regional offices. It was attended by representatives from PAGASA, DILG, DOH, DSWD, AFP, PNP, PIA, BFP, PCG, PRC and MMDA • Meeting of Emergency Rapid Assessment Team held on 05 December 2012 for the conduct of emergency rapid assessment on areas gravely affected by Typhoon "PABLO"

2. NDRRMC Member Agencies

PAG-ASA • Assigns Detailed Duty Officer (DDO) at the NDRRMC OPCEN for prompt coordination with their respective bureaus; Continuously disseminates Severe Weather Bulletins, provides media interviews re weather updates, continuously track the movement of Typhoon "Bopha"

AFP • 8th Infantry Division, TOG8-PAF, PCG, PNP and PN response teams are on standby for possible deployment.

PNP • Issued Memo to all RDs in affected areas and concerned NSUs on December 2, 2012, directing them to activate respective Disaster Incident Management Task Groups (DIMTGs); forward deployment and prepositioning of Search and Rescue (SAR) assets; inform/warn community leaders on necessary disaster preparedness/actions giving emphasis on pro active actions in terms of pre-emptive evacuation rather that rescue; and coordinate with the local DRRMCs likely to be affected by Typhoon ""PABLO""; provide police assistance in the evacuation of residents and deployment of personnel at the evacuation centers; clearing of major thoroughfares and highways in coordination with DPWH and LGUs; provide assistance in the conduct of medical and relief operations, and conduct of actual SAR operations

PRC • PRC VII prepositioned relief goods in City and held an emergency meeting for the tasking of volunteers • PRC VI held an emergency meeting and coordinated Vice Chairman Sy re assigning one (1) truck for evacuation purposes.

PCG • Warned the operators of small vessels, MBCAs and FBCs’ to avoid sailing in the south-eastern and eastern section of the country • CGS Davao has cancelled all voyages from Sta. Ana Pier Davao City bound to Kaputian and Talicud Island, Island Garden of Samal • PCG & NAVY of VI suspended sea vessels trips

DPWH • Directed its District Engineers in areas likely to be hit by Typhoon "Bopha" to be on stand-by and preposition all heavy equipment in strategic areas • DPWH IV-B prepositioned heavy equipment in strategic areas and coordinated with DPWH Field offices for possible clearing operations.

DOH

• DOH HEMS issued directive to CHDs likely to be hit by Typhoon "Bopha" to raise code white alert status and activate their respective disaster operations center (DOCs) in Regions V, VI, VIII, IX, X,XI, XII, CARAGA and ARMM • DOH-HEMS raised Code Alert from Code White to Code Blue and sent continuous weather updates to all regions affected thru SMART Infoboard • CHD IV-B, V, VI, VIII, IX, X, CARAGA and ARMM is on code white alert status effective 04 December 2012. • CHD IV-B in continuous coordination to all CHD Extension Offices in MIMAROPA Provinces, OCD and DOH Hospitals including Provincial Hospitals • CHD VIII - inventory of drugs, medicines and supplies and prepositioned logistics. • DepEd Disaster Risk Reduction teams have been activated and readied schools for use as evacuation centers.

BFP • BFP Tandag City together with PNP conducted roving and announced the upcoming Tropical Storm "PABLO" using its public address system to the communities near the shoreline areas and assisted in the pre emptive evacuation conducted in the same area.

• RDRRMC VII and all PDRRMCs activated their Disaster Operations Center and their Search and Rescue Teams; Cebu City, Mandaue City and Lapu Lapu City DRRMCs also met for preparations; all Provincial Social Welfare Office have prepositioned food and non-food items; regional office readied 2,000 family packs containing rice and canned goods ‹ Dumaguete Mayor declared suspension of work in government offices on 04 December 2012. ‹ LDRRMC Bogo City coordinated with the National Food Authority and City Social Welfare and Development for prepositioning of relief goods. ‹ Pre-emptive evacuation was done in six (6) coastal barangays in Amlan, Negros Oriental; in the low lying banks of Moalong River in Loon to Basac Elementary School; Sitio Pulangbukod, Alang-alang, Mandaue City; and in the coastal Barangays of Argao, Cebu. ‹ Seventy (70) families living along flood prone areas of Brgy Casbu and Trinidad of Guindulman, Bohol have been evacuated to higher grounds. • RDRRMC VIII closely coordinate with local PAGASA, LDRRMCs, RDRRMCs, member agencies, PNP and 8ID and placed rescue teams on standby ‹ CDRRMC-BISLIG - Activated all men in uniform (75th IB, PNP, PCG, Reservist) as standby responders. ‹ CDRRMC-TANDAG - Provision of standby vehicle (10 units) for massive evacuation if needed. ‹ MDRRMC-HINATUAN - Mass evacuation conducted this afternoon is some coastal areas. ‹ MDRRMC-LINGIG - Stockpiling of goods and force evacuation late afternoon to the residents living in the islands, near riverbanks and mountain slopes area (temporarily housed at their municipal gym). ‹ DRRMC Tacloban City, Baybay City, Hilongos, Southern Leyte, Ormoc City and Eastern Samar convened on 03 December 2012 re preparation on Typhoon "PABLO". • RDRRMC IX spearheaded the emergency meeting to prepare and coordinate all concerned agencies for Typhoon "PABLO". The said meeting was held at about 10:00 AM, 04 December 2012 at the OCD, RDRRMC IX Warehouse in Pagadian City. ‹ CDRRMC Pagadian City identified the evacuation centers in their AOR namely: Southern Mindanao Colleges Annex Gym, Tiguma Gymnasium, Pagadian City Gymnasium, and Plaza Luz Covered Court. • RDRRMC X held meetings re preparations of respective LDRRMCs and their alert level activation for the severe weather disturbance ‹ PDRRMC MISAMIS ORIENTAL Chair Gov. Oscar Moreno held an emergency meeting of inter-agencies and all its MDRRMCs discussing the preparations for the weather disturbance and ordered the suspension of work in the provincial capitol in exemption of the staff from PHO doctors, engineering, PDRRMC and PSWDO, OPCEN / Incident Command Team and all department heads. The PDRRMC has been on BLUE Alert since 02 December 2012. ‹ PDRRMC MISAMIS OCCIDENTAL conducted pre-emptive evacuation in Brgys Aloran, Jimeniz, Tudela, Sinacaban and Clarin. ‹ PDRRMC Camiguin and MDRRMCs Mambajao and Mahinog prepositioned personnel and equipment ‹ CDRRMC Cagayan de Oro conducted an emergency meeting on resource mobilization, response protocols, activation of BDRRMC OPCENs, its pre-emptive evacuation plan and assignment of evacuation centers for residents of flood prone areas, and prepositioning response assets for possible dispatch. ‹ DRRMC Iligan Chair Mayor Lawrence Cruz presided the expanded ExeCom meeting and series of committee meetings on the response preparedness measures for the typhoon ‹ Municipality of Talakag - Alerted 3 barangays located along the river banks (Barangay Coceña, Ticoloan, San Isidro). MDRRO provided handheld radio to barangays at risk to serve as feedback monitoring regarding status of water level ‹ LDRRMC-Cagayan de Oro City - implemented forced evacuation for selected barangays with the assistance of PNP and 4ID PA. • RDRRMC XI instructed LGUs to conduct pre-emptive evacuation in low-lying and flood prone areas ‹ PCG, TOG 11 PAF, 5053rd SRS, PCG, PNP 11, Eastern Mindanao Command augmented disaster response to the affected areas ‹ PDRRMC ComVal together with PNP and Phil. Army conducted road clearing ‹ RDRRMC XI Chairperson, Dir Liza Mazo presided an emergency meeting along with members of the RDRRMC XI response committee to assess the support capabilities for the affected LDRRMCs. ‹ PDRRMC/MDRRMC, PNP SARO teams, 67th IB PA of AFP, and LGUs of East coast municipalities led the conduct of force evacuation of the residents residing near the coastline. ‹ CDRRMC Tagum City, Davao Del Norte evacuated residents living near the coastal areas in Brgys Busaon and Libuganon at about 11:30 PM, 03 December 2012. ‹ MDRRMC of Banaybanay ordered pre-emptive evacuation on 3 December 2012 at 9:30 PM to all local communities especially those residing near coastal areas. ACTIONS TAKEN

1. NATIONAL

‹ NDRRMC Operations Center was on RED ALERT STATUS since 02 December 2012 and is continuously monitoring and disseminating 24 hour Public Weather Forecast, Severe Weather Bulletin, Gale Warning, and General Flood Advisories to all RDRRMCs/OCDRCs through SMS, facsimile and websites for further dissemination to their respective Local Disaster Risk Reduction and Management Councils (LDRRMCs) from the provincial to the municipal levels. • In view of the landfall of Typhoon Pablo over Baganga, Davao Oriental at 4:45 AM, 4 December 2012, the NDRRMC planned to implement the cluster approach coordination mechanism to immediately address the needs and requirements in the areas in Mindanao. • As part of the cluster approach humanitarian response, the UNDAC team through the UN–OCHA arrived in Manila to support the NDRRMC in the conduct of immediate Rapid Damage and Needs Assessment (RDANA) • The NDRRMC OPCEN, in coordination with the UN Humanitarian Country Team (UNHCT), conducted a brief initial planning meeting on 4 December 2012 9:00 AM for the conduct of Joint–NDRRMC-UNHCT- RDANA • It was agreed in the meeting that Regions X, XI and CARAGA will be prioritized for the initial conduct of RDNA and second assessment in Regions IV-B, VI, VII and VIII • Thus, the NDRRMC issued two (2) Memoranda :

° First, inviting the national and global cluster leads /or any representative to attend the Inter-cluster coordination meeting on 4 December at 1:00 PM to discuss the conduct of the Joint NDRRMC-HCT RDANA in the above regions. The agenda of the meeting will cover the following: initial update of impact of TY Pablo; issues and concerns, organization of the joint NDRRMC- HCT team (terms of reference and timeline of deployment)

° Second, informing the Regional Directors of OCD Regions X, XI and CARAGA that three (3) teams will conduct a Joint NDRRMC-HCT RDANA in their respective regions. Further, directing them to schedule a regional briefing for the Joint RNA upon arrival and to provide the itinerary and logistics for the rapid needs assessment teams in terms of accommodations, meals, vehicles, working area and internet connection. Also to provide the team with list of Regional counterpart (name and agency) and list of most affected areas that has not enough information/data received.

‹ The ASEAN Coordinating Center for Humanitarian Assistance (AHA) on Disaster Management along with the joint NDRRMC-HCT was deployed to conduct Rapid Needs Assessment (RNA) on 05 December 2012 in the Mindanao area to gather information/data on humanitarian needs. The AHA Center is also willing to provide support to the NDRRMC on the requirements in the disaster stricken areas. Moreover, the NDRRMC-OPCEN issued a Memorandum Order regarding the OCD Team who will join the joint NDRRMC-HCT RNA.

‹ UN-OCHA prepares their team for humanitarian action in all areas affected by Typhoon Pablo.

‹ AFP prepositioned a PAF Aircraft for the use of OCD5 for disaster response and assessment operation in Bicol, particularly in Masbate province and Ticao Island. ° The 65IB and 51IB of the Phil Army were dispatched to assist the population affected by flooding in Kapai, Tagoloan and Bubong in Lanao Del Sur. ° 67IB and 701Bde conducted search and rescue and retrieval operations at Municipality of Boston, Davao Oriental.

‹ DPWH provided three (3) trucks for the transport of 2,000 family packs from DSWD to the province of Davao Oriental. ‹ DPWH XI conducted assessment and coordinated clearing of roads.

‹ DILG provincial office identified the immediate needs in Compostela Valley. The needs are as follows: potable water, relief goods (food) and clothing. ‹ PNP thru PCRG donated 1,800 pcs. Blankets for relief operations for the flood victims. ‹ PNP-PRO IV-B activated DRRM Committee, assisted stranded passengers in Oriental Mindoro, Occidental Mindoro, Romblon, and provided assistance at the evacuation centers in Palawan. ‹ PNP-PRO X deployed 142 SAR Teams and cooperated in the joint clean-up operations conducted by various government agencies in the affected areas. ‹ PNP-PRO XI conducted search, rescue and retrieval operations in Compostela Valley and Davao Provinces. ‹ PNP MARITIME GROUP - RMU 8 deployed teams in Samar and Leyte. RMU 10 also deployed teams in Cagayan de Oro, , Oroquieta, Mahinog, Camiguin, Iligan, Lugait, and Manticao. RMU 11 deployed teams in Davao del Norte. ‹ BFP RO 10 conducted search and rescue operations, clearing operations, assisted in the evacuation centers, and rationed water to evacuation centers. ‹ BFP Tandag City Fire Station personnel distributes goods to the evacuees accomodated in Tandag City Gymnasium. ‹ NGCP Bohol activated its operation center to respond to power outages due to broken lines. ‹ DSWD is continuously managing evacuation centers nationwide. Food and non- food items were distributed in affected areas. ‹ DSWD XI provided 14,500 food packs for Davao Oriental and Compostela Valley. ‹ PNRC provided hotmeals and distributed relief items and bottled waters in evacuation centers in affected areas. SAR Teams were also deployed. ‹ DA XI in coordination with NIA XI conducted assessment on damages to agriculture. ‹ DOH coordinated with UNICEF for provision of three (3) units of Mobile Water Treatment Device, two (2) for Davao City and one (1) for CARAGA, 450 units of Latrine Slabs, and 100-200 units of Portable Toilets. Nutrition Cluster Partners involved in the Initial Rapid Assessment are UNICEF, WFP, Save the Children, ACF and Plan International. ° CHD-XI is on Code Red Alert Status, provided fifty (50) cadaver bags to Command Post in New Bataan, Compostela Valley, and deployed Assessment Team in New Bataan, Compostela Valley. ° DOH-CO prepared PHP 5,455,112.23 worth of assorted drugs and medicines, Aquatabs, Lime, Cadaver Bags, Waterine and Oral Rehydration Salt to be delivered to CHD XI and CARAGA.

‹ OCD V convened RDRRMC Emergency meeting attended by agency heads of DILG, DOST, DSWD, MGB, PAGASA, PHIVOLCS, PIA, APSEMO, TOG 5-PAF, and representatives from PA, PN, PNP, BFP, PCG, DOH and members of media. RDRRMC Bicol recommended to Masbate to conduct pre-emptive evacuation to high risk communities. ‹ PDRRMC Catanduanes coordinated with the PNP, PRC, PCG, and Phil Army and was dispatched in the Barangays affected by storm surge brought about by Typhoon Pablo. ‹ Albay PDRRMC convened for a pre-evacuation conference to discuss and plan requirements in the evacuation phase ‹ RDRRMC V Operations Center downgraded from RED to BLUE Alert Status effective 8:00 AM, 05 December 2012

REGION VI

‹ PDRRMC Guimaras provided food assistance to the stranded passengers bound for Iloilo and Guimaras upon the suspension of motorboat trips by the PCGWV. ‹ CDRRMC Bago City evacuated thirty nine (39) individuals in Brgy Poblacion for pre-emptive measures. ‹ CDRRMC Bago City passed Executive Order (1) 1 No. 21 suspending the work in the City of Bago, (2) No. 22 an order suspending classes in both public and private Elementary and Secondary levels, and (3) No. 23 an order suspending classes at all levels in both public and private educational institution in the City

REGION VII

‹ Dumaguete City Brgy Officials evacuated residents along the coastal line of Banica River. ‹ RDRRMC VII convened a meeting at around 11:00 AM, 04 December 2012. The member agencies presented their preparedness updates then created the Rapid Assessment Team composed of the Office of Civil Defense, DSWD, DPWH, DILG, NEDA, DA, NIA, NTC, DOH, DepEd, DTI, and AFPCC. ‹ DSWD VII increased their prepositioned food packs from 2,000 family packs to 3,000 packs containing rice and canned goods. ‹ DSWD Lapu-Lapu City provided relief goods to evacuation sites. ‹ Cebu Water District deployed water trucks to evacuation areas identified by LGUs. ‹ Provided PHP25,000.00 financial assistance to the five (5) injured victims hit by fallen trees at PHP5,000.00 each for computerized tomography (CT) scan ‹ On 05 December 2012, a total of 504 residents were evacuated from seven (7) low-lying barangays: Sipakit, Panabutan, Catuyan, Pulang Lupa, San Vicente, St. Mary and San Nicolas

REGION X

‹ OCD X responded to calls for assistance by various affected citizens. ‹ Bukidnon Rescue and Response Team were deployed in different Brgys with reports of fallen trees, electric posts, and other fallen objects due to strong winds. ‹ PDRRMC Misamis Oriental dispatched three (3) dump trucks and two (2) rio trucks for relief distribution to affected families from the municipalities of Misamis Oriental. ‹ PDRRMC ICP / OPCEN dispatched three (3) dump trucks covering the Eastern Municipalities from Sugbongcogon to Magsaysay; one (1) rubber boat stationed at Medina, Misamis Oriental; two (2) rubber boats covering the Western Municipalities from Opol to Lugait; and two (2) 6x6 trucks for augmentation and assistance for Brgy Molugan, El Salvador City and Municipality of Manticao. ‹ CDRRMC El Salvador and MDRRMC Kinoguitan distributed food and non-food items.

REGION XI

‹ CDRRMC Mati City, Davao Oriental evacuated 60 persons living near the coastline of Brgy Dahican, Mati City at about 12:15 AM, 04 December 2012.

REGION XII

‹ BDRRMC Balabag spotted six (6) minor landslides in Sitio Mawig, Balabag, Kidapawan City and evacuated 11 families / 45 individuals. ‹ CDRRMC Kidapawan is preparing for relief operations for the affected families.

CARAGA

‹ 402nd Infantry Brigade installed SSB transceiver at the RDRRMC Operations Center for gathering of indormation ‹ LDRRMCs Surigao Del Sur, Surigao Del Norte, Agusan Del Sur and Dinagat Islands facilitated the distribution of relief goods to evacuees in their respective Areas of Responsibilities (AOR). ‹ LDRRMCs Surigao Del Sur facilitated the distribution of relief goods to evacuated individuals. ‹ RDRRMC deployed an Emergency Rapid Assessment Team (ERAT) composed of representatives from OCD, DILG, DSWD, DPWH, DA, NEDA, DOH and DepEd in the areas of Lingig to Tandag, Surigao Del Sur to conduct Rapid Damage and Needs Assessment and identify the need requirements of the Internally Displaced Persons (IDPs) in the evacuation centers. ‹ DOST, AFP, PNP, PCG, DA, DOH, DILG, PRC, DPWH and DSWD CARAGA deployed their assets including vehicles and equipment. ‹ PDRRMC Agusan del Sur dispatched DANA Team and conducted assessment of the affected population and damages to agriculture, infrastructure and lifeline. ‹ PDRRMC Agusan Del Sur prepositioned Food and Non-food items including engineering equipment, transport vehicles, generator sets and rescue equipment.

ARMM

‹ At 9:00 AM, 04 December 2012, the RDRRMC, the Responders Team, and the UN OCHA convened for another meeting at the OPCEN, ORG ARMM as part of the region's preparedness and response activities. ‹ All public hospitals and health centers within the jurisdiction of the Integrated Health Provincial Office (IPHO) are placed under White Code Alert ‹ Offices were directed to activate Health Emergency Response Team in close coordination with their respective LGUs ‹ PDRRM Lanao del Sur provided shelter, relief and medical assistance to the people of Ditsaan Ramain which served 945 Families. ‹ Provincial Government and DSWD conducted relief operations and DOH extended medical assistance. ‹ The province of Maguindanao received 54 trapals distributed, LGU and DSWD conducted relief operation, DOH extended medical assistance and water supply from BFP Kabacan.
