This dismemberment is performed on a living fetus unless the abortion provider has induced fetal demise prior to initiating the evacuation procedure. HB 1429 prohibits a physician from knowingly performing a dismemberment abortion. The bill defines a dismemberment abortion as: An abortion in which a person, with the purpose of causing the death of an fetus, dismembers the living fetus and extracts the fetus one piece at a time from the uterus through the use of clamps, grasping forceps, tongs, scissors, or a similar instrument that, through the convergence of two rigid levers, slices, crushes, or grasps, or performs any combination of those actions on, a piece of the fetus’ body to cut or rip the piece from the body. The bill provides an exception to this prohibition under certain circumstances if a dismemberment abortion is necessary to save the life of a mother and no other medical procedure would suffice for that purpose. Any person who knowingly performs or actively participates in a dismemberment abortion commits a third degree felony. The bill exempts a woman upon whom a dismemberment abortion has been performed from prosecution for a conspiracy to violate this provision. -

Abstract Patient preference for medications in place of osmotic dilators as cervical preparation is well known. Twenty-four to 48 hours following mifepristone, vaginal and buccal misoprostol have demonstrated equal efficacy in second-trimester medical abortion but have not been compared as cervical preparation for second-trimester surgical abortion. This study will randomize participants undergoing a second-trimester surgical abortion between 16 0/7-20 6/7 weeks gestation to 200mg mifepristone followed 20-24 hours later with 400mcg vaginal misoprostol and buccal placebo versus 400mcg buccal misoprostol and vaginal placebo 1-2 hours prior to the procedure. Primary outcome is intraoperative procedure time as measured by time of first instrument in the uterus (dilator if further dilation is required or forceps if no further dilation required) to last instrument out of the uterus. The study uses a superiority design and is powered to detect a 4-minute difference in procedure time from a baseline of 10±5 minutes with 90% power and two-tailed alpha of 0.05 requiring 33 participants in each arm. -

However, the role of an important COX-2 metabolic product, prostaglandin (PG) I2, in AD development remains unknown. Using mouse-derived astrocytes as well as APP/ PS1 transgenic mice as model systems, we firstly elucidated the mechanisms of interferonγ (IFNγ) regulation by PGE2 and PGI2. Specifically, PGE2 accumulation in astrocytes activated the ERK1/2 and NF-κB signaling pathways by phosphorylation, which resulted in IFNγ expression. In contrast, the administration of PGI2 attenuated the effects of PGE2 on stimulating the production of IFNγ via inhibiting the translocation of NF-κB from the cytosol to the nucleus. Due to these observations, we further studied these prostaglandins and found that both PGE2 and PGI2 increased Aβ1–42 levels. In detail, PGE2 induced IFNγ expression in an Aβ1–42-dependent manner, whereas PGI2-induced Aβ1–42 production did not alleviate cells from IFNγ inhibition by PGI2 treatment. More importantly, our data also revealed that not only Aβ1–42 oligomer but also fibrillar have the ability to induce the expression of IFNγ via stimulation of NF-κB nuclear translocation in astrocytes of APP/PS1 mice. The production of IFNγ finally accelerated the deposition of βA 1–42 in β-amyloid plaques. -

49 Medical vs. surgical methods for second trimester abortion Lohr et al. (2008) compared dilatation and evacuation (D&E) to medical methods of abortion in the second trimester (≥ 13 weeks), specifically intra-amniotic installation of prostaglandin F2α and mifepristone and misoprostol. The outcomes considered were complications, side-effects, completion of abortion and patient satisfaction. Although this review is from 2008, it is considered up-to-date as a recent litera- ture review revealed no additional studies which would meet inclusion criteria. Only two trials were included, one addressing each comparison. Gestational age ranged from 13 to 20 weeks among included trials. The trial quality is rated as low, given only one trial is included in each comparison and for the D&E versus mifepristone and misoprostol comparison the trial was very small (n=18) and had a primary outcome (feasibility of randomising US women to one of two methods of abor- tion) differing from the outcomes assessed in the review. The review found that the incidence of combined minor and major complications was lower with D&E compared with installation of prostaglandin F2α (Table 33). Fewer women experienced adverse events with D&E compared with mifepristone combined with misoprostol, although there were no differences in efficacy between the two groups. These results should be interpreted with caution given they are based on one small trial (n=18). The authors conclude that D&E is superior to installation of prostaglandin F2α and that the limited available evidence also favours D&E over mifepristone and misoprostol for decreased rates of adverse events. -

In this review, we focus on their influence on platelets, which are key elements in thrombosis and hemostasis. The function of platelets is influenced by mediators in the blood and the vascular wall. Activated platelets aggregate and release bioactive substances, thereby activating further neighbored platelets, which finally can lead to the formation of thrombi. Prostanoids regulate the function of blood platelets by both activating or inhibiting and so are involved in hemostasis. Each prostanoid has a unique activity profile and, thus, a specific profile of action. This article reviews the effects of the following prostanoids: prostaglandin-D2 (PGD2), prostaglandin-E1, -E2 and E3 (PGE1, PGE2, PGE3), prostaglandin F2α (PGF2α), prostacyclin (PGI2) and thromboxane-A2 (TXA2) on platelet activation and aggregation via their respective receptors.
