Who is the service for? The services are based within the Child Development Centre’s at Diana, Princess of Wales Hospital (Grimsby) and St. Nicholas House, Shelford Street (Scunthorpe) Our NHS service is for young people aged 0 to 18 years who have been identified as having Occupational Therapy needs. Children can be referred to our NHS service by their GP, Consultant, School Nurse, Health Visitor or other Health Professional. Further guidance about making a referral is available at the end of this leaflet – Appendix One. Please note we are only commissioned to see children who are registered with a North Lincolnshire or North East Lincolnshire GP. Children, who live or attend school outside of our area, but have a North or North East Lincolnshire GP, will only be offered clinic based appointments and not home/school visits.

What to expect? Your child will receive an initial assessment in a clinic with a registered Occupational Therapist within four weeks of referral. Following that assessment, a plan of care is agreed and written for your child detailing advice, strategies and programmes to follow at home and/or school if required. Your child may be offered a block of therapy intervention or may only need to work from their programme at home/school. Your child will be discharged if after assessment they do not require further Occupational Therapy intervention.

Children/young people may require Occupational Therapy if: 1. They have a neurological condition or neuromuscular disorder 2. They have a syndrome which affects their gross/fine motor development 3. They have a condition where their function or access to daily activity is impaired 4. They are under 5yrs and as part of the Multi-Disciplinary Team assessment at the Child Development Centre have been identified with functional motor skills delay e.g. sitting, using cutlery, bathing and dressing etc. 5. They require post-operative or post illness rehabilitation

6. They have a diagnosis of Developmental Coordination Disorder and have been identified as having significant difficulties with their fine and gross motor skills that cannot be addressed by education staff or carers 7. They have been identified as needing specialist equipment and/or assessing for a Disabled Facilities Grant to support access to essential facilities and age appropriate daily living activities. (Please note in North East Lincs this is only for children who are under 5 yrs old, unless they are already known to the service)

You do not need to be seen by an Occupational Therapist if: 1. Your child is within the typical milestones range for gross/fine motor development 2. Your child has sensory difficulties only. Please note your child will receive a holistic assessment where their sensory skills development will be addressed. Strategies will then be offered as part of the recommendations to support involvement in age appropriate tasks. Sensory Integration Therapy is not offered 3. Your child has a diagnosed condition but there is no impact upon pain, function or participation in age appropriate activities 4. Your child has handwriting difficulties only

What will happen after assessment? If Occupational Therapy intervention is required, you and your child will be offered therapy sessions, either with an Occupational Therapist or a Therapy Assistant. These sessions usually take place in a clinic room. This may involve: • A block of therapy sessions, for a short specified period of time (up to 6 sessions) working on specific Occupational Therapy targets • An individual programme which can be completed by you outside of the therapy sessions • Review sessions with your Therapist to update targets and programmes

Children with an Education and Health Care Plan If your child has a specific Occupational Therapy target on their Education and Health Care Plan the therapist will work with the education setting towards achieving this target. Updated information for the Local Authority as part of the Education and Health Care Plan process will be provided when requested. Your child will be discharged from our service when: 1. Your child’s functional gross/fine motor skills are following typical developmental patterns or are within the typical range for their age/developmental level 2. Your child’s gross/fine motor skills are in line with their general development and learning 3. Your child’s targets have been met 4. Advice and support have been given and you can continue with strategies and activities at home and/or school. The Therapist will advise you to refer your child again if they need further intervention 5. Support strategies have been given but there is no current evidence on how this is being implemented 6. You do not bring your child for their appointments

Appendix One: Referral guidance A health professional (e.g. GP, Health visitor, School Nurse, Paediatrician.) identifies a concern about a child’s physical development. This is discussed with carers prior to making a referral.
