EMERGENCY INCIDENTS MONITORED (Tab E) • Moderate to occasionally heavy rains and strong winds brought about by Typhoon Mina triggered the occurrence of 42 emergency incidents such as: 13 flashfloods/floodings, 7 landslides, 9 tornadoes , 5 sea mishaps , 1 vehicular accident, 6 collapsed structures, and 1 soil erosion in various areas of Regions I, II, III, VI, VII, VIII, IX, CAR and NCR • Barangays along riverside in Dingras, Ilocos Norte is still isolated due to underwater

F. DAM / RIVERS MONITORING • Dams status as of 4:00 PM, 29 August 2011 - La Mesa, Angat, San Roque, Ipo, Pantabangan and Caliraya, are still below critical level; Ambuklao (five (5) gates opened @ 3 meters); Binga (five (5) gates opened @ 4.5 meters); and Magat (1 gate opened at 2 meters (620 cms) • Rivers as of 4:00 PM, 29 August 2011: Montalban, Nangka, Marikina, San Juan, Fort Santiago, Pandacan and Angono are still below critical level

G. STATUS OF LIFELINES

1. STATUS OF ROADS AND BRIDGES (Tab F) • As of 6:00 PM, 29 August 2011 , a total of 11 bridges and 31 road sections remain closed to traffic due to a series of major and minor slides, scoured slope protections, eroded shoulders, and floodwaters ( 11 roads in Region I; 7 bridges and 1 road in Region II; 2 bridges and 1 road in Region VI ; 1 bridge in NCR; and 19 roads in CAR)

2. STATUS OF POWER, WATER AND COMMUNICATION LINES • NGCP reported that 3 major transmission lines (La Trinidad-Bulalacao 69 kv line, Solana-Tabuk 69 kv line, and Tuguegarao-Magapit-Camalanuigan-Sta. Ana 69 kv line) tripped on the morning of 27 August 2011

Region I • Water supply in Ilocos Norte (provincial wide) has been interrupted • Power, communication, telephone lines, and transportation in Ilocos Norte are still operational

3 Region II • The province of Cagayan has no electricity since 26 August 2011 and some areas in Isabela particularly from Santiago City to San Pablo and Basco, Batanes are still experiencing power outage. (Power in Isabela was restored last 28 August 2011) • Water and Communication lines are still functional except for some PLDT lines (OCD R2 internet is not operational since 27 August 2011)

Region III • Several areas in Northern are experiencing power failures after transmission lines tripped due to Typhoon “Mina” As of 6:00 AM 28 August 2011, Dilasag, Aurora has still no power due to damaged electric post/power lines of local electric cooperatives

CAR • Power interruptions were experienced in Kalinga province (Tabuk area) since 11:00 PM, 26 August 2011; some areas in Baguio City, Atok and in Benguet due to damaged lines and fallen electric posts; a wide brown-out in Mt. Province since 10:00 AM, 27 August 2011; Flora, Sta. Marcela and Luna in Apayao have electrical problem. • Power in some areas in Baguio City Lower Apayao are not yet restored as repair by BENECO is ongoing; Kalinga, Mt Province and Abra (power restored)

III . EMERGENCY MANAGEMENT

A. NATIONAL RESPONSE AND PREPAREDNESS

• NDRRMC OpCen was activated to “Blue Alert” status effective 8:00 AM, 22 August 2011 to ensure continuous dissemination of weather bulletins and general flood advisories to all Regional Centers and RDRRMCs, PDRRMCs and OCDRCs in Regions I, II, III, IV-A, IV-B, V, VI, VII, VIII, IX, X, XI, XII, CAR, CARAGA, NCR and ARMM; and to closely monitor the preparedness measures being undertaken by

NDRRMC and concerned RDRRMCs’ member agencies relative to Typhoon “Mina”

‹ Effective 5:00 PM, 25 August 2011, NDRRMC OpCen was upgraded to RED ALERT status in view of the intensification of Typhoon “Mina” into a typhoon. NDRRMC member agencies such as: AFP, PNP, PCG, BFP, DOH, DSWD and DPWH were directed by USEC BENITO RAMOS to report at the Emergency Operations Center at Camp Aguinaldo to closely monitor the possible effects of Typhoon “Mina” ‹ USEC BENITO T. RAMOS directed all OCD Regional Directors with reported dead casualties in their AORs to provide PhP10,000.00 each dead victims for the bereaved families

• PAGASA representative is rendering duty at OpCen to closely monitor the movement and development of Typhoon “Mina” and continuously monitoring the status of dam water levels particularly Ipo, Magat and Binga

• DA activated all its Regional Command Centers to closely monitor any possible effects in the agricultural sector

• PIA ensured the timely information dissemination of bulletins/advisories and assisted the RDRRMCs in disseminating weather updates to the public through PIA news text blasting

4 • PRC is on a 24-hour alert status and placed on stand-by its WASAR teams, CISD teams, and PRC Volunteer response augmentation; prepositioned stockpiled food packs, first-aid kits and spine boards, four (4) rubber boats, three (3) ambulances, one (1) Toyota Land Cruiser, and one (1) mobile water treatment equipment equipped with a generator and water pump; and five (5) boxes of biscuits and packs of noodles good for 3-40 families for Aringay Amphitheater Community Kitchen

• DSWD-DROMIC alerted its Field Offices (FOs) along the typhoon path; advised FOs to closely monitor the situation particularly those flashflood and landslide–prone areas within their jurisdictions and the status of their standby funds and stockpile; closely coordinate with various field offices for any report of affected population; and pre-positioned PhP26.207,379.95 brokendown as follows : stand-by funds (PhP2,769,108.37) and worth of relief goods (PhP23,438,271.58), readily available for augmentation to LGUs affected:

‹ DSWD I pre-positioned 1,500 food packs for the entire region and prepositioned 500 family packs in Ilocos Norte; provided PhP277,972.50 worth of relief assistance to the affected families; continuously implement the cash for work program (canal de-clogging) as flood mitigation activity in flood prone areas in the provinces of Ilocos Sur, Pangasinan and La Union, and activated their SWAD team (pro-active stance in the province of Ilocos Norte and Ilocos Sur) for any eventualities ‹ DSWD II pre-positioned food and non-food items (3,579 family food packs and 3,290 packs of clothing in the 65 identified evacuation centers for augmentation support to the LGUs; provided PhP190,000.00 worth of relief assistance to the affected families; and alerted the Disaster Management Response Teams (DMRT) including the Social Welfare and Development (SWAD) teams of Batanes, Cagayan, Isabela, Quirino and Nueva Vizcaya. Sufficient stockpiles of food packs, used clothing and standby funds are readily available ‹ DSWD III SWAD team and PDRRMCs/MDRRMCs have enough stockpile ready to respond for any untoward situation ‹ DSWD VI through the MDRRMCs of Moises Padilla, Binalbagan and Isabela in Negros Occidental, provided PhP157,250.00 worth of food assistance to the affected families ‹ DSWD CAR pre-positioned 2,000 family packs in Benguet, Mt. Province, , Abra, Kalinga and Apayao; Quick Response Fund (QRF) worth

5 PhP500,000 is available; and identified evacuation centers and location of community kitchen; provided 100 family food packs to the affected families amounting to PhP25,000.00; provided bottled mineral water for the responders of vehicular accident in , Benguet; and provided PhP74,754.00 worth of relief assistance to the affected families. Repacking of family packs is ongoing ‹ DSWD NCR alerted and activated its Quick Action Response Teams (QUART) to continuously monitor the LGUs in 17 municipalities and cities in Metro Manila and coordinated with MMDA Flood Control Unit

• Philippine Coast Guard Action Center directed all Coast Guard Districts (CGD) to ensure high state of preparedness of their respective stations, detachments, operational control units and disaster response assets in preparations for TS Mina; reminded CGDs to always make sure that their stations and detachments promptly initiate and/or encourage aggressive public information dissemination through wide use of the tri-media and LGUs in warning the coastal residents and local fishermen; advised CGDs to closely coordinate with PCG Auxiliary and LGUs, local DRRMCs and media in information dissemination (warning/recalling fishermen; and reminded all Shipping Lines to strictly follow HPCG Memo 01-09 (Guidelines on the movement of motorboat/vessels in the absence of weather bulletin) and HPCG SOP No.002-09 (Guidelines on PCG Alert Condition/levels) ‹ CGDNL activated all District Operations Centers and alerted all WASAR Teams in their AORs for any possible response mobilization. Each WASAR team is equipped with rubber boats: one (1) in Pangasinan; four (4) in La Union; one (1) in Ilocos Sur; and One (1) in Ilocos Norte ‹ PCG Station Aparri under SCPO Ernesto Renon and PCG Detachment San Vicente under PO1 Anthony Estoesta both of Cagayan are placed on alert status ‹ CG Northern Luzon through the CG Detachment San Vicente, responded to the maritime incidents involving MV Eagle Ferry, MB 4 Brothers con Queen and 2 other motorbancas in Sta Ana, Cagayan ‹ Coast Guard Station San Fernando City immediately dispatched three (3) SOG and two (2) personnel which conducted SAR operations but it was temporarily terminated with negative result. SAR resumed using water vessel DF-325 as soon as sea condition improves ‹ PCG Aurora and Bantay Dagat fully implemented the prevention of fisher folks and other small vessels from venturing out into the sea ‹ Elements of CG Detachment Vitas and MEPU personnel of CGS Manila were dispatched and conducted investigation re maritime incident involving M/Tug Unity II which sunk at vicinity of Pier 18, Vitas, Tondo, Manila ‹ CG Action Center alerted CGD Eastern Visayas, CDS Cebu, CGS Masbate and CGD Cataingan, Masbate re maritime incident involving MV Maria Josefa which was listing 10 degrees to starboard due to shifting of cargo caused by rough seas ‹ CGS Sual, Pangasinan directed SARV-002, SARV-3502 to conduct search and rescue operation/aerial reconnaissance for the 23 missing fishermen in Infanta, Pangasinan ‹ PCG Infanta directed PCG BN Islander Aircraft 251 to conduct SAR/aerial reconnaissance operations in support to the ongoing CGDNLZ surface SAR operations and coordinated for additional support from the Philippine Navy

6 • DOH-HEMS activated Code “White Alert” in all CHDs, sent alert Memo to all CHDs along the typhoon path; stockpiled medicines and medical supplies; and released assorted drugs and medicines in 21 provinces of four (4) CHDs (CHD I- PhP300,000, CHD II-PhP210,000, CHD III-PhP730,000 and CHD CAR- PhP300,000 ‹ CHD 1 stockpiled assorted drugs and medicines in Ilocos Norte, Ilocos Sur, Pangasinan and La Union and pre-positioned Php75,000.00 worth of assorted drugs and medicines in each provinces. ‹ CHD CAR stockpiled medicines at the regional and four (4) provincial offices, established medical and health assessment teams (doctors, nurses and allied health professionals) on standby in respective offices; directs coordination and linkages with the health clusters of four (4) PDRRMCs and major hospitals in the region

• DPWH CO through the Bureau of Maintenance, instructed all DPWH Regional Directors concerned to activate their District Disaster Action Team and monitor status of national roads and bridges within their area of jurisdiction ‹ DPWH I activated its District Disaster Action Teams; deployed maintenance crew to perform clearing works/provision of turn outs, installed informative/warning signs to inform motorists about the situation, deployed road grader to the affected areas to clear slides; continuously monitor the status of road in the AOR; and placed on standby resources at the following provinces: ° Ilocos Norte – four (4) dump trucks, two (2) payloaders, one (1) payloader-dozer, two (2) road graders, 14 service vehicles, 16 maintenance crews, eight (8) engineers, ten (10) maintenance capataz ° Ilocos Sur – one (1) payloader, one (1) road grader, two (2) dump trucks, two (2) Toyota Hi-Lux, and one Mitsubishi Canter ° La Union – one (1) dump truck, one (1) road grader, one (1) backhoe, one (1) payloader-dozer, service vehicles, and 63 manpower ° Pangasinan – six (6) dump trucks, one (1) generator set, one (1) Toyota Hi-Lux, One 91) Mitsubishi pick-up truck, one (1) Ford Bronco, one (1) Nissan Vanette, one (1) Hyundai van, four (4) Ford Fiera, one (1) backhoe, one (1) payloader, two (2) road graders, one (1) Bomag road roller, one (1) payloader-dozer, two (2) pick-ups, two (2) chainsaws, and 12 manpower ° DPWH La Union conducted sandbagging operations to restore the eroded shoulder and scored slope protection in Agoo-Baguio Road. Hence, Damosil Bridge is already passable to all types of vehicles ‹ DPWH II deployed all District Disaster Teams, equipment and crew to the critical road section. Clearing and removal of debris along the national roads on the northern part of Cagayan is ongoing ‹ DPWH III pre-positioned resources for possible emergency and clearing operations (2 payloaders, 1 bulldozer, 1 grader, and 1 dump truck were deployed in Brgys. Puncan and Putian, Carranglan, Nueva Ecija, and 1 grader in Dalton Pass) and closely monitors possible landslides in Aurora Province and Dalton Pass, Maharlika Highway, Carranglan, Nueva Ecija ‹ DPWH CAR pre-positioned 76 heavy equipment (government and private) with manpower at the landslide prone areas and districts and maintenance crew are on standby ‹ DPWH District Antique conducted clearing operations in the affected areas and provided assistance to the affected barangays

• AFP pre-positioned and rationalized DRRO assets and equipment in disaster-prone areas; placed OPCON DRTUs to CENTCOM, NOLCOM, SOLCOM and NCRCOM; and alerted AFP local Commands (PA, PAF, PN) on standby for immediate deployment ‹ AFP units in Region 1 is on standby for the implementation of “OPLAN DAGLI” a tool in mobilizing AFP resources during disaster response and placed the following resources on standby: nine (9) M35 trucks, two (2) trailer trucks, two (2) ambulances, three (3) Navy Patrol Boats, four (4) rubber boats, and DRTG teams ‹ AFP-PA II Units in and Cordillera under MGEN ROMMEL GOMEZ AFP are placed on Red Alert for any eventualities ‹ 17 IB PA Cagayan conducted SAR operations at Brgy Casambalangan, Sta Ana and assisted in the conduct of evacuation in Gonzaga and Sta Teresita, Cagayan ‹ 41IB PA headed by Lt Col Kurt Decapia pre-positioned their resources to augment the possible evacuation operations in Northern Aurora; and personnel of 18 th Special Forces Company reported at RDRRMC III ready for augmentation in areas to be affected ‹ TOG 5-PAF deployed 1 DRRT team in Bato, Camarines Sur ‹ NAVFORSOL deployed 1 DRRT team (1 Officer, 9 Enlisted personnel with 1 M35 truck and rubber boat with OBM) in Polangui, Albay on 25 August 2011 ‹ 9ID PA dispatched two (2) M35 trucks with 14 enlisted personnel pre- positioned at OCDRC-5 Camp Ola, Legaspi City to Polangui, Albay on 25 August 2011 to provide mobility for evacuation ‹ AFP Units in CAR collaborated with local DRRMCs re: pre-positioning of assets and response mobilization ‹ NOLCOM have standby force ready for emergency assistance ‹ 503 rd Brigade conducted SAR operations for the two (2) missing persons in Abra ‹ J3, AFP directed NOLCOM to provide one (1) Helicopter to conduct DANA, evacuation and SAR to the three (3) municipalities of the Province of Ilocos Norte namely: Piddig, Solsona and Dingras ‹ PAF/TOG-1 Ilocos Norte will conduct aerial survey/assessment ‹ SAR Groups (3ID-PA, TOG6, Navy, PCG , BFP, and volunteer groups) in Region VI are on standby for possible mobilization

• PNP activated PNP National Disaster Operations Center for possible mobilization of different PNP Disaster Incident Management Task Groups and pre-positioned PNP SAR assets; and coordinated with LGUs for pre-emptive evacuation ‹ PRO1 alerted its Provincial Police Offices/Stations, conducted ocular inspection in the flooded areas of La Union, assessed their Disaster Preparedness Plan and conducted an inventory of resources and equipment for rescue and relief operations: 275 SAR trained personnel; 30 rubber boats, two (2) PNP ambulances, and 275 vehicles of various kinds ‹ PNP-PRO 2 under PC/SUPT Francisco Villaroman is placed on Red Alert Status for any eventuality. SAR Units of PPO Cagayan PPO, IPPO Isabela Santiago Police Office, together with their respective P/CDRRMC are on standby for any deployment

8 ‹ PPO Isabela conducted an emergency Conference at the Provincial Capitol of Isabela presided by PSSUPT Jimmy U. Rivera, CESE, PD to discuss preparations relative to Typhoon Mina ‹ PNP PROCOR activated its Operations Center, alerted all police stations, assisted LGUs in the dissemination of alert notices and pre-emptive evacuation, and attending to search and rescue operations and traffic management ‹ PPO Iloilo evacuated 43 families to nearest halls in Iloilo City: 15 in Brgy. So-oc, 7 in Brgy. Desamparados, 7 in Brgy. Bakhaw, and 14 in Brgy. Calahunan

• BFP -NHQ issued memorandum to all Regional Directors in regions likely to be affected and directed all BFP units personnel to be on standby ‹ BFP CAR, I, II and III directed all its emergency response teams and operating units to be on alert status for possible mobilization and to take precautionary measures in case there is an abrupt increase in water level in low-lying areas ‹ BFP I pre-positioned one (1) rubber boat in each of the following areas: Vigan City, Ilocos Sur; Naguilian, La Union; Rosales, Pangasinan; and Urdaneta City, Pangasinan ‹ BFP Ilocos Norte Provincial Headquarters and San Nicolas Fire Station Personnel proceeded to Brgy. 18 Pari-ir, Ilocos Norte to help the victims of tornadoes ‹ Bauang Fire Station together with MFM conducted rescue operation and evacuation of affected population to Calumbaya Elementary School, Calumbaya Bauang, La Union ‹ PNP Region II Provincial and Municipal Fire Marshalls coordinated with their respective P/MDRRMCs for possible evacuation or rescue assistance ‹ BFP 6 EMS-ORD, Oton Fire Station Personnel, together with Oton Search and Rescue Team, Bantay Karagatan, and MDRRMC Oton rescued and evacuated 87 persons to Oton National High School and Oton Elementary School

• DILG issued memorandum to all its regional centers on the implementation of pre- emptive evacuation in high risk areas if situation demands and disseminates weather advisories to LGUs concerned, and to continuously monitor the situation and coordinate with Local Chief Executives (LCEs) in areas with typhoon signals

• NTC alerted all accredited Civic Communications Group to assist the NTC on communication concerns as emergency communication back-up in case communication system failed

• NGCP sent personnel to areas with tripped transmission lines to assess the damages and restore the transmission lines

C. SUMMARY OF REGIONAL PREPAREDNESS

• RDRRMCs/OCDRCs I, II, III, IV-A, IV-B, V, VI, VII, IX, NCR and CAR immediately initiated their preparedness activities as early as 21 August 2011 when the LPA east of Visayas developed into a tropical depression. The preparedness measures undertaken by the above RDRRMCs were the following:

‹ Activation of respective disaster operations centers to ensure prompt dissemination of warning information such as general flood advisories, gale warnings and weather bulletin of Typhoon Mina to the local disaster risk reduction management councils from the provincial down to the municipal/city and barangay levels, media and general public in the community ‹ Alerting of local Disaster Risk Reduction Officers (DRROs), emergency responders, Task Forces and SAR and relief teams of various RDRRMCs member agencies, actions officers and provincial coordinators ‹ Prepositioning of local resources on hand in terms of relief supplies, equipment and logistics for immediate deployment if need arises ‹ Close monitoring of the situation and status of dams, river systems and tributaries in respective areas of responsibility ‹ Possible implementation of pre-emptive evacuation of residents in high risk areas if needed

RDRRMCs / PDRRMCs RESPONSES

Region I • RDRRMC/OCDRC I continuously disseminates warning information to RDRRMC Members, LGUs, association of fisher folks, communities and local radio stations; recommended the activation of local DRRMCs and response teams; strict implementation of pre-emptive evacuation in high risk areas if situation demands; emergency meetings; and coordinated with PDRRMCs for possible assistance from the Regional Council • PDRRMC Ilocos Norte distributed relief goods to the flooded Barangays in Piddig, Ilocos Norte • LGUs Piddig and Sarrat, Ilocos Norte provided PhP28,662.00 worth of relief assistance to their affected constituents: Sarrat (PhP17,612.00) and Piddig (PhP11,050.00)

10 • PDRRMCs Pangasinan, La Union, Ilocos Sur and Ilocos Norte activated their respective CDRRMCs, MDRRMCs, and BDRRMCs • PDRRMC Pangasinan pre-positioned rescue assets (personnel and rescue equipment) to Alaminos City (1 dump truck), Mangaldan (4 dump trucks and 2 rubber boats of LGU), San Fabian (5 dump trucks or reservist/naval detachment/WASAR/PHERT/PNP, 1 military truck, and 1 rubber boat of naval detachment), San Jacinto (1 dump truck of PN), San Nicolas (1 dump truck of PNP), and Tayug (1 dump truck) • The Sangguniang Panlalawigan of Ilocos Norte thru Provincial Resolution No. 052-2011 declared the province of Ilocos Norte under a State of Calamity on August 28, 2011 • LGUs in Ilocos Norte are still conducting search, rescue and relief operations and damage and needs assessment specifically in San Isidro, Sto Tomas, Sarrat, Dingras, Solsona and Bacarra

Region II • PDRRMC Cagayan closely coordinated with the MDRRMCs in the northern tip of Cagayan including Sta.
