Croydon Health Services provides integrated NHS services to care for people at home, in schools, and health clinics across the borough as well as at University Hospital and Purley War Memorial Hospital.

CUH provides more than 100 specialist services and is home to the borough’s only Emergency Department and 24/7 maternity services, including a labour ward, midwifery-led birth centre and the Crocus home birthing team.

Purley War Memorial Hospital (PWMH) in the south of the borough offers outpatient care, including diagnostic services, physiotherapy and ophthalmology services run by Moorfields Eye Hospital, alongside an onsite GP surgery.

The appointment is fixed term for 6 months.

The appointment is covered by the terms and conditions of service for Hospital Medical and Dental Staff.

The Job Description is a reflection of current service needs and may be reviewed and amended, in consultation with the post holder, in the light of future changes or developments.

The appointee will have a continuing responsibility for the care of patients in his/her charge and for the proper functioning of his/her department. He/she will undertake the administrative duties associated with the care of his/her patients.

Medical and dental staff will be required to assist management and its agents during investigations into claims alleging medical negligence and, where requested, to provide related factual statements.

Within the terms of DHSS Circular HC (88) 9 – Protection of Children – applicants are required when applying for this post to disclose any records of convictions, bind-over orders or cautions.

Main conditions of service The appointment will be on the basis of Terms & Conditions of Service for Hospital Medical & Dental Staff. Local arrangements have been reached and implemented by the Trust through a formal negotiating process. Any subsequent changes will be notified to the successful candidate and thereafter form part of his/her contract of employment. The arrangement of duties will be such as may be agreed from time to time between the employing Trust and the person appointed.

Pay The salary scale for this post is MN37 with a range from £30,002 - £34,402. Your starting salary will be determined upon commencement.

Payday - The monthly payday is the 26th day in each month. Your salary will be fully available on payday itself, but please remember that most banks quote “at end of business the preceding day” when asked about available funds. Some building societies may take longer to make funds available to you.

Registration It is a condition of your appointment to be appropriately registered with the General Medical Council with a License to Practice. It is your responsibility to present certificates to the HR Department on an annual basis. Failure to maintain registration could lead to disciplinary action being taken against you.

Indemnity The Trust will cover all medical and dental staff for NHS work under NHS Indemnity. The Trust is required to encourage medical and dental staff to ensure that they have adequate defence cover for any work that does not fall within the scope of the Indemnity Scheme (HSG (96) 48 refers). Any private practice undertaken on NHS premises must be covered by subscription to a medical defence organisation.

Funds No special funding can be made for any additional supporting staff, specialist equipment or other facilities unless such provision has already been specifically agreed.

Criminal Records Bureau All Trust staff are subject to a Police check (DBS Check) and are required to complete and return the Disclosure application form, with the relevant documentation.

Rehabilitation of offenders All Medical and Dental posts are exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act 1974(Exceptions) Order 1975. Applicants are, therefore, not entitled to withhold information about convictions which for other purposes are ‘spent’ under the provisions of the Act. Failure to disclose such convictions could, in the event of employment, result in dismissal or disciplinary action by the Trust. Any information given will be completely confidential and will be considered only in relation to the particular post.

Appraisal/Revalidation All Trust staff must participate fully in the annual appraisal process.

All staff members are expected to demonstrate the behaviours that support our patient promises and these will be incorporated into the appraisal process.

Performance Continuation of your appointment under this Agreement is subject to satisfactory performance. Your performance and progress in relation to agreed standards will be reviewed at regular intervals with the Medical Director/Clinical Directors. Unsatisfactory performance could be considered as grounds for action under the appropriate Disciplinary Procedures.

Radiation/Ionizing Protection Under the Ionizing Radiation Regulation Act, (1988) the Trust is required to maintain a register of all persons in its employment who, as part of their clinical duties, are required to clinically or physically direct a medical examination or procedure involving the use of Ionizing radiation. Such persons must demonstrate to the Trust that they have received “Core of Knowledge” training in the safe use of ionising radiation, before their name can be entered on the register. Because the Trust has an obligation to ensure compliance with these training requirements, any person who fails to register with the Trust having received proper notification to do so, and who subsequently clinically or physically performs an examination or procedure using Ionizing radiation, can be prosecuted under these regulations. It will be an essential requirement for the successful candidate to be in possession of an IRMER (Ionising Radiations Medical Exposures Regulations) 2000 Certificate. Following the Ionising Radiation (Medical Exposure) Regulation Act, (2000) the post holder would act as a referrer. In addition, if the post holder is taking responsibility for, and is involved in physically administering a radioactive substance for the purpose of patient therapy, they will assume the roles of practitioner and operator. Schedule 2 of the regulations requires the post holder to be adequately trained and have the necessary experience. As a guide the regulations state:

“A registered medical practitioner acting as practitioner under these Regulations, wishing to take responsibility for a range of nuclear medicine procedures would require approximately one year’s experience and additional theoretical training of approximately one week.” Evidence of experience is required to meet these requirements. The above replaces the requirements of the 1988 Ionising Radiation Regulation.

Occupational Health All prospective employees are required to complete a Work Health questionnaire. This is primarily to identify any disability that may require support at work. Staff who have patient contact, will be required to undertake screening for tuberculosis and advised of additional immunisations and screening, dependent on their job role or produce satisfactory evidence. Clinical staff who are to undertake exposure prone procedures (EPP) may be required to undertake screening for HIV, Hepatitis B and Hepatitis C, prior to this work or produce satisfactory evidence. Staff who fail to comply with recommended guidance may have restrictions placed upon their work. IT Requirements for All Medical Staff It is a condition of employment that all medical staff comply with the Trust’s IT policy in respect of secure use and safeguarding confidentiality.

It is a condition of employment that the post holder:

Attends and satisfactorily completes training on Trust’s clinical IT systems as they relate to the medical and dental care and management of patients Conforms to the Trust’s IT security policy, which includes: Safeguarding the confidentiality of his/her password Not allowing the use of his/her password by anyone else Logging off promptly at the end of each task Not using other people’s passwords Not misusing the Internet facilities available through the Trust’s IT system Safeguarding patient confidentiality The Trust will require all dental staff to hold the European Community Computer ‘Driving Licence’ as soon as this becomes a requirement for continued registration with the General Dental Council. The Trust will provide adequate IT training. The Trust monitors the use of its IT facilities, passwords, e-mail and Internet traffic.

Failure to comply with the Trust’s IT security policy will be taken seriously and may result in formal disciplinary action and has resulted in dismissal for severe offences.

Annual Leave Appointments of twelve months duration give an entitlement of 27 days. Annual leave can only be taken Monday to Friday – on call weekends must be swapped and this must be with agreement of your Consultants and notification to the Business Support Administrator, together with the completed swap form. Where posts rotate at three (7 days), four (9 days) and six (13.5 days) monthly intervals, all leave must be taken before commencing the next part of your rotation.

Some posts within some specialties have rostered leave built within the shift pattern, and therefore there is no further entitlement to annual leave. It is not possible to swap weeks of annual leave with your colleagues.

Application for all leave must be applied for, giving six weeks notice. The application must be signed by a Consultant member of the applicant’s team and handed to the Service Manager/Business Support Administrator. The annual leave form is available via the Trust intranet.

With regard to prospective cover, forms still need to be completed and it must contain the name of your colleague who will be undertaking the duty and a swap form completed and duly signed by all parties concerned.

The Service Managers of your directorate are responsible for all forms of leave i.e. A/L, Special Leave etc.

Study Leave Application must be made six weeks prior to the leave and submitted to the PGMC with accompanying information (e.g. Conference leaflet, etc). A letter of confirmation of study leave will be sent from the Postgraduate Medical Centre Department after authorisation from the Medical Director. Please contact the Postgraduate Medical Centre for confirmation of your study leave entitlement.

Sickness If you are unable to work due to sickness you must notify the following immediately:  Consultant  Service Manager/Business Support Administrator for your directorate

Local guidance on reporting procedures will be in place and made available to you at Departmental Induction. It is necessary for you to make contact on a daily basis, unless you have been signed off sick by your GP for a confirmed period, which will be detailed on the certificate.

A Trust self-certificate must be completed for all days up to and including the seventh day of sickness (including any day from Monday to Sunday). Absence beyond seven days requires a medical certificate obtained from your GP and appropriate forms completed in line with the Trust’s Sickness and Attendance Management Policy. Sick leave cannot be paid without submitting these forms.

It is important to contact Service Manager/Business Support Administrator and your Consultant of any sickness that will prevent you from working in order for the Trust to cover and maintain the service. Failure to do this will result in unauthorised absence and no pay.

Health & Safety Employees are required to be aware of health and safety legislation and Trust policy and should take responsibility for ensuring this is adhered to. You have a responsibility to follow the incident reporting system, which will ensure that the Trust can manage risks to patients, visitors and staff.

CROYDON

The Surrounding Area The Borough of Croydon is the largest of the London boroughs and the tenth largest in the UK with a population of 340,000. It is one of the outer London Boroughs and is coterminous with the Croydon Primary Care Trust, sharing boundaries with Lewisham, Bromley, Sutton and Caterham and Tandridge.

It has one of the largest shopping centres in South East England with modern developments like the which characterize modern Croydon.

There are still however, numerous features recalling its prosperity in earlier times. The Archbishop’s Palace (now a girl’s school) was occupied by the Archbishops of Canterbury until the 18th century. The oldest shop in the town can be seen in the south end, a 16th to 17th century building with over hanging upper storey. The Wrencote in the high street is a fine example of an 18th century town house.

Other places of interest include the 16th century Whitgift Hospital (an almshouse). A Flemish style 19th century Town hall and the parish church of St. John the Baptist, rebuilt in 1867 - after a fire - by Sir Gilbert Scott, in the original 15th century style. Croydon and its surrounding area are ideally placed for visits both to London and the country. There are a whole host of sites to suit everyone from the moated Hever Castle to the entertaining expanse of Crystal Palace Park. Historic houses like Chartwell, home to Sir Winston Churchill, some marvellous museums and family fun venues like Chessington World of Adventures and Thorpe Park are all easily accessible for enjoyable trips all year round.

Travel The Croydon area is very well connected for travel by rail and provides regular access to places within the local Croydon area and many fast trains to central London (Victoria and London Bridge) and Thameslink. These services operate round the clock which makes access to London west end particularly easy. From there are good services to Gatwick Airport, Brighton, Sussex, Hampshire and Dorset.

The first tram service in London for many years (Tram link) was opened in 2001 and connects Croydon with Wimbledon, , and Beckenham.

By road, there is quick and easy access to the M23 and M25 motorways, Gatwick Airport being reached in less than half an hour and Heathrow in approximately 50 minutes.

Recreation and Leisure Croydon has a selection of indoor and outdoor recreation facilities. These include golf courses, leisure and fitness centres, tennis, swimming and sports clubs. Crystal Palace Sports Centre lies close to Croydon.

Within easy range of Croydon are numerous country parks and gardens including Park, Boxhill and Country Park. Brands Hatch motor racing circuit is within a 40 minutes drive.

Croydon Clock Tower houses a central library, life times - a local museum, and exhibition gallery and the David Lean cinema which shows art house films. Theatres within Croydon include the and . The offers venues for classical music and other varied shows.
