A guideline for the types of services that AT&T Telco will bill is provided in the "Billing Policy Guidelines" section of this binder. Depending upon the type of charge, AT&T Telco will determine whether the charge will be deniable or non-deniable. If a charge is non-deniable, end-user service denial cannot be utilized if payment is not received on these charges. If state regulation changes and loss of toll denial occurs, all charges will be considered non- deniable. In addition, AT&T Telco business office procedures vary, as per the contract, for non-deniable (non-transmission) charges. Charges for these records vary based upon the service being billed. Refer to your B&C Agreement for applicable charges. Detailed Process Requesting Billing of Miscellaneous Charges/Enhanced Services AT&T Telco requires that all Customers requesting to bill miscellaneous charges/enhanced services obtain written notification that the information has been reviewed. Customers must provide AT&T Telco with a detailed written description of the service. The description must include the following: • adhere to all Truth-In-Billing requirements • details of what the service is (what service the customer receives) • the price of the service • how the service is accessed • the charge description that is to appear on the bill (text phrase) • an indication that the charge is either recurring or non-recurring Additionally, AT&T Telco will be reviewing the text phrase form and the supporting documentation to determine if the two documents match. Please refer to Exhibit I for a form that should be used when requesting such billing. Although AT&T Telco does not require testing for these types of messages, AT&T Telco systems must be updated to allow for processing of the different record types. The Customer's implementation forms should be updated to indicate this type of billing will be processed. If a Customer desires testing, this can be arranged through the Time & Cost Process. Billing and Collections Product Binder Section 23 Miscellaneous Charges/Enhanced Services Page 3 of 7 CA, NV January 2006 B & C Customers are required to submit for review the text phrases for each service the customer is requesting to bill. The Text Phrase is limited to 12 positions for enhanced services record types (18 records), and 24 positions for miscellaneous charge records (42 records). This information should be forwarded to AT&T Telco Product Management through the Customer's Account Manager. Product Management will review the request and make a determination if the charge is deniable or non-deniable. All Customers are required to obtain written notification from Product Management that the text phrase and supporting documentation has been reviewed and systems updated even when a similar service has been previously reviewed. An example of the detail required is: "Monthly Calling Plan - The end-user customer is charged a fixed monthly fee to belong to a specialized calling plan for traditional toll services. The end user will receive discounted per minute charges for all long distance calls. Customers find out about the service through print advertising and are ordered by calling 800-XXX- XXXX." Types of miscellaneous charges that have been reviewed include: • calling plan fees • set-up or installation fees (one time charge only) • telecommunications equipment charges • pager fees • voice mail fees • certain internet fees It is the responsibility of each Customer subscribing to Miscellaneous Charge processing to keep track of reviewed text phrases. Record Processing 425001 and 010118 records can be intermingled with other record types when submitted to AT&T Telco for billing. No special sequencing or testing is required. The charge phrase field on the EMI record (positions 123 and 146) will appear on the end-user customer bill. 425001 records are used for services that are billed on a fee basis, either recurring or non-recurring. These records should not be used for charges that are incurred per call and based on minutes of use. 010118 records populate the service name in the "to place" section on the end-user customer bill. Billing and Collections Product Binder Section 23 Miscellaneous Charges/Enhanced Services Page 4 of 7 CA, NV January 2006 The 415001 record should be utilized when adjusting charges submitted on a 425001; also the 410118 for 18 record adjustments that will apply a credit amount to a customer’s bill utilizing a phrase that describes the reason for the credit. Edits applicable to these records are noted further in this section. Taxes Taxes on miscellaneous charges/enhanced services records will be based upon the tax status of the end-user account and any additional exemptions identified on Indicator 27 of the EMI records. Implementation Time Frames AT&T Telco schedules the implementation of these services on a monthly basis. Implementation will be based upon the dates provided in the Implementation Schedule included in the "Ordering Services" section of this binder. Please note that phrases must be reviewed by SWBT Product Management prior to these dates. Processing Requirements Miscellaneous Charges The 425001 EMI record will be utilized for billing miscellaneous non-detail charges. The B&C Customer must sign an addendum to AT&T Telco's B&C Contract agreeing to certain terms for use of promotional credit records. This addendum may be obtained through the Customer's Account Manager. The customer must submit a proposed text phrase associated with each unique promotional service credit, according to the 42 record Implementation Schedule included in the “Ordering Services” section of this binder. Please note that phrases must be reviewed by AT&T Telco Product Management prior to these dates. The promotional service credit records will appear in the itemized call section of the end-user's bill. These credits are not to be used in lieu of post billing adjustments. Post billing adjustments are visible to the AT&T Telco service rep and impact the balance due immediately, unlike promotional credits, which do not impact the amount due until the bill date and do not post as an adjustment for the service rep to see. Formatting of EMI Record • EMI 415001 • Type of Credit Indicator = 3 (credit for promotional activity) • Indicator 27 = 9 (tax exempt) • The text phrase (to be associated with the text code) must be submitted to Product Management for approval 30 days prior to implementation. • Text phrase cannot exceed 24 positions. • All promotional credit records must be together in the same pack, and

But we are left with wondering how both the telephone companies as well as their consultants, were so wrong. Let's look at the options: There were three massive errors in judgment: • Mistakes in the costs of rolling out the network • Mistakes in overestimating demand • Mistakes by the research/consulting suppliers Let's walk through each one: • Mistakes in the Costs of Rolling Out the Network: The original cost model for the I-Way was estimated at around $1,200 per household. However, Bell Atlantic stated that the cost of their trials came to $16,000 per line. This includes the cost of the various Info Highway components in the home, described earlier, as well as the cost of the fiber- optic networks. But, that's a difference per line of 1233%. Of course there are caveats. Most importantly, that the trickle of a rollout was only a "test" of advanced services, and with larger volumes of users, the costs would decline. In fact, Bell Atlantic's original plans may have actually called for a great deal less spending than $1,200 a line.
