Alameda County Safe Drug Disposal Ordinance (“Ordinance”) Section 6.53.080 and Alameda County Safe Drug Disposal Regulations (“Regulations”) Section 15 require MED-Project LLC (“MED-Project”) to prepare and submit an annual written report describing activities between January 1, 2016 and December 31, 2016. MED-Project submits this 2016 Alameda County, California MED-Project Annual Report (“Report”) on behalf of its participating producers and in compliance with the Ordinance and Regulations.

The Alameda MED-Project Product Stewardship Plan (“Plan”) was submitted on January 30, 2015 and approved by the Alameda County, Department of Environmental Health (“Alameda County DEH”) on February 25, 2015. MED-Project began implementation of the Product Stewardship Program (“Program”) after the Plan approval date. On February 2, 2016, a revised Ordinance was approved by the Alameda County Board of Supervisors that, among other revisions, added controlled substances and over-the-counter medicines to the definition of a Covered Drug.1 The revised Ordinance became effective on March 3, 2016.2 MED-Project immediately thereafter began the collection of Covered Drugs at take-back events. The call center, website, and materials were updated to comply with the revised Ordinance.

In mid-August 2016, MED-Project submitted a request, which was subsequently approved by Alameda County DEH, to modify kiosk signage in order to help direct residents to disposal options for Covered Drugs that are not accepted in kiosks.

On October 26, 2016, Alameda County DEH approved in part and denied in part a MED-Project request for modification to the approved Plan proposing changes to the transport method for sealed inner liners containing Unwanted Medicine from households. Alameda County DEH approved the transport of sealed inner liners by a common carrier from certain kiosk drop-off sites to the disposal facility as an additional option to the transport method described in the approved Plan.

In keeping with the Plan, MED-Project is implementing the Program in phases. During 2016, priority was given to establishing kiosk drop-off sites throughout Alameda County (“County”), conducting take-back events in alignment with approved Plan goals, and performing outreach to eligible pharmacies and Law Enforcement Agencies (“LEAs”) throughout the County.

Kiosks were activated in pharmacies and LEAs that met all local, state, and federal

Page 3 requirements and that signed agreements with MED-Project prior to kiosk installation and activation. In addition, outreach and educational materials were distributed to all interested and participating sites, including signage and/or brochures. Online media (such as the website), video and audio messaging (such as public service announcements) and the call center continue to be revised to meet the needs of County residents.

The ultimate goal of the Program is to provide County residents with convenient, safe, and accessible methods to dispose of Unwanted Medicine. While operational challenges, including Ordinance revisions and Plan modifications, delayed kiosk installation and activation in 2016, Plan modifications simultaneously led to the stabilization of collection, transport, and disposal procedures. MED-Project anticipates a more stable and steady increase in Collection Sites throughout 2017 as a result of Plan modifications in 2016.

MED-Project continues to communicate with Collection Site Hosts to improve outreach materials, agreements, procedures, and overall Program design.

2. REPORTING ON 2016 GOALS Alameda Health & Safety Code, Chapter 6.53.080 – Report. A.8. and A.9. Alameda County Safe Drug Disposal Regulations, Section 15 – Annual Reports C.ii.

2.1. Collection Goals Medium-term collection goals, as outlined in the approved Plan, include the expansion of kiosk placement to additional eligible LEAs and pharmacies during the second year of Program operations, with a goal of establishing more than 20 kiosk drop-off sites. Program goals set an ongoing target of 12 take-back events per year. MED-Project held 23 take-back events in 2016, 11 of which were conducted to make-up for the single take- back event held in 2015.

Agreements to support take-back events were executed with several LEAs including the Alameda County Sheriff’s Office (“ACSO”), and four (4) police departments (“PDs”) in the cities of Oakland, Pleasanton, San Leandro, and Alameda. In 2016, law enforcement oversight of events was performed by ACSO for 19 of the events and by PDs for four (4) of the events. See Table 1 for more details.

MED-Project plans to conduct 12 or more take-back events in 2017 by collaborating with Alameda County Household Hazardous Waste (“HHW”) drive-through events, and participating at fairs and festivals. For more information about this effort, please see Section 3.

Agreements to host kiosk collection services in 2016 were updated and executed with nine (9) organizations and agencies, including two (2) Law Enforcement Agencies and seven (7) healthcare organizations, consisting of a total of 14 kiosk drop-off sites. Kiosks were subsequently installed in ten (10) sites and became available to the public in late 2016. The four (4) additional kiosk drop-off sites had kiosks scheduled for installation in early 2017 to accommodate for site renovations, facility approvals for installation, or site schedules. See Table 2 for more details.

During 2016, MED-Project submitted a request for a Plan modification to Alameda County DEH in order to modify the method of transport of Unwanted Medicine to the disposal facility via common carrier transport. This modification was approved in part and denied in part by the Alameda County DEH and is now implemented at all active kiosk drop-off sites.

MED-Project intends to increase the number of active kiosk drop-off sites in 2017 by continuing to work with all eligible LEAs and pharmacies that express interest in participating in the Program. For more information about this effort, please see Sections 3 and 5 and Appendix A. The Executive Summary describes challenges MED- Project encountered in meeting its kiosk installation goals for 2016.

2.2. Education and Public Outreach Goals The Plan’s medium-term education and public outreach goals provide that MED-Project may revise or add communication materials based on Plan modifications and that MED-Project will collect feedback to continuously improve operations.

The toll-free number for the call center continues to be available in both English and Spanish for County residents. Messages left on the voicemail are returned within one business day per the approved Plan.

In an effort to increase website visits, MED-Project continued outreach through social media websites, such as Twitter and Facebook. Brochures and materials were distributed to nearby pharmacies, law enforcement agencies, clinics, veterinary facilities, hospitals, and community groups, and other media outlets were contacted to increase outreach prior to events. Website statistics for 2016 can be viewed in Table 3.

Page 6 The MED-Project website (http://www.med-project.org/locations/alameda) has been live for nearly two years, and is periodically updated to ensure alignment with the revised Ordinance and approved Plan. The website now provides contact information County residents, Collection Site Hosts, and producers can use to obtain Program information. In addition, any kiosks that are installed and active are displayed on the website after the kiosk is available to the public.

MED-Project will continue to review website and call center performance and statistics, and revise processes, systems, and agreements as needed to improve the infrastructure, quality, efficiency, and sustainability of the Program, and to ensure alignment with local, state, and federal laws, regulations, and other legal requirements.

Over 4,700 pounds of Unwanted Medicine was collected at the 2016 events and subsequently incinerated. MED-Project collaborated with five (5) Law Enforcement Agencies and held events at 15 different venues. Nine (9) of the events were in collaboration with County Household Hazardous Waste drive-through events, and 14 of the events were conducted at festivals or fairs.

In March 2016, MED-Project submitted a request for exception to reimburse law enforcement for the transportation and disposal of Unwanted Medicine due to an unexpected disposal facility closure, which was granted in part by Alameda County DEH.

The MED-Project safe drug disposal collection system places emphasis on Collection Sites at LEAs and pharmacies to best serve County residents. In December 2016, kiosks were installed and became available to the public at ten (10) locations throughout the County. Kiosk collection agreements continue to be distributed to interested sites and are under review by several agencies and organizations. The location of each site and the status of the agreements are outlined in Tables 4 and 5 below.

Agreements with LEAs that are “pending review” include sites that have not reviewed the documents to date or may not be able to host kiosks. Agreements that are under “review” include sites that have acknowledged receipt and are in the process of reviewing the document. Agreements that are under “final review” include sites that have reviewed the documents and provided information to finalize the agreement.

3.1. Take-Back Events MED-Project is working closely with Alameda County DEH to conduct take-back events in collaboration with up to 11 HHW drive-through events throughout the County in 2017. In addition, MED-Project is exploring several options for hosting take- back events at festivals and fairs throughout the coming year, including:

Take-back event services agreements were updated to reflect changes to the revised Ordinance and will continue to be issued to LEAs that are interested in overseeing take- back events.

3.2. Kiosk Collection Sites MED-Project continues to work closely with LEAs, pharmacies, and hospitals/clinics with on-site pharmacies that are interested in becoming kiosk drop-off sites. Agreements have been distributed to 18 LEA sites and 40 pharmacies or hospitals/clinics with on-site pharmacies for review. See Tables 4 and 5 for more information. MED-Project continues to schedule kiosk installation and activation at eligible sites with signed agreements, with the goal of establishing an aggregate of approximately 100 kiosks by 2018. Pharmacies that are targeted for continued outreach as potential Collection Sites are listed in Appendix A.

Additional kiosk installations are being scheduled during 2017 for Collection Site Hosts that have signed agreements in 2016 with MED-Project. Please see Table 7 for more information.

MED-Project implemented a combination of take-back events and kiosk drop-off sites to support collection efforts in 2016. Policies and procedures for collecting, transporting, and disposing of Unwanted Products, as described in the approved Plan, were followed during the reporting period, except with respect to one circumstance described in

Page 17 Section 10.2.

Take-back events were conducted from March through October with 23 separate events hosted at 15 separate venues. Over 4,700 pounds in total were collected during 2016, with an average of approximately 205 pounds collected per event. Oversight was performed by five (5) Law Enforcement Agencies that had signed agreements with MED-Project.

Kiosk services agreements were signed with a total of nine (9) agencies representing 14 sites, including two (2) LEAs and 12 pharmacies. Kiosk drop-off sites were implemented at LEAs and pharmacies in late 2016 with a total of ten (10) kiosk installations and activations at two (2) LEAs and eight (8) pharmacies. Kiosks were available to the public within one week of installation.

MED-Project continues to capture data from take-back events and kiosk drop-off sites to evaluate performance on an ongoing basis.

MED-Project uses a combination of printed materials, online media outlets, public service announcements, and other methods, such as the call center, to increase awareness of the Program, and to educate County residents about safe and available methods for disposal of Unwanted Medicine. In late 2016 brochures (Figure 6) and posters were distributed to all active Collection Sites (Table 2), to ensure materials an information are available for residents.

A primary objective of outreach efforts in 2016 was to raise awareness of the revised Ordinance and the new Program. Materials will continue to be sent to active Collection Sites, community organizations, agencies, associations, and health systems to increase awareness and recruit additional sites for potential kiosk placement.

The total weight of collected products incinerated at the Covanta Stanislaus disposal facility totaled 175 pounds. This amount reflects Unwanted Medicine destroyed from the March 3, 2016 and March 12, 2016 take-back events. On March 8, 2016, MED-Project informed Alameda County DEH that its designated disposal facility was closed for maintenance and that, as a result, MED-Project had reimbursed ACSO for the transportation and disposal of Unwanted Medicine from the March 3, 2016 take-back event. MED-Project requested approval from Alameda County DEH to use this same arrangement when its designated disposal facility is unavailable to accept Unwanted Medicine.
