Terapi hiperurisemia menggunakan obat sintesis seperti allopurinol memiliki berbagai efek samping. Hasil penelitian ekstrak tumbuhan sebagai antihiperurisemia dengan dosis yang bervariasi telah banyak dilakukan, tetapi sulit untuk mengambil kesimpulan dari banyaknya penelitian tersebut dan perlu dilakukan tinjauan sistematis. Tujuan: Untuk mengetahui ekstrak tumbuhan yang berkhasiat sebagai antihiperurisemia berdasarkan variasi dosis, efektivitas dan efisiensi. Metode: Penelitian ini merupakan tinjauan sistematis kualitatif menggunakan metode Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA), dengan memformulasi pertanyaan penelitian, menetapkan database dan kata kunci, menyeleksi artikel yang relevan berdasarkan kriteria inklusi, mengekstraksi data, dan mensintesis hasil penelitian ekstrak tumbuhan yang memiliki aktivitas antihiperurisemia dalam menurunkan kadar asam urat (KAU) pada tikus dan mencit yang diinduksi dengan berbagai penginduksi. Hasil: Jumlah artikel yang memenuhi kriteria inklusi penelitian diperoleh sebanyak 16 dari 317 artikel yang diakses. Ekstrak dengan nilai penurunan KAU yang dianalisis dari yang tertinggi (dalam %): daun mahkota dewa, 70,75; daun kelor, 65,69; herba suruhan, 63,56; rimpang temu putih, 60,17; buah naga putih, 59,88; daun hijau pucuk merah, 55,04; herba anting-anting, 53,41; rimpang temu lawak, 43,38; daun cocor bebek, 42,46; daun pegagan, 40,00; daun binahong, 37,58; biji alpukat, 34,36; tumbuhan jelatang, 24,15; daun bambu tali 21,83; buah andaliman, 18,55; daun murbei, 12,26. Kesimpulan: Ekstrak etanol (EE) daun mahkota dewa dan EE daun kelor merupakan ekstrak tumbuhan yang efektif berdasarkan efektivitas dan kemudahan pemanenan. Kata kunci: tinjauan sistematis, ekstrak tumbuhan, antihiperurisemia, kadar asam urat viii Universitas Sumatera Utara SYSTEMATIC REVIEW OF ANTIHYPERURICEMIC OF INDONESIAN PLANT EXTRACTS IN RATS AND MICE ABSTRACT Background: Hyperuricemia remains as a global health issue. Synthetic medications like allopurinol, which are used to treat hyperuricemia, have a variety of negative effects. Plant extracts as antihyperuricemia have been studied at various doses, but it is difficult to draw conclusions from so many trials, thus a systematic review is needed. Objectives: The purpose of this study was to investigate which plant extracts are effective as antihyperuricemia agents based on dosage variations, effectiveness and efficiency. Method: This research is a qualitative systematic review using the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) method, by formulating research questions, setting da tabases and keywords, selecting relevant articles based on inclusion criteria, extracting data, synthesizing research results from plant extracts which has antihyperuricemic activity to decrease uric acid levels (UAL) in rats and mice induced with various inducers. Results: The number of papers that satisfied the inclusion criteria was obtained as many as 16 out of 317 articles accessed. Extracts with decreasing UAL values were examined from the highest (in %): Phaleria leaves, 70.75; Moringa leaves, 65.69; Peperomia herbs, 63.56; zedoaria rhizome, 60.17; white dragon fruit, 59.88; green leaves of oleana, 55.04; acalypha herbs, 53.41; zanthorrhiza herbs, 43.38; pinnatum leaves, 42.46; centella leaves, 40.00; cordifolia leaves, 37.58; avocado seeds, 34.36; urtica plants, 24.15; gigantochloa leaves, 21.83; zanthoxylum fruits, 18.55; morus leaves, 12.26. Conclusion: Ethanol Extract (EE) of Phaleria leaves and EE of Moringa leaves are effective plant extracts based on their effectiveness and ease of harvesting.     CHAPTER II LITERATURE REVIEW 2.1. The Immune System The Immune system is a form of body defense against the antigens such as infectious agents and malignancies. This system recognizes, processes, and then eradicates antigens. Based on the specification and memory, the immune system defense is classified into two arms, the innate and adaptive immunity. Both the systems share the coordination and develop a well-adjusted mechanism to eradicate the pathogens.1 Innate immunity is known as non-specific immunity. It is defined inasmuch of its ability to eradicate antigens at first line without previously exposed to this specific antigen. This type of immune system is consisted of epithelial barriers, molecules (complement system, C-reactive proteins) and cells of innate immunity such as NK cell, neutrophils, basophils, eosinophils, and monocytes-macrophages. Epithelial barriers are mucosal lining or skin that cover the body and protect with their intact lining. They are the very first barriers that need to be passed by the antigens before invading the host. Complement system consists of soluble proteins that interact one to another resulting in microbial cell wall lysis.3 C-reactive proteins (CRPs) are the same soluble proteins, which are secreted by the liver in response to macrophages cytokines interact with the cell 8 9 walls of several pathogens such as C-polysaccharides of Pneumococcus (Streptococcus pneumoniae).15 Phagocytes of innate immunity are the first cellular responses that encounter the pathogens. The cells are equipped with the pattern recognition receptors (PRRs), phagocytic capability, intracellular killing and some of them are able to present the antigens to the other immune cells, or also known as Antigen- Presenting cells (APCs).

This led to studies on its phytochemicals and their functions, but very few were focused on the structures of flower and fruits. Hence, this study aims at determining the development of the structure of Phaleria macrocarpa’s flowers and fruits including the seeds. Tagging methods and observation of the anatomy and morphology of the flowers and fruits were carried out at Taman Herba Universiti Pendidikan Sultan Idris. Phaleria macrocarpa’s flower is tubular shaped, whitish, fragrant, perfect flower with four sepals, four or five petals, 8 stamens 4 (4[5]) 8 1 and a carpel with floral formula K br C br A G . The inflorescences are umbel type, borne from all over the tree, down the trunk and the branch axils. The development of the fruit took 80 – 90 days after flowering. The colour of matured fruit is green and turns to brightly red or maroon when it ripens. The fruit is round or elliptical in shape, 3 – 5 cm in diameter of matured fruit, simple fleshy fruit in drupe form with one or two seeds.

This research used survey method through in-situ visit and specimen collection. Monitoring and visits were conducted based on prediction of flowering period and fruit-bearing season. Eighty plant families of flowering plants were found in the mount Nglanggeran. Based on the phylogenetic arrangement of flowering plants it was found that all the main groups (clades) of flowering plants were found at this location. Medicinal plants have curative properties due to the presence of various complex chemical substances of different composition, which are found as secondary plant metabolites in one or more parts of these plants. Plants have been used for thousands of years to flavour and conserve food, to treat health disorders and to prevent diseases including epidemics. The knowledge of their healing properties has been transmitted over the centuries within and among human communities. Active compounds produced during secondary vegetal metabolism are usually responsible for the biological properties of some plant species used throughout the globe for various purposes, including treatment of infectious diseases. In recent years, the utilization of certain medicinal plants as therapeutic agents has drastically increased. Phaleria macrocarpa (Scheff.) Boerl is frequently used in traditional medicine. Phaleria macrocarpa, commonly known as Mahkota dewa is a medicinal plant that is indigenous to Indonesia and Malaysia. Extracts of P. macrocarpa have been used since years in traditional medicine that is evaluated scientifically as well.
