The vast majority of cases occur in intact male dogs that are middle-aged or older. It has been hypothesized that anatomic factors, hormonal imbalances, damage to the nerves of the pelvic diaphragm, and straining due to prostate gland enlargement may contribute to the development of a perineal hernia. Signs and diagnosis The first signs of a perineal hernia include straining during bowel movements, constipation, and swelling around the anal region. Subsequently, the pet may have a loss of appetite. Straining to urinate may be seen if the bladder has become displaced into the hernia (see illustration right). If the small intestine gets trapped in the hernial sac, vomiting and depression may be seen if the bowel’s blood supply is compromised. The diagnosis of a perineal hernia is made by digital rectal palpation performed by a veterinarian. In 22 dogs, herniorrhaphy was performed by standard perineal herniorrhaphy and in 18 dogs by internal obturator muscle transposition (six bilateral herniorrhaphies). Castration was performed in 13 (59%) dogs operated by standard perineal herniorrhaphy and in 14 (77%) dogs operated by transposition of the internal obturator muscle. Rectal disease was preoperatively observed in 22 (46%) cases. Recurrence was recorded in six (27%) dogs operated by standard perineal herniorrhaphy and in two (11%) dogs operated by internal obturator muscle transposition. Postoperative complications developed in 30 (65%) cases. The most common complications were wound complications (swelling, seroma, dehiscence and hematoma), lameness and tenesmus. Patient evaluation Evaluation of the patient with upper airway disorders should include a thorough history, physical examination, radiographic examination, and pharyngoscopic, laryngoscopic and tracheoscopic evaluation in the anesthetized patient. These hernias begin to bulge when they fill with fat, abdominal tissue, the urinary bladder, or part of the rectum slides into the pocket. Causes for PAH: There are a number of possible causes for perineal hernias. It is believed that intact male dogs, due to their often enlarged prostate, exert more pressure when urinating and defecating and the tissues around the rectum eventually stretch, weaken and then tear, resulting in a perineal hernia. Some veterinarians also speculate that hormonal differences in intact male dogs predispose to perineal hernias as such hernias are far less common in castrated dogs. Predisposed breeds and clinical signs associated with Perineal hernia: Perineal hernias are most common in middle aged and geriatric intact male dogs, and are rarely seen in cats. The breeds that are most commonly affected are Boston Terriers, Boxers, Welsh Corgis, Pekingese, and Dachshunds. More than 30% of perineal hernias occur on both sides of the rectum. The most common symptoms of a perineal hernia are swelling beside the rectum, constipation, and straining to defecate.

    Perineal Hernia What is the Perineum? The perineum is the part of the body around the anus and genitals. There are strong muscles that support the contents of the pelvis. Part of their job is to support the rectum during straining to pass faeces. What is a Perineal Hernia and what causes it? A perineal hernia is the displacement of pelvic contents between perineal muscles. In particular, there is loss of support of the rectum, which tends to bulge into the hernia. Faeces tend to accumulate in this dilated portion of rectum leading to the typical clinical signs seen (see below). Perineal hernia occurs mostly in older, uncastrated, male dogs. It is rare in females or in cats of either sex. Some breeds are over-represented: Pekingese, Boston Terriers, Corgis, Boxers, Poodles, Bouviers des Flandres and Old English Sheepdogs. The cause is multifactorial. Affected dogs usually have atrophy of the pelvic diaphragm muscles, which become too small to function effectively. The condition has been related to male hormones and so castration is recommended in affected dogs to reduce recurrence after hernia surgery. Perineal hernia may also develop due to persistent straining to pass urine or faeces (e.g. prostatic disease, bladder stones, intestinal disease) or from chronic coughing. These dogs will require further investigation and treatment of the underlying cause of straining, as recurrence of hernia is more likely in dogs that strain after hernia surgery. What are the clinical signs? • Common: Swelling in the perineum – usually directly to the side +/- beneath the anus. This may reduce in size after passing faeces.
