• Year 6 pupils to benefit from additional support with transition. • Adversity factors, e.g. diagnosis of ADHD, Autism, significant behavioural difficulties, family issues affecting engagement, attendance, attainment and/or behaviour. • Group of families to meet for a consecutive 10 week period, which involves the school, the family and community activity. • Meetings to be held at the the young person will be attending. • Possible second group in the year for year 7 pupils who have been identified as struggling with transition. • Follow up with the families 3-6 months after the 10 week programme to identify what changes have been made and sustained beyond the group work.
