West Liberty University’s Dental Hygiene Clinic has been contracted by the CDCA to administer the ADEX manikin clinical exam on Tuesday, June 15, 2021. This site information sheet will provide information to alleviate some of the additional stress encountered by testing at an unfamiliar site. A $25 site fee covers the costs of use of the facility and air/water tips, saliva ejectors, and other incidentals that may be needed for the exam. This fee, made payable to , can be paid upon arrival to your exam. Clinic Information and Location

The Dental Hygiene Clinic is located on the main floor of Campbell Hall on the campus of West Liberty University. West Liberty University is located about 10 minutes past in the winding hills of Wheeling, . You will enter the clinic directly by entering the far right door on the main floor of the building. Signs outside of the building designate the entrance to the dental hygiene clinic. You will park across the street in front of Campbell Hall. A parking pass will be given to you once you check in to our front desk. The clinic can be found by using the following address: West Liberty University Campbell Hall 208 University Dr. West Liberty, WV 26074 GPS directions have a tendency to take you through more backroads than you may want to travel. If you do not prefer this type of travel, then the preferred route to campus is by way of I- 70 to Exit 2A following to Route 88 North. You will pass the beautiful Oglebay Park and will continue another 10 minutes or so until you arrive at West Liberty’s campus. Campbell Hall can most easily be accessed by taking the third entrance to West Liberty’s campus on your right. You will see a large dorm on your left and an older stately looking building on your right. Proceed on the road between the two buildings and the newly built Campbell Hall will be in front of you.

Cavitron Units

Should you wish to use the cavitron unit for your exam, you will be responsible for supplying your own tips. While we likely will have enough handles to accommodate the exam, it is highly recommended that if you have access to a handle, you supply your own to avoid any shortages or malfunctioning of the handle. The cavitron units are built into the dental units, manufactured by Dentsply, and require the 30K inserts. The inserts used most frequently in our clinic are the 30K FSI-SLI-1000 and the 30K BELLISS FSI-SLI 10S insert by Dentsply. The 30K HuFreidy inserts will work as well. The required handle is the Steri-mate 30K HDPC by Dentsply. West Liberty University will not be responsible for the exam results of any candidate who experienced cavitron malfunctioning or the candidate provided improper cavitron inserts and/or handles.

Candidates must supply the following: All instruments Magnifying loupes/lights or safety glasses (whichever the candidate uses) Cavitron inserts Cavitron handle (recommended) Face Shields All clinic attire

COVID-19 Policies West Liberty University requires masks while in all campus buildings. Candidates will need to provide their own masks and keep them in place during the entire time they are in our facility.
