Report of Funded Projects Past 2001- Training Grant in Epidemiology and Prevention of Breast Cancer (PI: Colditz) 2004 Department of Defense Training grant DAMD17-00-1-0165 Predoctoral Fellow This grant provided predoctoral training in cancer epidemiology with a primary focus on breast cancer for my dissertation. 2004- Nutritional Epidemiology of Cancer (PI: Stampfer) 2007 NCI Training Grant R25 CA098566 Postdoctoral Fellow This grant provided postdoctoral training with a focus on the role of diet and nutrition in the etiology of breast cancer. With this funding, I published several papers and was subsequently funded to pursue the role of plasma carotenoids in breast cancer risk. 2007- Predictors of Plasma Carotenoids Levels 2008 NCI Career Development Award Breast Cancer Specialized Programs of Research Excellence (SPORE) P01 CA089393 Principal Investigator ($24,429) The aim of this grant was to develop a statistical model to predict plasma carotenoid levels using dietary and lifestyle information and validate the model in an independent data set. 7 2007- Plasma Carotenoids and Breast Cancer Risk – Pooled Analysis of Prospective Studies 2009 NCI R03 CA128073 Principal Investigator ($100,000) In this study, I utilized existing data from six prospective case-control studies of plasma carotenoids and the risk of breast cancer. The following studies are included in this proposal: Columbia, Missouri; Umeå, Sweden; New York University Women’s Health Study; Washington County, Maryland; Nurses’ Health Study (NHS); and Women’s Health Study. These studies represented >80% of the world’s published literature on plasma carotenoids and breast cancer risk; the combined analysis included 3,055 cases and 3,956 controls. 2007- Premenopausal Hormone Levels and Risk of Breast Cancer (PI: Hankinson) 2014 NCI R01 CA67262 Co-Investigator Served as Project Director of this grant and as such, led analyses related to the specific aims (e.g., premenopausal sex steroid hormones, urinary estrogen metabolites, plasma vitamin D, plasma insulin and c-peptide) and mentored doctoral students and post- doctoral fellows working on the grant. 2008- Urinary Estrogen Metabolites and Breast Cancer Etiology in Premenopausal Women 2009 Brigham and Women’s Hospital Fellowship, Eleanor and Miles Shore Fellowship for Scholars in Medicine Principal Investigator ($30,000) The goal of this project was to further our understanding of the role of estrogen metabolites in breast cancer etiology in premenopausal women. My aims included assessing the reproducibility of 15 estrogens and estrogen metabolites over time, examining the association between these metabolites and breast cancer risk, and investigating the correlations between the metabolites and breast cancer risk factors. 2009- Plasma marker of oxidative stress and risk of breast cancer: a prospective study 2013 Susan G. Komen Foundation KG0906845 Principal Investigator ($470,332) Utilizing a novel biomarker for global oxidative stress, we will investigate the role oxidative stress plays in the etiology of breast cancer among premenopausal women in the NHS and the NHSII. 2009- Oxidative Stress, Antioxidants and Risk of Breast Cancer: A Prospective Analysis 2014 NCI R01 CA131218 Principal Investigator ($1,338,113) The goal of this grant is to further our understanding of the role of oxidative stress in the etiology of breast cancer. Specifically, we will assess a novel plasma marker of oxidative stress, plasma carotenoid levels, and genetic variation in antioxidant-related genes with respect to breast cancer risk in the NHS. 2009- Risk Factors for Breast Cancer in Younger Nurses (PI: Willett) 2014 NCI R01 CA050385 Site Principal Investigator ($2,953,310) The goal of this grant is to continue following the 116,430 women enrolled in 1989 in the NHSII. We will assess risk of breast cancer in premenopausal and postmenopausal women through an evaluation of dietary and lifestyle factors during adolescence and early adult life, biomarkers that reflect dietary factors, mammographic density, and a new genetic risk score. As Project Director, I oversee the cohort infrastructure and the

8 scientific aims, as well as mentor doctoral students and post-doctoral fellows working on the grant. 2004- Biochemical Markers in the Nurses' Health Study (PI: Hankinson) 2014 NCI R01 CA49449 Co-investigator This project seeks to identify and validate biomarkers – particularly hormonal markers – that predict risk of invasive breast cancer in postmenopausal women. Using a prospective nested case-control design, we plan to analyze blood samples collected from the 32,826 participants in the NHS who provided a blood sample in 1989-90 and, for 18,743 of these women, a second sample in 1999-2000. As a co-investigator, I have led analyses related to specific aims including plasma sex steroid hormones, plasma vitamin D, and plasma estrogen metabolites. 2013- Breast Cancer Risk Factors Among Women Aged 75 and Older (PI: Schonberg) 2016 National Institute of Aging (NIA) R01 AG041869 Site Principal Investigator ($171,591) In this application we will identify risk factors for late-life breast cancer and use these factors to develop and validate a clinical prediction tool for women aged 75 and older to be used by older women and their doctors to make more informed decisions about breast cancer screening, thereby improving both their quality of care and quality of life. 2013- Biomarkers and Breast Cancer Risk Prediction in Younger Women (PI: Zeleniuch- 2016 Jacquotte) NCI R01 CA178949 Site Principal Investigator ($223,635) The goal of this proposal is to improve the performance of the Gail breast cancer risk prediction model 2 for women 35-49 years of age, by the addition of biomarkers (testosterone or free testosterone, and/or Müllerian Inhibiting Substance (MIS)). 2012- Circulating Fatty Acids and Breast Cancer Risk: A Prospective Study 2016 NCI R01 CA160246 Principal Investigator ($952,456) The goal of this grant is to further our understanding of the role of specific fatty acids, from both diet and endogenous production, in breast cancer. Specifically, we will measure 45 individual fatty acids in archived red blood cell samples in a nested case- control study within the NHSII. 2013- Life Course Cancer Epidemiology Cohort in Women (PI: Willett) 2018 NCI UM1 CA176726 Site Principal Investigator ($4,323,783) The goal is to continue the follow-up of the NHSII, a cohort of 116,430 women with repeated, cycle timed biomarkers and measures of diet, physical activity, and other exposures over several decades. Cohort follow-up is reaching the most informative phase and is a unique resource for scientific aims that integrate diet, other lifestyle variables, biomarkers, genetic predisposition and mediating variables in relation to cancer. As co-PI of the cohort, I oversee the infrastructure and the scientific aims, as well as mentor doctoral students and postdoctoral fellows working on the grant.

Current 2015- Risk Factors for Breast Cancer in Younger Nurses (MPI: Willett, Eliassen) 2020 NCI R01 CA050385 Multiple Principal Investigator ($2,872,511) 9 The goal of this grant is to examine plasma metabolites in relation to breast cancer risk, assess the interrelation of diet and metabolomics in breast cancer etiology, and explore the role of potentially modifiable risk factors in activation of the PI3K pathway. The aims will be conducted within the Nurses’ Health Study cohort of 116,430 women followed since 1989. 2015- Dietary and Hormonal Determinants of Cancer in Women (Stampfer, Tamimi) 2020 NCI P01CA87969 Co-Investigator Our long-term objective is to identify novel hormonal, dietary and genetic determinants of breast, colorectal, and ovarian cancer risk in women, focusing on prevention and survival. We will leverage questionnaire data, blood and urine samples, and tumor tissue, coupled with long-term (up to 36 years) follow-up. 2015- Adverse health effects of shift work (Schernhammer) 2019 NIOSH R01 OH009803 Co-Investigator Our study seeks to further our understanding of how exposure in utero to shift work may increase risk for chronic diseases later in life. We will utilize our unique data to study the effects of shift work, particularly during pregnancy, on the health of women’s children. 2016- Cancer Center Support Grant (Glimcher) 2021 NCI P30CA006516 Co-Investigator The primary goal of the Center is to promote collaborative interactions that will lead to new approaches to cancer prevention, diagnosis and treatment. 2017- Endogenous hormones and postmenopausal breast cancer: Etiologic insights and 2022 improving risk prediction (PI: Hankinson) NCI R01 CA207369 Site Principal Investigator ($998,519) We propose to continue our research group’s work to identify and validate hormonal markers that predict risk of invasive breast cancer in postmenopausal women. Specifically, we will (1) evaluate several new hormonal markers relevant to the estrogen and insulin pathways, (2) incorporate tumor heterogeneity into these analyses, (3) extend and validate the addition of these biomarkers to current breast cancer risk prediction models. 2018- Life Course Cancer Epidemiology Cohort in Women (MPI: Willett/Eliassen) 2023 NCI U01 CA176726 MPI ($6,731,569) The goal is to continue the follow-up of the NHSII, a cohort of 116,430 women with repeated, cycle timed biomarkers and measures of diet, physical activity, and other exposures over several decades. Narrative Report My research focuses on the associations between lifestyle factors, biomarkers of lifestyle and hormones, and breast cancer risk. I have studied ways women may alter their lifestyle to reduce breast cancer risk, using data from the NHS and NHSII cohorts. I have obtained funding from the NIH and Komen Foundation as an independent investigator, with a focus on carotenoids, oxidative stress, metabolomics, and breast cancer risk. My main administrative responsibilities include serving as co-PI of the NHSII cohort and as Director of the BWH/Harvard Cohorts Biorepository. The remainder of my time is spent teaching and mentoring.

In my study of potentially preventive lifestyle factors, I showed that weight loss and increased physical activity after menopause reduced breast cancer risk. As chair of an international collaborative group, I led a pooled analysis of 8 cohort studies and showed that higher blood levels of carotenoids, prominent in 33 fruits and vegetables, significantly reduced breast cancer risk. As a result, I obtained an NIH R01 to continue studying carotenoids and explore the role of oxidative stress in breast cancer etiology. In an analysis of a biomarker for oxidative stress, we showed oxidative stress may not be an important factor in the relation between carotenoids and breast cancer risk. However, in a more in-depth examination of carotenoids and breast cancer risk, I observed a significantly lower risk of lethal/recurrent breast cancer with higher carotenoid levels. Continuing the examination of biomarkers that reflect both diet and endogenous synthesis and breast cancer risk, I am investigating the role of circulating fatty acids and breast cancer risk by tumor expression of inflammatory and immune markers, and am co-PI of the main scientific R01 of the NHSII cohort, in which we are investigating metabolomics and breast cancer risk.

I also have expanded our understanding of the role of hormones in breast cancer etiology, and the intersection between hormones and lifestyle factors. I led the first extensive analysis of urinary estrogen metabolites with risk of breast cancer in premenopausal women, finding unexpected inverse associations. Building upon these findings, I have overseen several analyses to understand how lifestyle factors may impact patterns of estrogen metabolism.

As the co-PI of NHSII, I have multiple leadership responsibilities. Locally, I serve as a senior scientific resource to others wishing to collaborate, help determine the scientific direction of the study, and lead students in their analyses of lifestyle factors and biomarkers in relation to breast cancer risk. I also have represented the cohort nationally and internationally. My reputation in the breast cancer research community has led to invitations to serve on several grant study sections, including the DoD, and to give talks at NCI seminars and workshops. Our infrastructure grant renewal application (U01 CA176726), on which Dr. Willett and I serve as multi-PIs, was reviewed in July 2017 and received a perfect priority score of 10 and is currently funded.

I am active in teaching and mentoring at the Harvard Chan School, including as the primary instructor for Epi 246: Applied Biomarkers in Cancer Epidemiology. I mentored two masters and two doctoral students in Epidemiology (Harvard Chan School) and three post-doctoral fellows as they conducted epidemiologic research. I currently mentor one masters student and one doctoral student in Epidemiology and two postdoctoral fellows. I also serve on oral exam and thesis committees for eight (seven past, one current) doctoral students.

Through contributions to the framework of the NHS/NHSII cohorts, directing the BWH/Harvard Cohorts Biorepository, applying the data obtained to study lifestyle factors and breast cancer etiology, collaborating with others in the field, and mentoring younger researchers, I intend to continue making meaningful contributions to the field of breast cancer epidemiology.
