Job title: Consultant Psychiatrist (Acute Inpatient)

Job purpose

You will be required to work with the multidisciplinary teams offering a recovery and community orientated treatment model to adults with a range of mental health needs. The successful applicant will work closely with other Consultant Psychiatrists in the recovery team and other teams.

The principle aim of the post is to provide medical leadership to the recovery team. Such medical leadership is pivotal to the success of the teams.

The post holder is expected to have or be willing to undertake the training required to gain Section 12(2) approval and approved clinician status under the Mental Health Act 2007 and to undertake further training to maintain AC status as required

On-call for the City of is currently shared between General Adult Psychiatry and Old Age Psychiatry colleagues, and comprises approximately one night on call per month and one weekend in three months.

The Remuneration for on-call is 1% of basic salary. This is based on the frequency of on-call being low and the category B.

The on-call duty is always well supported by trainees and Specialist Registrars. The appointee will be required to cover for colleagues during periods of absence.

General requirements

Applicants should have a higher qualification in psychiatry (Membership of the Royal College of Psychiatrists or equivalent) and be included on or be eligible for inclusion on the GMC’s Specialist Register. Specialty Registrars who are within 6 months of attainment of CCST at the time of interview are eligible to apply. The appointee will need to be both S12 approved and an Approved Clinician to fulfill contractual duties. If the applicant was not approved at the time of the interview, they must be fully eligible for approval.

The successful applicant will be based at the Michael Carlisle Centre which is located in the South West of the City of Sheffield. Burbage ward is a male single sex ward with 16 beds for people with mental health problems. The post holder will provide consultant medical input to the care of the people admitted for mental health problems.

The post holder will have office accommodation, with IT support for e-mail, intranet and internet, and secretarial support there.

Principal duties

• Provide medical leadership to the Burbage ward teams

• Attendance at team and governance meetings • Provision of medical/psychiatric assessments to inform care planning and risk assessment

• Formulation and delivery of appropriate pharmacological and. psychological interventions • Assessment and management of people subject to the Mental Health Act

• Assessment and management of people subject to the Mental Capacity Act • Completion of administrative tasks related to clinical responsibilities • Liaison with other consultants and other colleagues in CMHTs and other inpatient wards • Attendance at CPA and discharge meetings

• Participation in the city wide out of hours on call rota

• Clinical and educational supervision of junior medical staff • Teaching of undergraduate medical students

There are approximately 170 admissions per annum to the ward (mental health beds) with approximately three quarters being detained under the Mental Health Act. In a one year period there were 74 tribunals/managers hearings regarding Burbage patients.

The appointee will be expected to undertake the ordinary administration duties of a Consultant post including participation in professional audit, business planning and related activities. A contribution to the management and strategic direction of the service is seen as essential. There are opportunities for professional development in medical management.

All Consultants are automatically members of the Medical Staff Committee, and each specialty is represented by the Service and Clinical Director on the Operational Management Group.

Service development

Medical staff are involved in medical and clinical audit and the appointee will be expected to take an active part at team, directorate and Trust level.

There are excellent library facilities at the for those clinical staff holding honorary contracts with the University, the Northern General and the Royal Hallamshire hospital, with computer research and journal acquisition services. In addition the Trust's Directorate of Research and Development is able to assist in computer literature search.

Commissioned audit projects will receive excellent support from the department of clinical effectiveness throughout the audit cycle.

Job description review

This is not an exhaustive list of duties and responsibilities and in discussion with the manager the postholder may be required to undertake other duties which fall within the Banding of the post. The job description will be reviewed regularly in the light of changing service requirements and any such changes will be discussed with the post holder.

The post holder should be comfortable in the use of computer technology, have basic keyboard skills and have the ability to use email.

The Sheffield Health & Social Care NHS Foundation Trust, established in July 2008, previously known as Sheffield Care Trust, was one of the first care trusts in the country and the first in South Yorkshire.

Services for people with psychosis are well developed through the trust with an established Early Intervention team and Recovery teams.

The acute care pathway for service users, with the full range of acute mental health problems, includes CMHTs, crisis and home treatment team, inpatient wards and an intensive care service.

Gulu Sheffield Health Partnership

In 2012 the Sheffield Health and Social Care NHS Foundation Trust, together with Gulu Regional Referral Hospital and the Gulu Branch of Mental Health Uganda formed the “Gulu Sheffield Mental Health Partnership”. The Partnership works with the Ministry of Health in Uganda and the Uganda UK Health Alliance to ensure that together we work within Ugandan priorities. It also works with Health Education England and other NHS Trusts operating in Uganda.

As a result of the recent conflicts in Gulu area, Ugandan research now indicates high levels of anxiety, depression, PTSD, Epilepsy ( considered a mental illness in Uganda) and trauma associated with reintegrating abducted people back into the communities in which they may have caused harm or been harmed as children.

Staff working in SHSC have access to a unique opportunity to travel to Uganda with the partnership and work with Mental Health teams in Africa.

Several members of staff from all grades including, Consultants, junior doctors, nurses, support workers and trainers have already taken this opportunity and travelled to Gulu. SHSC are very proud to maintain these links and 23 staff from Gulu mental health services have travelled to Sheffield to gain experience in the UK.

This was supported through the Commonwealth Fellowship Program, funded through the British Council and managed by the Commonwealth Scholarship Commission.

The post holder would be supported and welcomed to seek this opportunity if appropriate and if they so wished.

Education & Teaching The Trust has a strong focus on the delivery of a high standard of medical education. It is the lead employer for the Regional South Yorkshire Psychiatry training rotation. Therefore the Medical Education Department is responsible for the co- ordination of training, core and higher rotations, and education and welfare of doctors in training. There are strong links with the local Training Programme Directors and Head of School of Psychiatry. This department is well staffed to support not only postgraduates but now has a well- established undergraduate department with consultants and support staff having dedicated roles in the delivery of education. The post holder will be encouraged and supported in taking advantage of any roles available in medical education. The appointee will provide teaching and supervision to nursing staff and medical students, as well as contribute to seminar and lecture teaching for the Psychiatry Training Course. These sessions will be on an ad- hoc basis. The appointee will be expected to participate actively, and supervise junior staff, in case presentations and journal clubs, as well as supporting their research interests. The Trust has very close ties with the University of Sheffield School of Health and Related Research (ScHARR). Consultants will be offered the opportunity to lecture within the Local University structure. Several consultants within the Trust have Honorary Senior Lecturer status due to their work in teaching. The appointees will provide teaching and supervision to junior medical staff, nursing staff and medical students, as well as contribute to seminar and lecture teaching for the Psychiatry Training Course. These sessions will be on an ad-hoc basis. The appointee will be expected to participate actively, and supervise junior staff, in case presentations and journal clubs, as well as supporting their research interests. It is anticipated that the appointee will be the named supervisor for one trainee.

Continuing Professional Development

Consultants are expected to be registered for Continuing Professional Development and will need to demonstrate good standing and ongoing development at an annual performance review (appraisal).

The Trust encourages continued professional development and to this end applications for training and study leave are generously considered in terms of both time and financial support (study leave of 30 days in a 3 year period with a nominal budget of £1500 per annum). The Trust has its own in-house leadership training programs; members of staff are encouraged to avail themselves of this opportunity.

Peer groups are well established in the Trust and the post holder will join an appropriate group.

Job Plan The post holder will negotiate a job plan with the Clinical Director. The duties are subject to review on at least an annual basis. If alterations to the described duties are required, these will be agreed between the appointee and the manager. SHSC has a robust electronic job planning system in place with a focus on negotiating a job plan that ensures a good work life balance whilst incorporating your personal interests and ambitions where possible

Appraisal and Revalidation In line with GMC requirements consultants have an annual appraisal. Appraisal is conducted by the lead consultant or another senior doctor, trained and approved within the Trust as a medical appraiser. Consultants are expected to take an active part in these reviews and are given time and assistance to prepare the necessary paperwork. The Revalidation Team focus on providing the essential supporting information to free up time for you to focus on reflection and the consideration of your own personal development Old Age Psychiatry Services The service was set up in Sheffield in 1978, with the appointment of two Consultants, one for the north of the city and one for the south. It now provides a service to approximately 86,000 people over the age of 65 and dementia sufferers of any age with nine consultants. Five Consultants work with community mental health teams. Two consultants are responsible for old age liaison psychiatry, two consultants work with the Memory Service, one with the Older Adult Home Treatment Team, one consultant is responsible for dementia in- patients and one is responsible for functional in-patients. There are active Community Mental Health Teams and close collaboration with colleagues in Social Services, Primary Care and the Private sector.

Adult Psychiatry Services Adult Mental Health Services in Sheffield are now integrated with Social Services. The single service includes all specialist mental health services and has a single management structure. Community and inpatient services are organised on a locality basis; these localities are now congruent with the Primary Care Groups.
