Exercises are supplemented with other activities, for example, “NO FALLS” aerobic/flexibility focus.

Falls prevention is a major hospital priority throughout the state and staff are utilising procedures developed to lessen the possibility of falls.

There are many contributory reasons as to why falls occur in hospitals. Acute medical treatment, weakened medical condition due to illness or unfamiliarity with surrounds may leave a patient more susceptible to falls. Patient falls are monitored and reviewed and despite our efforts to reduce falls this year the number of patient falls or slips was 23 compared to 18 last year. This represents 2.2% of all individual patients and residents and 0.54% of patient/resident days. However, any fall is recognised as a danger and staff are endeavouring to lessen their incidence.

Strategies implemented to reduce risk of falling have included:

• Use of a risk screening tool on admission, which takes into account a patient or resident’s physical disabilities, the possible impact of medications and the hospital environment.

• Patients are encouraged to seek assistance before getting out of bed.

• Use of specialised equipment such as walking frames and wheelie walkers is encouraged.

• Staff education to increase awareness of falls risk and actions to minimise risk.

Timboon & District Healthcare Service - 17 Falls assessment has been conducted at senior citizens Measures introduced for the care management of people and adult planned activity groups and the Service is at risk include: participating in a regional falls prevention program providing education and promoting activities to prevent • Appropriate documentation of skin conditions within falls within the community. the patient’s care plan upon admission • Following of care procedures such as the use of aids PRESSURE ULCER POINT PREVALENCE to prevent or minimise pressure wounds Pressure ulcers are internationally recognised as a • A clinical team approach seeking additional advice patient safety problem and this year Timboon and District such as continence management or nutrition Healthcare Service participated in the Victorian Quality Council second stage Pressure Ulcer Point Prevalence • Monitoring of any pressure wound occurrence Survey (PUPPS 2). Both pressure ulcers were rated stage 2 on a severity scale of 1 –4. FOOD SAFETY STANDARDS Prevention of the development of pressure wound sores for patients at risk is a focus of our nursing care. Food Safety is governed by the Food Safety Act, which requires adherence to a food safety plan. The food safety plan covers the safe handling, preparation, cooking, food storage, reheating of food and the personal hygiene of staff and cleanliness of equipment and working surfaces. Food temperatures are regularly monitored throughout the daily routine.

An annual audit of procedures is conducted by a Food Safety Auditor registered with the Department of Human Services and spot checks are undertaken from time to time by the Shire’s Environmental Food Officer. As well as our main kitchen, food preparation, cooking and handling procedures are monitored at our two adult day activity centres. The Food Safety Auditor has reported high compliance with standards.

In addition to compliance with standards our quality of care requires that meals are appetising, nutritious and meet patients’ dietary requirements.
