  • MASS CASUALTY TRAUMA TRIAGE PARADIGMS and PITFALLS July 2019
1 Mass Casualty Trauma Triage - Paradigms and Pitfalls EXECUTIVE SUMMARY Emergency medical services (EMS) providers arrive on the scene of a mass casualty incident (MCI) and implement triage, moving green patients to a single area and grouping red and yellow patients using triage tape or tags. Patients are then transported to local hospitals according to their priority group. Tagged patients arrive at the hospital and are assessed and treated according to their priority. Though this triage process may not exactly describe your agency’s system, this traditional approach to MCIs is the model that has been used to train American EMS As a nation, we’ve got a lot providers for decades. Unfortunately—especially in of trailers with backboards mass violence incidents involving patients with time- and colored tape out there critical injuries and ongoing threats to responders and patients—this model may not be feasible and may result and that’s not what the focus in mis-triage and avoidable, outcome-altering delays of mass casualty response is in care. Further, many hospitals have not trained or about anymore. exercised triage or re-triage of exceedingly large numbers of patients, nor practiced a formalized secondary triage Dr. Edward Racht process that prioritizes patients for operative intervention American Medical Response or transfer to other facilities. The focus of this paper is to alert EMS medical directors and EMS systems planners and hospital emergency planners to key differences between “conventional” MCIs and mass violence events when: • the scene is dynamic, • the number of patients far exceeds usual resources; and • usual triage and treatment paradigms may fail.

I have tried to show when we are in another LOCATION by using BOLD CAPITALS. I have used DUAL DIALOGUE as people talk over each other on a SINGLE SHOT. From the beginning - for the next hour - everything about ONE is to do with the inexorability of things in life and A+E. Time cannot be reversed, events cannot be interrupted. Like life. FIRST FLOOR OF SMALL HOUSE... Absolute stillness. At first we can’t work out what we are looking at. Just the green numbers of a digital alarm clock - 14.21. A horizontal slither of light, smudging with smoke. Somewhere close a baby gurgles and then coughs. Things are starting to get clearer. Then we hear JEZ shouting and coughing. JEZ(O.O.V) Hello? You in here... Hello? * You in here? * Hello! * She’s here! She’s ... * I got you. It’s OK. * Anyone there? Anyone... * I got her. I got her! * IAIN Was anyone else in there? You listening to me? Anyone else in there? JEZ * No! I shouted. Looked everywhere. No. SUN-MI * Leave me alone. I am OK. I want my * daughter. IAIN * Do you speak English? What’s your name? Miss? SUN-MI (in KOREAN) I want my daughter. JEZ What’s she saying? IAIN I don’t know. Let’s get her on O2 and check her Obs, yes, Jez? Episode 45 - SHOOTING SCRIPT 'One' 1. Casualty 31 Episode 45 - JEZ She was unresponsive when I got to her. Sorry, Iain. * IAIN * Late to the party again, boys. * Superman himself here... ... saw her at the window.

  • Mass Casualty Incident (MCI) Response Module 1
Mass Casualty Incident (MCI) Response Module 1 (Hamilton County Fire Chief's Association, 2013) 1 Objectives Purpose: This module will educate staff on mass casualty triage incident response, including how to: • Define mass casualty triage • Determine considerations for adults and pediatrics • Understand the importance of a patient tracking system • Recognize and implement the patient admission/ discharge MCI triage process • Determine how to appropriately handle the deceased in a large-scale MCI • Recognize the range of incidents that may cause MCIs 2 MCI Basics 3 What is an MCI? • A mass casualty incident (MCI) is an incident where the number of patients exceeds the amount of healthcare resources available. • This number varies widely across the country, but is typically greater than 10 patients. 4 Types of MCI Notifications • During a large scale incident such as a mass casualty, it is important to have a mass notification system. Successful mass notification systems will: . Internally: alert staff to activate MCI protocols and prepare for a potential surge of patients . Externally: increase community awareness 5 Assisting in MCI Response Considerations for hospital staff in an MCI: • Some patients may arrive to the hospital without having been assessed/ triaged at the scene • MCI response requires efficiency and coordination • Non-clinical personnel (including hospital volunteers) can assist in moving patients to designated areas based on level of care • Help gather patient information in the emergency treatment area • Staff should review patients in clinical assignment for any potential discharges/ transfers to make room for potential MCI admissions, a process known as “surge discharge” (Chung S, 2019) 6 Triage Basics Definition of MCI Triage Triage means “to sort.” Triage in an MCI is the assignment of resources based on the initial patient assessment and consideration of available resources.

  • I Am Writing to You to Express My Deep Concern About the Recent
I am writing to you to express my deep concern about the recent news from the BBC that they have applied to OFCOM to ask if they can reduce news output on the CBBC channel – by axing the main afternoon edition of the much loved Newsround. I am appalled that the BBC would even think this is a good decision. As far as i understand, they say that less children are watching Newsround and more are getting their news online, from the Newsround website and other sources. I cannot understand this reasoning. This is same reasoning they applied to the removal of BBC3 as linear service – and we all know that the end result of that was the BBC reached even less young people. To say more children are watching things online is a cop out. Why are children spending more time watching online? The answer is because they are poorly catered for by linear services. Don’t children spend enough time online as it is? Do we really want to encourage them to spend more time online? Newsround has been a fixture in the CBBC schedule for over 40 years. Surely, this is what the BBC should be spending their money on – a daily news programme for children aged 7- 15. CBBC broadcasts for 14 hours a day- is it really too much to ask that they provide 7 minutes of news in the afternoon / evening? The CBBC channel narrowed its focus to 7-12 year olds in 2013. Some of the continuity presenters can hardly string a legible sentence together and others would be more suited to Cbeebies.

The Future of Children’s TV 1. Introduction Page 2 2. What’s the purpose of Children’s TV? Page 2 3. BBC Children’s Television Failings Page 2 4. Recommendations Page 4 5. Conclusion Page 4 Appendix A Page 5 Appendix B Page 7 Appendix C Page 8 Appendix D Page 10 1 1. Introduction In response to the OFCOM consultation of 3rd October 2007 Children's TV is in crisis. But why does anyone care? Why would it matter if children's television disappeared completely? Is this just about losing jobs in an industry, or is it an issue of providing an important and worthwhile service to young people and therefore to society as a whole? 2. What’s the purpose of Children’s TV? There is a children's TV crisis, and although the reason is due in part to advertising restrictions that doesn't in any way excuse the BBC's shoddy treatment of children detailed in this submission to OFCOM. Jocelyn Hay, from the Voice of the Listener and Viewer, told BBC News 24 on 15 May 2007 that children need programmes made in this country, and pointed out that they "want to be entertained but they also have a right to information, to educational programmes, live programmes, programmes that reflect their own rich heritage of speech, language, culture and their own environment and locations which help them to grow up to be informed citizens of this country, not of some mid-Atlantic fantasy world." Mrs Hay's statement goes to the heart of why it is so important that this country has our own good quality children's programmes.
