LIABILITY WAIVER

The risk of injury from the activity and equipment involved in the scenario and search portion of training given by TFLW/The FRONTLINE WARRIOR, LLC is significant, including the potential for permanent disability and death, and while particular protective equipment and personal discipline will minimize this risk, the risk of serious injury does exist;

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE of those persons released from liability below, and assume full responsibility for my participation; and, I, for myself and on behalf of my heirs and HEREBY RELEASE AND HOLD HARMLESS the TFLW/The FRONTLINE WARRIOR, LLC, the owners and lessors of premises used to conduct the training, their officers, officials, agents, and/or employees (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, except that which is the result of gross negligence and/or wanton misconduct.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTANDING ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

My signature is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law. I am 18 year of age or older and mentally competent to enter into this waiver.

The FRONTLINE WARRIOR, LLC/TFLW and its instructors do NOT teach any biased-based policing, including and form of racial profiling. Racial profiling does not work and is not an approved law enforcement technique. TFLW supports legislation outlawing racial profiling and unethical practices.
