2.Submit (1) original and (8) copies in the format prepared by the Housing & Community Development Division. Attachments specified on page 9 should only be submitted with original proposal application.

3.Number all pages, including attachments.

  1. Application forms/formats from prior application funding cycles are prohibited.

5.Original Application - do not use staples binders, perforated or bind.

6. Application copies - clip, or use rubber bands.

7.Every page must be on standard size paper.

8. Answer each section In the Order provided.

9.Each Entity must submit a separate application for each proposed activity.

10. All applicants must provide certifications, corporate documents and project financial commitment letters to support budget summaries (If applicable). Current project commitment letters must not expire prior to award of funds.

11.Eligibility files will be maintained on current and prospective applicants for funds. The file will contain pertinent information which verifies that the body is legally constituted under the laws of the State of Florida; where applicable, substantiate legal and representative make-up of officials or governing bodies; document actions and efforts to effect Citizen Participation (CP) among persons to be affected by projects, and verification that business is conducted in an open manner accessible to affected citizens.

13.Applications that do not meet the Low/Mod Income (LMI)(80 percent of area median income (AMI) requirements will not be considered.

14.Applications that do not provide documentation that the project deadline can be met will not be considered.

FATAL FLAW CHECKLIST

Failure to comply with or include any of the Required Items listed below that apply to the Applicant Agency’s status (Non-Profit, For-Profit, or Unit of Government) will result in a Fatal Flaw and removal of the Proposal from further consideration of approval.

APPLICATION –2005 Community Development Block Grant Program (CDBG) Disaster Recovery Initiative (DRI) grant/2005 CDBG SUPPLEMENTAL DRI/2008 CDBG Disaster Recovery grant (affordable rental housing requirement)

All applicants structuring housing projects are encouraged to consult with staff prior to the application submission deadline. Applicants requesting funds for construction or rehabilitation must have construction plans and feasible estimates prior to submission of application if applicable.

The 2005 Community Development Block Grant Program (CDBG) Disaster Recovery Initiative (DRI) grant funding was made available to Florida by the United States Department of Housing and Urban Development, pursuant to the Military Construction Appropriations and Emergency Hurricane Supplemental Appropriations Act. CDBG DRI provides funds to serve low and very low income households or that were directly affected by Hurricane Wilma.

Program funding areas: Housing projects ONLY (Single Family/Multi-family).

  1. 2005 CDBG DRIGrantfunds can only be used for Housing Rehabilitation/repair or Replacement Housingof damaged homes directly related to 2005 hurricane Wilma.($894,000.00)
  • Proof of damage must be provided (insurance claim, FEMA, red-tagged etc...)
  • No duplication of benefits (will not duplicate what an agency received funding to repair). No exceptions unless the organization applied the funds to the repairs and the funds were not enough to cover all the damages, in which case,it must be demonstrated and all documentation must be attached. Each case will be reviewed and approved by DCA and Broward County before award consideration.
  • Projects must be LMI (If multi-family at least 80% of units must be LMI)

LMI is defined (at or below 80 percent of area median income (AMI).

Documentation must include, but is not limited to:

  1. Photographs (before);
  2. Reports: FEMA, Damage Assessment Report, insurance, police, county, etc.;
  3. Newspaper articles;
  4. Television news stories (provide DVD);
  5. Code Enforcement or Public Works Department Staff; certification of local

housing authority

  1. 2005 CDBG SUPPLEMENTAL DRI Grant: Housing ($2,000,000.00 for Multi-family)

2005 CDBG Supplemental DRI can be used torehabilitate harden/mitigate the home to protectit against future storms. Single family housing repair, rehabilitation, or replace the damaged home (If repairing/replacing damaged home must provide proof of damage and all the requirements of the 2005 CDBG DRI grant apply)andhardening. Homes that are assisted must meet the appropriate building code standards, HUD’s Section 8 Housing Quality Standards (HQS).

  • Projects must be LMI (If multi-family at least 80% of units must be LMI)

LMI is defined (at or below 80 percent of area median income (AMI).

2008 Community Development Block Grant (CDBG) disaster relief funding was made available to Florida by the United States Department of Housing and Urban Development, under Title IV of the Robert T. Stafford Disaster Relief and Emergency Assistance Act (42 U.S.C. 5121 et. seq.) for federally declared natural disasters that occurred during 2008

  1. 2008 CDBG Disaster Recovery Grant: Housing ($104,327.00)

Funds may be used for affordable rental housing activities. These activities may include housing rehabilitation, replacement, temporary or permanent relocation, Hardening/Mitigation Rehabilitation, buyout of housing units in the floodplain, etc. If applying for activities that include housing rehabilitation, replacement, temporary or permanent relocation, proof of damage is required.

  • Projects must be at least LMI (If multi-family at least 80% of units must be LMI)

LMI is defined (at or below 80 percent of area median income (AMI).

Examples of Hardening/Mitigation Eligible Activities:

1. Improving the strength of the roof deck attachment. For example, if the roof consists of shingles

nailed to plywood sheets, the inspection may reveal that the plywood sheets are not adequately

nailed to the roof trusses, and that additional nails and/or longer nails need to be added to prevent

the plywood from being blown off in a hurricane.

2. Creating a secondary water barrier to prevent water intrusion. For example, using strips of “peel

and stick-on” material that cover the joints between the plywood sheets can reduce leakage if a

hurricane blows off the roof shingles.

3. Improving the survivability of the roof covering. For example, upgrading to thicker and stronger

hurricane-resistant roof shingles, attached with properly sized and properly applied roofing nails,

to reduce the susceptibility of the roof shingles blowing off in a hurricane.

4. Bracing gable-ends in the roof framing. This is usually done inside the attic to decrease chances

that the roof will collapse under hurricane wind loads.

5. Reinforcing roof-to-wall connections. For example, installing metal tie-down straps that attach

roof rafters to wall studs to decrease chances that all or a portion of the roof will simply lift the

house during a hurricane.

6. Upgrading exterior wall opening protections. For example, installing hurricane-rated window

shutters.

7. Upgrading exterior doors and windows. For example, replacing a standard garage door with a

hurricane-rated garage door.
