We may have to charge you, or a third party, a fee.

CERTIFICATES WITHOUT EXAMINATION

Simple Certificate of fact / £17
Freedom of Infection Certificate / £30
Private Sick Note/ Accident or sickness incapacity or insurance certificates / £30
School Fees/ holiday insurance certificates / £40
To Whom It May Concern Letter (dependent on length and content) / £20-£40
Certification of PMI claims or ‘pre-treatment’ paperwork / £40
Health club written report to certify patient fit for exercise / £40

MEDICAL EXAMINATIONS AND REPORTS

Work of this nature will be charged depending on which of the following scenarios are requested of GP:-

• Extract from Medical Records only

/ £50

• Report on Proforma- usually with examination (for example used for all driving reports)

/ £85

• Written Report (with exam) to provide detailed opinion and statement on condition of patient

The above work could include accident or sickness Insurance claims, Employment requests,
Private Medical Insurance paperwork, Fitness for Education or
Suitability assessments for pharmaceutical trials.
All fees will be applied on the basis above.

TRAVEL SERVICES

Fit to Travel Certificate / £17
Fitness to Travel Examination and report / £85
Vaccination Certificate / £30
Hep B and Rabies vaccinations (Course of 3 vaccinations plus private prescription) / £45

• Please note- in addition patient will be required topurchase vaccine from pharmacy

Private prescription for Malaria / £15

MISCELLANEOUS

Search fee (Access to Records under Data Protection Act)- If whole record is held electronically / £10
Search fee (Access to Records under Data Protection Act)- If record is held electronically and paper copies / £50 max
Photocopies of Medical Records / £0.35/ sheet
