6) BRIEF RESUME OF INTENDED WORK:

6.1) NEED FOR THE STUDY:

Pre operative anxiety plays a major role in patient morbidity during surgery. The actual surgical and anesthetic management starts with pre operative psychological preparation of the patient. The most important objective is to relieve anxiety before surgery.

Acharya Sushruta, considered as Father of Surgery, explained in detail about poorva karma, pradhan karma and pashcat karma on surgically posted patient1. Anxiety causes instability and behavior variation before surgery. Our Acharyas mentioned many methods and drugs, like Sarpagandha2 and jatamansi3, 4 to overcome anxiety.

Previous works proved that Sarpagandha and jatamansi are anxiolytic5, 6. As a part of continuation of previous research, to compare the therapeutic efficacy of both these drugs in the pre operative anxiety, the dissertation topic entitled “A comparative clinical study on Sarpagandha vati and Jatamansi kwatha in the management pre operative anxiety” is selected.

6.2) REVIEW OF LITERATURE:

This section will contain:-

Ø  Description of manas and manoroga its nidana, lakshana, chikitsa, upadrava in Ayurveda literature.7

Ø  Description of anxiety and its treatment in Modern science.8

Ø  Detailed study of sarpagandha vati and jatamansi kwath.

6.3) OBJECTIVES OF STUDY:

Ø  To study the literature about anxiety in detail

Ø  Comparative study to evaluate the effect of sarpagandha vati and jatamansi kwatha in the management of pre operative anxiety.

7) MATERIAL AND METHODS:

7.1) SOURCE OF DATA:

Among patients posted for surgery, twenty are selected from Dhanvantari ward, S.D.M. Ayurveda Hospital; Udupi

7.2) METHOD OF COLLECTION OF DATA:

Data will be collected based on a detailed proforma.

Minimum of twenty patients suffering from pre operative anxiety in the age group of 20-60years will be selected and subjected to clinical trials.

7.3) DESIGN OF STUDY

This is a single blind comparative clinical study with pre test and post test design. Twenty patients will be selected. And made into two groups of ten patients each

Group A 10 patients Jatamansi kwatha 40ml orally

Group B 10 patients Sarpagandha vati 1gm orally

DURATION OF TREATMENT

Sarpagandha vati and jatamansi kwatha are given on the previous day of surgery in the evening at around 7pm and also in the morning on the day of surgery.

OBSERVATION PERIOD AND FOLLOW UP

The observations are made at different intervals of time, .i.e. ½ hour before and after administration of drug, 1hour after administration of drug, 1 ½ hour after administration of drug, on the day of surgery and after surgery. During surgery observations are not carried out.

INCLUSION CRITERIA:

Ø  Patients aged between 20-60years.

Ø  Patients of either sex are taken.

Ø  Patients undergoing surgical procedure under spinal anesthesia like

- Herniorraphy

-  Appendicectomy

-  Hysterectomy

-  Haemorrhoidectomy

EXCLUSION CRITERIA:

Ø  Patient undergone any surgical procedure before.

Ø  Patient having history of hypertension, diabetes mellitus and other systemic disorders.

Ø  Patient on anti depressant treatment.

ASSESSMENT CRITERIA:

Ø  The response will be assessed on subjective and objective parameters.

SUBJECTIVE PARAMETERS:

ACCORDING TO DSM-4

Ø  Palpitation or accelerated heart rate

Ø  Sweating

Ø  Trembling or shaking

Ø  Breathlessness

Ø  Chocking

Ø  Chest pain or discomfort

Ø  Nausea

Ø  Dizziness

Ø  Fear of dying

Ø  Chills or hot flushes

Ø  Paresthesias

Ø  Fear of losing control

Ø  depersonalisation

OBJECTIVE PARAMETERS:

Ø  Blood pressure

Ø  Pulse rate

INVESTIGATION

Ø  Hb%

- TC

- DC

- ESR

Ø  Blood sugar and urine sugar

Ø  Any other necessary investigations.

7.3) Does this study require any investigations or interventions to be conducted on patients or other humans or animals? If so please describe briefly?

Yes, the study will be conducted on humans and not on animals.

7.4) Has the ethical clearance been obtained from your institution in the case of 7.3?

YES, ethical clearance is obtained through the ethical committee formulated in this institution.
